Exclusive breastfeeding is the practice of providing only breast milk without any additional food or drink until the baby reaches 6 months of age. This is crucial for a child’s growth, intelligence, and immune system, which ultimately can improve the quality of human resources and reduce future healthcare costs. However, the coverage of exclusive breastfeeding in North Sumatra still needs improvement, reaching only 43,9%. This study aims to identify the dominant factors associated with exclusive breastfeeding among infants aged 6–23 months in North Sumatra Province. The research design used was cross-sectional, utilizing secondary data from SKI 2023. The analysis included univariate analysis, bivariate analysis using the chi-square test, and multivariate analysis using multiple logistic regression. The results showed that the rate of exclusive breastfeeding in North Sumatra was 44,2%. Bivariate analysis indicated that maternal age, maternal education, maternal employment status, history of high-risk pregnancy, method of delivery, gestational age, and birth attendant were significantly associated with exclusive breastfeeding (p-value < 0,05). Place of delivery was identified as the dominant factor, with mothers who gave birth in referral health facilities having a 16,116 times higher likelihood of practicing exclusive breastfeeding compared to those who delivered in non-health facilities (p-value = 0,002; OR = 16,116; 95% CI = 2,739–94,831).
