Abstrak
Penelitian ini bertujuan menganalisis efisiensi belanja program penanggulangan tuberkulosis (TB) pada puskesmas di Provinsi DKI Jakarta tahun 2023–2024. Analisis dilakukan pada 44 puskesmas dan dibandingkan dengan 42 puskesmas wilayah daratan setelah Puskesmas di Kepulauan Seribu dilakukan eksklusi. Metode yang digunakan adalah Data Envelopment Analysis (DEA) untuk mengukur efisiensi teknis, Malmquist Productivity Index (MPI) untuk menilai perubahan produktivitas, dan regresi Tobit untuk menganalisis faktor-faktor yang memengaruhi efisiensi. Hasil penelitian menunjukkan bahwa efisiensi belanja program TB bervariasi antar puskesmas. Pada analisis 44 puskesmas, efisiensi teknis cenderung lebih rendah dan beragam dibandingkan analisis 42 puskesmas wilayah daratan. Setelah Puskesmas di Kepulauan Seribu dikeluarkan, hasil efisiensi menjadi lebih stabil dengan sebaran yang lebih sempit. Analisis MPI menunjukkan produktivitas layanan TB meningkat pada sebagian besar puskesmas, dengan pola yang lebih proporsional pada 42 puskesmas wilayah daratan. Pada analisis 44 puskesmas, Puskesmas Kepulauan Seribu Selatan menunjukkan peningkatan produktivitas tertinggi, sedangkan pada wilayah daratan puskesmas seperti Menteng dan Palmerah juga menunjukkan peningkatan yang tinggi. Hasil regresi Tobit menunjukkan bahwa kepadatan penduduk berpengaruh negatif terhadap efisiensi. Temuan ini menunjukkan bahwa karakteristik wilayah memengaruhi efisiensi program TB. Oleh karena itu, kebijakan alokasi sumber daya dan evaluasi kinerja perlu mempertimbangkan kondisi geografis serta beban layanan masing-masing wilayah.

This study aimed to analyze the efficiency of expenditure on the tuberculosis (TB) control program at community health centers (Puskesmas) in the Special Capital Region of Jakarta Province during 2023–2024. The analysis included 44 community health centers and was subsequently compared with an analysis of 42 mainland community health centers after excluding those located in the Seribu Islands. The study employed Data Envelopment Analysis (DEA) to measure technical efficiency, the Malmquist Productivity Index (MPI) to assess productivity changes over time, and Tobit regression to identify factors influencing efficiency. The findings revealed that the efficiency of TB program expenditure varied considerably across community health centers. In the analysis involving all 44 facilities, technical efficiency was generally lower and more heterogeneous than in the analysis restricted to the 42 mainland facilities. After excluding the Seribu Islands community health centers, the efficiency results became more stable with a narrower distribution. The MPI analysis indicated that TB service productivity improved in most community health centers, with a more proportional pattern observed among the 42 mainland facilities. In the analysis of all 44 community health centers, South Seribu Islands Community Health Center demonstrated the highest productivity improvement, while among the mainland facilities, Menteng and Palmerah Community Health Centers also exhibited substantial productivity gains. The Tobit regression analysis showed that population density had a significant negative effect on efficiency. These findings suggest that regional characteristics influence the efficiency of TB program implementation. Therefore, resource allocation policies and performance evaluations should take into account geographical conditions and the service burden of each region.
