Abstrak
Latar Belakang: Pada 2023, Kota Bogor mencatat 360 kasus positif naik 9 kali lipat dibanding tahun sebelumnya dengan incidence rate tertinggi kedua di Jawa Barat dimana penyebaran hampir merata di seluruh kelurahan. Penelitian ini diperlukan untuk menganalisis pola spasial dan faktor risiko campak pada balita di Kota Bogor sebagai dasar ilmiah untuk intervensi yang lebih efektif. Metode: Penelitian ini merupakan studi ekologis dengan analisis spasial dengan unit analisis adalah kelurahan. Data dianalisis menggunakan pendekatan deskriptif, eksploratori, inferensial, dan prediktif dengan model regresi spasial untuk mengidentifikasi faktor risiko campak pada balita. Hasil: Analisis spasial kasus positif campak balita menunjukkan bahwa proporsi usia kasus, PHBS rumah tangga sehat, dan cakupan vitamin A memiliki pola spasial mengelompok signifikan (p < 0,05) berdasarkan Moran’s Index. Hasil LISA mengidentifikasi beberapa kelurahan dalam kuadran high-high, seperti Cikaret dan Mulyaharja. Hasil uji SAR menunjukkan terdapat efek spasial signifikan (ρ = 0,142; p = 0,05). Secara parsial, proporsi usia kasus dan angka pendidikan SD–SMP berpengaruh signifikan (p < 0,001), sementara cakupan vitamin A menunjukkan efek protektif secara marginal signifikan (p = 0,05). Model ini menunjukkan bahwa distribusi campak dipengaruhi oleh faktor biologis, pendidikan, dan spasial wilayah. Kesimpulan: Kasus campak balita menunjukkan pola spasial mengelompok signifikan dengan faktor usia, pendidikan, dan cakupan vitamin A sebagai determinan utama. Diperlukan penguatan analisis spasial secara rutin dan pemantauan wilayah yang terindikasi hotspot.

In 2023, Bogor City recorded 360 positive measles cases, a ninefold increase compared to the previous year, with the second highest incidence rate in West Java and nearly uniform distribution across all sub-districts. This study aims to analyze the spatial patterns and risk factors of measles among children under five in Bogor City as a scientific basis for more effective interventions. Methods: This ecological study used spatial analysis with sub-districts as the unit of analysis. Data were analyzed using descriptive, exploratory, inferential, and predictive approaches, including spatial regression models to identify measles risk factors in children under five. Results: Spatial analysis of positive measles cases showed that the proportion of cases by age, healthy household behavior (PHBS), and vitamin A coverage exhibited significant spatial clustering (p < 0.05) based on Moran’s Index. LISA results identified several sub-districts in the high-high quadrant, such as Cikaret and Mulyaharja. SAR tests indicated a significant spatial effect (ρ = 0.142; p = 0.05). Partially, the proportion of cases by age and primary to junior high school education level had significant effects (p < 0.001), while vitamin A coverage showed a marginally significant protective effect (p = 0.05). This model suggests that measles distribution is influenced by biological, educational, and spatial factors. Conclusion: Measles cases in children under five exhibit significant spatial clustering, with age, education, and vitamin A coverage as key determinants. Routine spatial analysis and monitoring of identified hotspot areas are recommended.
