Abstrak
Nyeri punggung bawah (low back pain/LBP) merupakan salah satu penyebab utama disabilitas global yang berdampak terhadap penurunan kualitas hidup secara fisik, psikologis, dan sosial. Hidroterapi sebagai bagian dari intervensi rehabilitatif dinilai memiliki potensi untuk memperbaiki kondisi fungsional pasien LBP melalui efek terapeutik media air. Tujuan: Penelitian ini bertujuan untuk mengevaluasi pengaruh hidroterapi terhadap perubahan kualitas hidup pasien LBP serta mengidentifikasi hubungan antara faktor individu dan sosial-ekonomi dengan perubahan tersebut. Metode: Penelitian ini menggunakan desain pre-post test tanpa kelompok kontrol. Sebanyak 40 pasien LBP di Instalasi Rehabilitasi Medik RSUP Fatmawati yang menjalani minimal empat sesi hidroterapi diikutsertakan. Kualitas hidup diukur menggunakan instrumen EQ-5D-5L dan EQ-VAS, dengan nilai utilitas diturunkan dari value set Indonesia (Hybrid C-TTO Model). Nilai utilitas dihitung berdasarkan bobot disutility pada masing-masing dimensi EQ-5D-5L yang dikurangkan dari nilai sempurna 1,000. Hasil: Hasil menunjukkan adanya peningkatan signifikan pada nilai utilitas EQ-5D-5L (sebelum: 0,64 ± 0,15; sesudah: 0,82 ± 0,12; p < 0,001) dan skor EQ-VAS (sebelum: 65,3 ± 10,2; sesudah: 78,7 ± 9,4; p < 0,001). Terdapat korelasi positif antara perubahan EQ-5D-5L dan EQ-VAS (r = 0,534; p < 0,001). Uji bivariat menunjukkan bahwa faktor individu (usia, jenis kelamin, IMT, aktivitas fisik, merokok, komorbiditas, waktu tempuh) dan faktor sosial-ekonomi (pendapatan, pendidikan, status pernikahan, kepemilikan asuransi) tidak berhubungan signifikan dengan perubahan kualitas hidup. Kesimpulan: hidroterapi terbukti efektif meningkatkan kualitas hidup pasien LBP secara signifikan, terlepas dari perbedaan karakteristik individu dan sosial-ekonomi.

Low back pain (LBP) is a leading cause of global disability that adversely affects physical, psychological, and social quality of life. Hydrotherapy, as part of rehabilitative interventions, is considered to have potential therapeutic effects in improving the functional status of LBP patients through the properties of the aquatic environment. Objective: This study aimed to evaluate the effect of hydrotherapy on quality of life changes in LBP patients and to identify the association between individual and socioeconomic factors with these changes. Methods: A pre-post test design without a control group was employed. 40 LBP patients at the Medical Rehabilitation Unit of RSUP Fatmawati who completed at least four hydrotherapy sessions were included. Quality of life was measured using the EQ-5D-5L and EQ-VAS instruments, with utility values derived from the Indonesian value set (Hybrid C-TTO Model). Utility scores were calculated based on disutility weights assigned to each EQ-5D-5L dimension and subtracted from a perfect health value of 1.000. Results: There was a significant increase in EQ-5D-5L utility scores (pre: 0.64 ± 0.15; post: 0.82 ± 0.12; p < 0.001) and EQ-VAS scores (pre: 65.3 ± 10.2; post: 78.7 ± 9.4; p < 0.001). A positive correlation was found between changes in EQ-5D-5L and EQ-VAS scores (r = 0.534; p < 0.001). Bivariate analysis showed no significant association between individual factors (age, sex, BMI, physical activity, smoking status, comorbidities, travel time) and socioeconomic factors (income, education, marital status, insurance ownership) with quality of life changes. Conclusion: Hydrotherapy is effective in significantly improving quality of life in LBP patients regardless of individual and socioeconomic characteristics.
