Hypertension is a major public health problem and one of the leading risk factors for cardiovascular disease. Variations in population characteristics and lifestyle may contribute to differences in the prevalence of hypertension across regions. This study aimed to compare lifestyle factors associated with hypertension, systolic hypertension, and diastolic hypertension among the productive-age population (18–59 years) in Central Kalimantan and South Kalimantan Provinces, Indonesia. This study used secondary data from the 2023 Indonesian Health Survey (SKI) and employed a cross-sectional design. The study included 5,925 respondents from Central Kalimantan and 8,747 respondents from South Kalimantan aged 18–59 years. Data were analyzed using univariate analysis, bivariate analysis with the chi-square test, and multivariable logistic regression. Results: The prevalence of hypertension in Central Kalimantan was 36.6%, with systolic hypertension and diastolic hypertension accounting for 35.7% and 70.4%, respectively. In South Kalimantan, the prevalence of hypertension was 30.8%, while the prevalence of systolic hypertension and diastolic hypertension was 32.6% and 66.1%, respectively. Age, body mass index (BMI), and waist circumference were consistently associated with hypertension, systolic hypertension, and diastolic hypertension in both provinces. However, lifestyle-related factors differed between provinces. In Central Kalimantan, hypertension was primarily associated with dietary patterns, particularly the consumption of high-fat foods and sugary foods and beverages. In contrast, hypertension in South Kalimantan was more strongly associated with social and behavioral factors, including educational attainment, consumption of processed foods containing preservatives, alcohol consumption, and smoking. Individuals aged 45–59 years had approximately 3.7 times higher odds of developing hypertension than those aged 18–29 years in Central Kalimantan (AOR = 3.711; 95% CI: 3.000–4.590) and South Kalimantan (AOR = 3.757; 95% CI: 3.114–4.534). Conclusion: Both similarities and differences were identified in the factors associated with hypertension, systolic hypertension, and diastolic hypertension among the productive-age population in Central Kalimantan and South Kalimantan. Age was the strongest factor associated with all three outcomes in both provinces. These findings highlight the need for hypertension prevention and control strategies tailored to the risk factor profiles of each province.
