Abstrak
Latar Belakang: Penyakit Ginjal Kronis (PGK) merupakan penyakit progresif dengan beban mortalitas dan pembiayaan yang masif dalam sistem Jaminan Kesehatan Nasional (JKN). Fase prediabetes, yang secara patobiologis masih bersifat dapat kembali, sering kali tidak teridentifikasi hingga terjadinya komplikasi. Tujuan: Menganalisis probabilitas kesintasan bebas PGK serta mengidentifikasi determinan sosiodemografi yang memengaruhinya pada pasien prediabetes. Metode: Penelitian ini menggunakan data sekunder bersumber dari Data Sampel BPJS Kesehatan periode 2019–2024. Analisis probabilitas kesintasan dilakukan menggunakan kurva Kaplan-Meier, sedangkan faktor risiko dianalisis menggunakan regresi Cox Proportional Hazards. Hasil: Selama periode pengamatan, insidensi progresi menuju PGK ditemukan sebesar 4,68%, dengan median waktu kejadian secara langsung belum tercapai (not reached). Meskipun demikian, analisis perjalanan penyakit pada subkohort menunjukkan laju perburukan yang cepat, dengan median waktu transisi dari prediabetes menjadi Diabetes Melitus Tipe 2 (DMT2) selama 503 hari (1,5 tahun). Setelah terdiagnosis DMT2, 25% pasien di antaranya mengalami progresivitas menuju PGK dalam 2.014 hari (5,5 tahun). Secara keseluruhan, analisis kesintasan melaporkan bahwa probabilitas kumulatif bebas PGK pada tahun kelima berada di angka 88,08% (95% CI: 0,8771–0,8843). Berdasarkan pemodelan multivariat, usia saat diagnosis teridentifikasi sebagai prediktor independen yang secara konsisten memengaruhi progresivitas penyakit, dengan setiap penambahan usia 10 tahun meningkatkan risiko kejadian PGK sebesar 22% (aHR = 1,222; 95% CI: 1,003–1,490). Kesimpulan: Probabilitas kesintasan bebas PGK dan DMT2 pada pasien prediabetes tergolong rendah. Oleh karena itu, diperlukan upaya preventif yang komprehensif, mulai dari masyarakat, BPJS Kesehatan, hingga pemangku kebijakan.

Background: Chronic Kidney Disease (CKD) is a progressive condition associated with a massive burden in terms of mortality and healthcare costs on the National Health Insurance (NHI) system. The prediabetes stage, which is pathobiologically reversible, often goes unrecognized until complications arise. Objective: To analyze the probability of CKD-free survival and identify the sociodemographic determinants influencing it in patients with prediabetes. Methods: This study utilized secondary data sourced from the BPJS Health Sample Data for the 2019–2024 period. Survival probability analysis was performed using Kaplan-Meier curves, while risk factors were analyzed using Cox Proportional Hazards regression. Results: During the observation period, the incidence of progression to CKD was 4.68%, with the median time to event not yet reached. However, disease course analysis in a subcohort revealed a rapid rate of progression, with a median time to transition from prediabetes to T2DM of 503 days (1.5 years). After being diagnosed with T2DM, 25% of these patients progressed to CKD within 2,014 days (5.5 years). Overall, survival analysis reported that the cumulative probability of remaining CKD-free at the fifth year was 88.08% (95% CI: 0.8771–0.8843). Based on multivariate modeling, age at diagnosis was identified as an independent predictor that consistently influenced disease progression, with each 10-year increase in age increasing the risk of CKD by 22% (aHR = 1.222; 95% CI: 1.003–1.490). Conclusion: The probability of CKD-free and T2DM-free survival in prediabetic patients is relatively low. Therefore, comprehensive preventive efforts are needed, involving the community, BPJS Kesehatan, and policymakers.
