Rekam Medis Elektronik berevolusi dari sebuah sistem yang sepenuhnya data dilaporkan oleh penyedia layanan kesehatan menjadi tren yang memungkinkan pasien untuk memiliki akses ke data kesehatan mereka sendiri, menjadikan mereka pemilik dara tersebut yaitu personal health record. Pada pasien ODHIV, penggunaan PHR dapat membuat pasien ODHIV aktif dalam perawatan kesehatan mereka sendiri, seperti ingin mengetahui jumlah sel T, viral load yang masih ada didalam tubuh, kepatuhan pengobatan ARV, serta perilaku yang berisiko. Penelitian ini adalah pengembangan sistem informasi Rapid Application Development (RAD) dengan teknik prototyping. Hasil penelitian diperoleh Analisa kebutuhan sistem informasi, desainmodel terstruktur dan interface untuk kemudahan penerapan sistem informasi oleh pengguna. Sistem informasi yang dikembangkan dapat memberikan kemudahan mengakses informasi kesehatan pasien ODHIV dengan cepat, mudah, akurat dan kapan saja. Sistem informasi pelayanan pasien ODHIV memberikan solusi untuk dapat mengidentifikasi kasus lost to follow up.
