Abstrak
Latar belakang: Hemofilia sebagai penyakit langka menyumbang pembiayaan katastropik tertinggi JKN, mencapai Rp1,2 triliun pada 2023. Rendahnya deteksi dan penggunaan obat memperburuk ketimpangan alokasi anggaran, sehingga menjadi tantangan besar dalam keberlanjutan pembiayaan kesehatan nasional. Tujuan: Menganalisis Cost of Illness (COI) peserta JKN dari perspektif sosial, dengan fokus pada sudut pandang BPJS sebagai pembayar dan pasien yang mengalami kehilangan produktivitas Metode: Penelitian ini menggunakan desain studi observasional dengan pendekatan cross-sectional dengan analisis univariat dan bivariat. Sampel penelitian adalah peserta JKN dengan diagnosis Hemofilia (D66, D67, D68.0, dan D68.1), menggunakan data sekunder dari BPJS Kesehatan dan data upah rata-rata per jam berdasarkan provinsi dari BPS. Hasil: Pada tahun 2022-2023, rata-rata COI per peserta berdasarkan jumlah sampel adalah Rp728.832.232 dengan total Rp34.255.114.924. Berdasarkan jumlah populasi, total COI mencapai Rp3.019.551.937.176 Kesimpulan: Variabel jenis kelamin, usia, status perkawinan, segmentasi kepesertaan, kelas rawat, jenis pelayanan, lama hari rawat, tipe FKRTL yang sering dikunjungi, tingkat keparahan, dan penggunaan special drugs berpengaruh signifikan secara statistik terhadap COI
Background: Hemophilia, as a rare disease, contributes to the highest catastrophic health expenditure in the National Health Insurance (JKN) system, reaching IDR 1.2 trillion in 2023. Low detection rates and limited drug usage exacerbate the disparity in budget allocation, posing a significant challenge to the sustainability of national healthcare financing. Objective: This study aims to analyze the Cost of Illness (COI) for JKN participants from a societal perspective, focusing on the BPJS as the payer and patients experiencing productivity loss. Methods: This observational study uses a cross-sectional design, utilizing both univariate and bivariate analyses. The sample consists of JKN participants diagnosed with Hemophilia (D66, D67, D68.0, and D68.1), using secondary data from BPJS Kesehatan and average hourly wage data by province from BPS. Results: In 2022-2023, the average COI per participant based on the sample size was IDR 728,832,232, totalling IDR 34,255,114,924. Based on the total population, the COI reached IDR 3,019,551,937,176. Conclusion: Variables such as gender, age, marital status, membership segmentation, class of care, type of service, length of stay, type of healthcare facility frequently visited, severity level, and use of special drugs have a statistically significant impact on COI.
