Abstrak
Stroke merupakan salah satu penyakit dengan prevalensi yang tinggi di Indonesia, khususnya Jawa Tengah. Pada tahun 2023, BPJS Kesehatan mengalokasikan anggaran sebesar Rp5,2 triliun untuk membiayai 3,4 juta kasus stroke dengan biaya rata-rata Rp1,53 miliar per kasus. Penelitian ini bertujuan untuk menganalisis total biaya klaim stroke pada peserta JKN di FKRTL Provinsi Jawa Tengah Tahun 2023 serta hubungan faktor-faktor yang mempengaruhinya. Penelitian menggunakan metode kuantitatif dengan data sekunder dari Data Sampel BPJS Kesehatan 2024 serta desain studi cross-sectional. Penelitian memberikan gambaran hubungan total biaya klaim stroke melalui analisis univariat dan analisis bivariat yang menggunakan uji Mann Whitney dan Kruskal-Wallis. Hasil menunjukkan bahwa total biaya klaim stroke pada peserta JKN di FKRTL Provinsi Jawa Tengah tahun 2023 adalah Rp5.592.314.702 (5,6 miliar) dengan rincian total biaya klaim stroke pada pelayanan Rawat Jalan Tingkat Lanjut sebesar Rp95.315.500 (95,3 juta) dan Rawat Inap Tingkat Lanjut sebesar Rp5.496.999.202 (5,5 miliar), dengan faktor yang berhubungan secara signifikan adalah usia, wilayah tempat tinggal, segmentasi kepesertaan, hak kelas rawat, kepemilikan FKRTL, jenis stroke, tingkat keparahan kasus, lama hari rawat, dan jenis pelayanan FKRTL. Penelitian ini menyimpulkan bahwa Program PROLANIS dan peningkatan pengawasan terhadap klaim diperlukan untuk meningkatkan kesehatan dan kualitas hidup peserta JKN serta memastikan efisiensi dan keakuratan biaya.
Stroke is one of the most prevalent diseases in Indonesia, particularly in Central Java. In 2023, BPJS Kesehatan allocated a budget of Rp5.2 trillion to cover 3.4 million stroke cases, with an average cost of Rp1.53 billion per case. This study aims to analyse the total stroke claim costs for JKN participants at FKRTL in Central Java Province in 2023, as well as the factors influencing them. The study employs a quantitative method using secondary data from the BPJS Health 2024 Sample Data and a cross-sectional study design. The study provides an overview of the relationship between total stroke claim costs through univariate and bivariate analyses using the Mann Whitney and Kruskal-Wallis tests. The results show that the total stroke claim costs for JKN participants at FKRTL in Central Java Province in 2023 were IDR 5,592,314,702 (5.6 billion), with the breakdown of total stroke claim costs for advanced outpatient services amounting to IDR 95,315, 500 million (95.3 million) and advanced inpatient care at Rp5,496,999,202 (5.5 billion), with significantly associated factors including age, residential area, participant segmentation, class of care entitlement, ownership of FKRTL, type of stroke, severity of the case, length of hospital stay, and type of FKRTL service. This study concluded that The PROLANIS programme and increased monitoring of claims are necessary to improve the health and quality of life of JKN participants and to ensure cost efficiency and accuracy.
