Abstrak
Metabolik sindrom (MetS) menjadi tantangan kesehatan serius pada pekerja sedenter seperti operator truck coal hauling. Penelitian ini bertujuan menganalisis hubungan faktor manusia, pekerjaan, lingkungan kerja fisik, dan stresor psikososial terhadap risiko MetS pada operator di PT X, Kalimantan Timur. Metode penelitian menggunakan kuantitatif observasional analitik dengan rancangan cross-sectional. Data primer dikumpulkan melalui kuesioner GPAQ, FFQ, NASA-TLX, dan survei stres kerja Permenaker No. 5 Tahun 2018. Data sekunder berupa rekam medis (Medical Check-Up 2025) digunakan untuk menegakkan diagnosis MetS berdasarkan kriteria IDF/NCEP-ATP III (minimal 3 dari 5 parameter). Pengukuran lingkungan dilakukan di kabin truk. Sampel final berjumlah 125 operator laki-laki yang memenuhi kriteria inklusi dan eksklusi. Analisis data menggunakan uji Chi-Square dan Regresi Logistik Berganda. Hasil penelitian menunjukkan prevalensi MetS sebesar 24,8% (31 orang). Secara individual, persentase kondisi klinis abnormal cukup tinggi: tekanan darah tinggi (88%), kolesterol HDL rendah (74%), trigliserida tinggi (67%), obesitas abdominal (56%), dan gula darah puasa tinggi (35%). Analisis bivariat menunjukkan faktor yang berhubungan signifikan (p < 0,05) dengan MetS adalah usia, aktivitas fisik, konsumsi karbohidrat, konsumsi buah, perilaku merokok, dan lama bekerja. Faktor lingkungan fisik dan komponen psikososial tidak berhubungan signifikan (p ≥ 0,05). Pemodelan akhir multivariat menegaskan lama bekerja < 10 tahun (p = 0,004; OR = 0,228) dan aktivitas fisik aktif (p = 0,012; OR = 0,217) sebagai faktor protektif. Perilaku merokok menunjukkan kecenderungan kuat meningkatkan risiko MetS hingga 3,86 kali lipat (p = 0,054; OR = 3,860). Penelitian menyimpulkan bahwa aktivitas fisik, masa kerja, dan merokok merupakan faktor penentu paling dominan. PT X disarankan mengintegrasikan program workplace wellness, mengontrol nutrisi kantin/katering bersama ahli gizi, mengoptimalkan skrining berkala parameter MetS pada operator senior, serta memfasilitasi program berhenti merokok.

Metabolic syndrome (MetS) poses a severe health challenge for sedentary workers, such as coal hauling truck operators. This study aims to analyze the relationship between human, job, physical work environment, and psychosocial stressors toward the risk of MetS among operators at PT X, East Kalimantan. This quantitative study utilized an analytical observational approach with a crosssectional design. Primary data were collected using the GPAQ, FFQ, NASA-TLX  questionnaires, and the work stress survey from Permenaker No. 5/2018. Secondary  data from the 2025 Medical Check-Up (MCU) reports were used to diagnose MetS  based on the joint IDF/NCEP-ATP III criteria (meeting at least 3 out of 5 clinical  parameters).  Environmental measurements (noise, temperature, vibration) were conducted directly inside the truck cabins. The final sample consisted of 125 male operators who met all inclusion and exclusion criteria. Data were analyzed using Chi-Square tests for bivariate analysis and Multiple Logistic Regression for multivariate analysis. The results showed a MetS prevalence of 24.8% (31 operators). Individually, the percentage of abnormal clinical conditions was considerably high: high blood pressure (88%), low HDL cholesterol (74%), high triglycerides (67%), abdominal obesity (56%), and high fasting blood glucose (35%). Bivariate analysis revealed that age, physical activity, carbohydrate consumption, fruit consumption, smoking behavior, and length of employment were significantly associated with MetS (p < 0.05). Physical work environment factors and psychosocial stress components showed no significant association (p ≥0.05). The final multivariate model confirmed that a length of employment < 10 years (p = 0.004; OR = 0.228) and being physically active (p = 0.012; OR = 0.217) acted as significant protective factors. Smoking behavior demonstrated a strong trend toward increasing MetS risk by 3.86 times (p = 0.054; OR = 3.860). The study concludes that physical activity, length of employment, and smoking are the most dominant determinants of MetS. PT X is advised to integrate structured workplace wellness programs, manage canteen/catering nutrition alongside professional nutritionists, optimize routine MetS screenings for senior operators, and facilitate smoking cessation initiatives.
