Pasien ICU yang mengalami gagal nafas memerlukan ventilator. Pasien dengan ventilator sangat memerlukan perawatan ekstra dan mahal. Pasien dengan prolonged mechanical ventilation (tidak bisa lepas dari ventilator lebih dari 21 hari) memiliki risiko mengalami gangguan fungsi organ permanen. Trakheostomi dapat mempercepat pasien lepas dari ventilator dan mempersingkat lama rawat di ICU. Trakheostomi yang dilakukan lebih awal akan mempercepat pasien lepas dari ventilator. Tujuan penelitian ini adalah untuk mengetahui pengaruh trakheostomi dini serta faktor konfounding yang berpengaruh terhadap durasi penggunaan ventilator pada pasien ICU RSPAD. Desain studi yang digunakan adalah kohort retrospektif dengan menggunakan data rekam medis dan flow sheet pasien ICU RSPAD tahun 2015. Sampel berjumlah 40 pasien. Analisis data dilakukan dengan menggunakan survival analysis. Hasil analisis multivariat menunjukkan bahwa trakheostomi dini berpengaruh terhadap durasi penggunaan ventilator (HR=0,31, 95% CI: 0,07-1,41) setelah dikontrol variabel jenis kelamin, status gizi, berat penyakit, dan penurunan kesadaran. Patients on ventilator need extra care that lead to spend high cost. Patients on prolonged mechanical ventilation (can not be disconnected from ventilator more than 21 days) are prone to experience organ dysfunction. Tracheostomy is one of the medical procedure that can shorten duration of patient on ventilator and shorten length of ICU stay. Duration of mechanical ventilation will be shorter when the tracheostomy is performed earlier. The aim of this study is to find out the effect of early tracheostomy to duration of ventilator in RSPAD ICU patients. Study design applied to this study is retrospective cohort using medical record and registration book of ICU RSPAD at the year 2015. Fourty patients was enrolled at this study. Data process using survival analysis. Multivariate analysis results show that early tracheostomy has effect on duration of ventilator (HR=0,31, 95% CI: 0,07-1,41) after been controlled by covariat factors such as sex, nutrient status, severity of illness, and consciousness deterioration.
