Abstrak
TB (Tuberkulosis) adalah penyebab utama kematian pada orang dengan HIV (ODHIV). Terapi pencegahan TB (TPT) dapat mencegah infeksi TB menjadi penyakit TB aktif. TPT 3HP adalah rejimen yang direkomendasikan karena durasi singkat yang dapat meningkatkan completion therapy. TPT menimbulkan efek samping dan salah satu yang sering terjadi adalah hepatotoksisitas yang dapat diukur dengan pemeriksaan biomarker fungsi hati. Cakupan program TPT pada ODHIV sampai saat ini rendah dan belum diketahui hambatannya. Penelitian ini bertujuan untuk untuk menganalisis tren perubahan biomarker fungsi hati meliputi ALT, ALP, bilirubin total, albumin, faktor R secara longitudinal dan mengevaluasi implementasi program terapi pencegahan tuberkulosis (TPT) 3HP pada orang dengan HIV. Penelitian ini menggunakan pendekatan embedded mix method dengan penelitian kuantitatif  berupa studi panel longitudinal pada 189 ODHIV yang terbagi 2 kelompok yaitu penerima TPT 3HP dan non-TPT untuk uji fungsi hati meliputi ALT, ALP, Bilirubin total, albumin dan faktor R dengan analisis statistik model GEE (Generalized Estimating Equation) dengan working correlation auto regressive, distribusi gamma link function log menggunakan SPSS 24. Penelitian kualitatif menggunakan metode evaluasi CIPP dengan indepth-interview kepada 28 informan dengan menggunakan analisis tematik. Penelitian ini dilaksanakan di 5 puskesmas dan 1 rumah sakit di Kabupaten Gowa dan Kota Makassar, Provinsi Sulawesi Selatan. Hasil penelitian menunjukkan bahwa terdapat efek interaksi yang signifikan pada  kelompok TPT 3HP dengan waktu terhadap perubahan peningkatan rasio mean ALT 1,16 dan ALP 1,20 pada T1 dan penurunan ALT 0,787 dan ALP 0,765 pada T2. Sedangkan pada dua biomarker lainnya menunjukkan penurunan rasio mean yang konsisten pada T1 dan T2 yaitu bilirubin total 0,848 dan 0,76, albumin 0,924 dan 0,956. Hanya faktor R yang tidak menunjukkan efek interaksi yang signifikan. Terdapat 18 jenis efek samping obat (ESO) dari 47 (49,5%) bulan pertama, kedua  (20% ) dan ketiga (7,4%). Completion therapy TPT 3HP 97,9% dengan faktor pendukung kepercayaan terhadap PJ HIV dengan pendekatan interpersonal, terdapat role model, opinion leader subkelompok, intervensi perilaku sederhana (repacking 3HP), efek samping ringan yang self-limiting, dan punya riwayat TB. Kendala utama yang tersisa adalah mutasi SDM, logistik, stigma sosial, variasi pengetahuan nakes, dan ketimpangan akses informasi antar ODHIV. Kesimpulan: Pemberian TPT 3HP dapat merubah tren biomarker ALT, ALP, Bilirubin total dan albumin, dengan kadar yang masih berada dalam rentang nilai normal. Keluhan ESO ringan dan dapat ditoleransi dengan baik oleh ODHIV sehingga completion therapy tinggi.

TB (tuberculosis) is the leading cause of death among people living with HIV (PLHIV). Tuberculosis preventive therapy (TPT) can prevent TB infection from progressing to active TB disease. The 3HP TPT regimen is recommended because its short duration can improve treatment completion rates. TPT causes side effects, and one of the most common is hepatotoxicity, which can be measured through liver function biomarker tests. Coverage of the TPT program among PLHIV remains low, and the barriers to its implementation are not yet known. This study aims to analyze trends in changes in liver function biomarkers—including ALT, ALP, total bilirubin, albumin, and R factor—over time and to evaluate the implementation of the 3HP tuberculosis preventive therapy (TPT) program among people living with HIV. This study employed an embedded mixed-methods approach, with a quantitative component consisting of a longitudinal panel study of 189 people living with HIV divided into two groups: 3HP TPT recipients and non-TPT recipients. Liver function tests—including ALT, ALP, total bilirubin, albumin, and R factor, using statistical analysis with the GEE (Generalized Estimating Equations) model featuring an autoregressive working correlation and a log-gamma link function in SPSS 24. The qualitative study employed the CIPP evaluation method through in-depth interviews with 28 informants, utilizing thematic analysis. This study was conducted at 5 community health centers and 1 hospital in Gowa Regency and Makassar City, South Sulawesi Province. The results of the study showed that there was a significant interaction effect in the TPT 3HP group with time regarding changes in the mean ALT ratio (1.16) and ALP ratio (1.20) at T1, and decreases in the mean ALT ratio (0.787) and ALP ratio (0.765) at T2. Meanwhile, the other two biomarkers showed consistent decreases in mean ratios at T1 and T2, namely total bilirubin at 0.848 and 0.76, and albumin at 0.924 and 0.956. Only the R factor did not show a significant interaction effect. There were 18 types of drug-related adverse events (DRAs) occurring in 47 (49.5%) of the first, second (20%), and third (7.4%) months. The completion rate for 3HP antiretroviral therapy (ART) was 97.9%, supported by factors such as trust in HIV care providers through an interpersonal approach, the presence of role models and subgroup opinion leaders, simple behavioral interventions (3HP repackaging), mild self-limiting side effects, and a history of TB. The main remaining challenges include staff turnover, logistics, social stigma, variations in healthcare workers’ knowledge, and disparities in access to information among people living with HIV (PLHIV). Conclusion: Administration of TPT 3HP can alter trends in the biomarkers ALT, ALP, total bilirubin, and albumin, with levels remaining within the normal range. Adverse drug reactions were mild and well-tolerated by PLHIV, resulting in high treatment completion rates.
