Abstrak
Angka Kematian Ibu di Indonesia masih tinggi (189/100.000 KH) dan KB Pasca Persalinan (KBPP) merupakan salah satu intervensi yang efektif untuk menurunkannya. Namun, cakupan KBPP nasional baru mencapai 48,9% dan determinan penggunaannya pada tingkat nasional belum banyak diteliti. Penelitian ini bertujuan untuk mengetahui determinan penggunaan KBPP di Indonesia. Penelitian ini merupakan analisis lanjut data Survei Kesehatan Indonesia (SKI) 2023 dengan desain cross-sectional. Sampel penelitian sebanyak 70.387 wanita usia subur (15–49 tahun) yang memiliki riwayat persalinan dalam lima tahun terakhir, diambil dengan teknik multistage cluster sampling sesuai prosedur SKI 2023. Analisis data menggunakan regresi Poisson dengan robust variance dan pendekatan complex sample design untuk menghasilkan adjusted Prevalence Ratio (aPR). Hasil penelitian menunjukkan prevalensi KBPP sebesar 28,9%, dengan metode dominan suntikan 3 bulan (46,1%). Tempat melahirkan, paritas, pemanfaatan ANC, dan kepemilikan jaminan kesehatan berhubungan dengan KBPP. Tempat melahirkan menjadi determinan paling dominan; ibu yang melahirkan di fasilitas kesehatan memiliki prevalensi penggunaan KBPP 1,44 kali lebih tinggi dibanding ibu yang melahirkan bukan di fasilitas kesehatan setelah dikontrol oleh paritas, pemanfaatan ANC dan kepemilikan jaminan kesehatan (aPR=1,436; 95% CI: 1,290–1,600). Disarankan kepada Kementerian Kesehatan untuk menerapkan protokol nasional konseling KBPP sebelum pulang dan memperkuat kapasitas tenaga kesehatan; kepada BKKBN untuk memperluas sasaran program ke ibu paritas rendah dan memastikan ketersediaan MKJP; kepada BPJS Kesehatan untuk memperluas cakupan pembiayaan KBPP melalui JKN; serta kepada fasilitas pelayanan kesehatan untuk mengoptimalkan momentum persalinan sebagai titik kontak utama KBPP.
The Maternal Mortality Rate in Indonesia remains high (189/100,000 live births), and Postpartum Family Planning (PPFP) is one of the effective interventions to reduce it. However, the national PPFP coverage has only reached 48.9%, and its determinants at the national level have not been widely studied. This study aimed to identify the determinants of PPFP use in Indonesia. This study is a further analysis of the 2023 Indonesia Health Survey (SKI) data using a cross-sectional design. The study sample consisted of 70,387 women of reproductive age (15–49 years) who had a history of childbirth within the past five years, selected through multistage cluster sampling in accordance with the 2023 SKI procedures. Data were analyzed using Poisson regression with robust variance and a complex sample design approach to produce adjusted Prevalence Ratios (aPR). The results showed that the prevalence of PPFP use was 28.9%, with the 3-month injectable contraceptive as the dominant method (46.1%). Place of delivery, parity, ANC utilization, and health insurance ownership were associated with PPFP use. Place of delivery was the most dominant determinant; mothers who delivered at a health facility had a 1.44 times higher prevalence of PPFP use compared to those who delivered outside a health facility, after controlling for parity, ANC utilization, and health insurance ownership (aPR=1.436; 95% CI: 1.290–1.600). It is recommended that the Ministry of Health implement a national protocol for pre-discharge PPFP counseling and strengthen the capacity of healthcare workers; that BKKBN expand the program target to mothers with low parity and ensure the availability of Long-Acting Reversible Contraceptives (LARCs); that BPJS Kesehatan expand PPFP financing coverage through the National Health Insurance (JKN) scheme; and that healthcare facilities optimize the momentum of childbirth as the primary contact point for PPFP services
