Abstrak
Kanker serviks menempati urutan ketiga kanker terbanyak di Indonesia dengan 36.964 kasus baru dan menjadi penyebab kematian akibat kanker tertinggi kedua pada perempuan dengan 20.708 kematian. Kementerian Kesehatan merespon situasi ini dengan menginisasi Rencana Aksi Nasional: Eliminasi Kanker Leher Rahim Tahun 2023-2030 untuk memperkuat deteksi dini pada perempuan. Di Provinsi DKI Jakarta, program skrining telah dilaksanakan melalui pendekatan co-testing HPV DNA dan Inspeksi Visual Asam Asetat (IVA). Penelitian ini bertujuan untuk menganalisis faktor-faktor yang berhubungan dengan hasil IVA positif menggunakan desain cross-sectional berdasarkan Data Hasil Skrining Kanker Serviks Dinas Kesehatan Provinsi DKI Jakarta tahun 2025, dengan melibatkan 149.879 peserta yang dianalisis menggunakan perhitungan prevalence ratio (PR). Prevalensi IVA positif ditemukan sebesar 0,7% (1.093 peserta). Analisis bivariat menunjukkan kelompok usia < 30 tahun memiliki prevalensi IVA positif tertinggi, di mana kelompok usia 30-39 tahun (PR: 0,65; 95% CI: 0,568-0,750), 40-49 tahun (PR: 0,34; 95% CI: 0,293-0,411), dan ≥ 50 tahun (PR: 0,17; 95% CI: 0,144-0,222) menunjukkan prevalensi yang lebih rendah. Infeksi high-risk HPV merupakan faktor dengan asosiasi terkuat terhadap IVA positif (PR: 3,3; 95% CI: 2,656-4,101), dengan asosiasi yang semakin kuat pada genotipe hr-HPV tipe 16/18 (PR: 3,92; 95% CI: 2,869-5,365). Inisiasi hubungan seksual <18 tahun (PR: 1,51; 95% CI: 1,231-1,841), riwayat persalinan pertama <20 tahun (PR: 1,46; 95% CI: 1,196-1,784), dan status nullipara berhubungan dengan peningkatan prevalensi IVA positif, di mana kelompok multipara rendah (PR: 0,61; 95% CI: 0,535-0,695) dan kelompok grand multipara (PR: 0,40; 95% CI: 0,319-0,507) menunjukkan prevalensi lebih rendah dibanding nullipara. Penggunaan kontrasepsi, baik hormonal (PR: 1,44; 95% CI: 1,172-1,795) maupun non-hormonal (PR: 1,41; 95% CI: 1,175-1,701), menunjukkan prevalensi IVA positif yang lebih tinggi dibandingkan yang tidak menggunakan KB. Status merokok membentuk pola dose-response yang konsisten, di mana perokok aktif memiliki prevalensi IVA positif 2,94 kali lebih tinggi (PR: 2,94; 95% CI: 2,294-3,774) dan perokok pasif 1,20 kali lebih tinggi (PR: 1,20; 95% CI: 1,018-1,437) dibandingkan yang tidak merokok. Studi ini menyimpulkan bahwa infeksi hr-HPV, usia muda, inisiasi seksual dan persalinan dini, paritas, penggunaan kontrasepsi, serta status merokok berhubungan bermakna dengan hasil IVA positif, sekaligus menekankan perlunya penguatan program skrining yang lebih inklusif dan berbasis faktor risiko di DKI Jakarta.
Cervical cancer ranks third among the most common cancers in Indonesia with 36,964 new cases and stands as the second leading cause of cancer death among women with 20,708 deaths. The Ministry of Health responded to this situation by initiating the National Action Plan: Elimination of Cervical Cancer 2023-2030 to strengthen early detection among women. In DKI Jakarta, screening has been implemented through a co-testing approach combining HPV DNA testing and Visual Inspection with Acetic Acid (VIA). This study aimed to analyze factors associated with positive VIA results using a cross-sectional design, drawing on cervical cancer screening data from the DKI Jakarta Provincial Health Office for 2025, involving 149,879 participants analyzed using prevalence ratio (PR) calculations. The prevalence of positive VIA was 0.7% (1,093 participants). Bivariate analysis showed that the age group below 30 years had the highest positive VIA prevalence, with older age groups showing lower prevalence: 30-39 years (PR: 0.65; 95% CI: 0.568-0.750), 40-49 years (PR: 0.34; 95% CI: 0.293-0.411), and ≥ 50 years (PR: 0.17; 95% CI: 0.144-0.222). High-risk HPV infection demonstrated the strongest association with positive VIA (PR: 3.3; 95% CI: 2.656-4.101), particularly for hr-HPV genotypes 16/18 combined (PR: 3.92; 95% CI: 2.869-5.365). Sexual debut before age 18 (PR: 1.51; 95% CI: 1.231-1.841), first delivery before age 20 (PR: 1.46; 95% CI: 1.196-1.784), and nulliparity were associated with higher positive VIA prevalence, while low multiparity (PR: 0.61; 95% CI: 0.535-0.695) and grand multiparity (PR: 0.40; 95% CI: 0.319-0.507) showed lower prevalence compared to nulliparous women. Contraceptive use, both hormonal (PR: 1.44; 95% CI: 1.172-1.795) and non-hormonal (PR: 1.41; 95% CI: 1.175-1.701) was associated with higher positive VIA prevalence compared to non-users. Smoking status demonstrated a consistent dose-response pattern, with active smokers showing 2.94 times higher prevalence (PR: 2.94; 95% CI: 2.294-3.774) and passive smokers 1.20 times higher (PR: 1.20; 95% CI: 1.018-1.437) compared to non-smokers. This study concludes that hr-HPV infection, young age, early sexual debut and childbearing, parity, contraceptive use, and smoking status are all significantly associated with positive VIA results, underscoring the need for more inclusive, risk-factor-based cervical cancer screening programs in DKI Jakarta.
