Abstrak:
Industri tambang batubara merupakan sebuah industri dengan risikokeselamatan dan kesehatan kerja yang tinggi. Karenanya, penulis memandangperlunya melakukan sebuah penelitian pada sektor industri tambang yangdiharapkan kemudian akan bermuara pada peningkatan upaya-upaya sistematisdalam rangka penurunan angka kecelakaan kerja dan penyakit akibat kerja, sertapenurunan angka fatalitas pada aktivitas tambang batubaraPenelitian ini membahas penilaian dan evaluasi risiko K3 pada aktivitastambang di PT. Adaro Indonesia Site Tanjung Tabalong Kalimantan SelatanTahun 2013. Penelitian ini adalah deksriptif analitik dengan pendekatanobservasional. Penelitian ini menggunakan metode identifikasi bahaya dan risikomenggunakan JHA (Job Hazard Analysis) dan analisis penilaian risiko semikuantitatif menurut AS/NZS ISO 31000 dengan mengalikan consequences,probability dan exposure. Hasil penelitian ini menunjukkan bahwa aktivitas tambang di PT Adaro Indonesia Site Tanjung Tabalong memiliki 182 risiko keselamatan dan kesehatan kerja.Kata kunci: risiko, batubara, aktivitas tambang.

Abstrak:
Berdasarkan hasil kajian literatur, aktivitas hauling, loading, dan dumping merupakan aktivitas berisiko tinggi di pertambangan (Kecojevic dan Radomsky, 2004; MSHA, 2018). Pada tahun 2015 sampai 2018, di PT. XYZ telah terjadi beberapa kecelakaan pada aktivitas tersebut, sehingga menyebabkan fataliti. Oleh karena itu perlu dilakukan kajian risiko mendalam terkait tiga kegiatan tersebut di PT. XYZ. Kajian risiko dilakukan dengan metode failure modes and effects analysis (FMEA). Ditemukan 71 mode kegagalan potensial di PT. XYZ, terdiri dari 7 temuan tahap persiapan, 18 temuan proses pemuatan, 35 temuan proses pengangkutan, dan 11 temuan proses pembongkaran. Dari 71 mode kegagalan, 25% mode kegagalannya memiliki tingkat risiko sangat tinggi, seperti kerusakan ban dumptruck akibat batu tajam, kegagalan fungsi rem, ban bocor saat berjalan, unit loader menabrak batu besar ketika manuver, unit loader terkena pentalan batu dan mengenai kabin ketika pengisian, ban unit loader mengalami sayatan besar akibat ceceran batu tajam, unit dumptruck terperosok di permukaan labil, kendaraan ringan terjatuh saat berjalan di tebing. Oleh karena itu, perlu dilakuan peningkatan perawatan pada unit alat berat, lingkungan kerja aman, dan peningkatan kompetensi operator.

As recommendation, it is important to strengthen maintenance of heavy equipment, improve safe environment, and increase operator competence.

Abstrak:
Industri tambang merupakan salah satu industri yang mempunyai potensi bahayatinggi yang dapat menyebabkan terjadinya kecelakaan, dimana menempati urutanjumlah kecelakaan tertinggi bila dibandingkan dengan sektor lain. PT. Smerupakan kontraktor tambang permukaan yang telah menerapkan SistemManajemen Integrasi, namun hasil observasi menunjukkan banyaknyapelanggaran dan ketidakpedulian terhadap permasalahan keselamatan dankesehatan kerja. Perlu dilakukan kajian persepsi risiko pada pekerja sektortambang permukaan sebagai upaya pengendalian risiko kecelakaan kerja. Tujuanpenelitian ini adalah untuk mengetahui gambaran persepsi risiko keselamatan dankesehatan kerja menggunakan pendekatan psikometri. Desain penelitian crosssectional, menggunakan kuesioner, analisis data menggunakan univariat danbivariat. Hasil penelitian menunjukkan gambaran persepsi risiko keselamatan dankesehatan kerja pada kategori seimbang antara persepsi risiko baik dan persepsirisiko buruk. Persepsi risiko baik terdapat pada dimensi kesegeraan dampak,keparahan konsekuensi dan pengendalian risiko. Sedangkan persepsi burukterdapat pada dimensi kesukarelaan terhadap risiko, pemahaman risikoberdasarkan pengalaman, potensi dampak, reaksi yang ditimbulkan, pengetahuanterhadap risiko dan kebaruan risiko. Disarankan bagi PT. S untuk melakukanupaya promotif melalui pelatihan yang terencana, memaksimalkan forumm safetytalk dan toolbox meeeting, meningkatkan pengawasan kerja melalui inspeksi,pembuatan rencana kerja yang sistematis dan terperinci serta menerapkan sistemhadiah dan hukuman.

Abstrak:
Kecelakaan yang berkaitan dengan lalu lintas dan insiden yang berkaitan dengan kendaraan menjadi penyebab utama kecelakaan di area pertambangan. Salah satu penyebabnya adalah kelelahan pada operator truk tambang. Penelitian ini dilaksanakan untuk menjelaskan gambaran kelelahan subjektif dan menganalisis faktor-faktor risiko yang berhubungan dengan kelelahan subjektif pada operator kendaraan tambang batu bara di area mining dan hauling PT Adaro Indonesia. Faktor-faktor risiko yang diteliti meliputi faktor risiko tidak terkait pekerjaan (usia, status gizi (IMT), lingkar leher, keluhan kesehatan, kuantitas tidur, dan kualitas tidur) dan faktor risiko terkait pekerjaan (area kerja, masa kerja, shift kerja, commuting time, dan lingkungan kerja terutama temperatur, kebisingan, getaran, dan pencahayaan). Penelitian dilaksanakan pada bulan Februari hingga Juli 2022. Data yang digunakan dalam penelitian ini berasal dari kuesioner yang disebarkan secara dalam jaringan (daring), yang meliputi kuesioner karakteristik individu dan pekerjaan, Fatigue Assessment Scale (FAS) dan Pittsburgh Sleep Quality Index (PSQI). Data dianalisis menggunakan analisis deskriptif dan analisis inferensial dengan uji chi-square serta uji regresi logistik ganda model prediksi. Besar sampel minimal dalam penelitian ini adalah 436 operator, namun data yang berhasil dianalisis adalah sebanyak 440 responden. Hasil penelitian menunjukkan bahwa sebanyak 130 operator (29,5%) mengalami kelelahan subjektif. Hasil analisis statistik inferensial menggunakan uji chi-square menunjukkan bahwa terdapat hubungan yang signifikan antara faktor risiko tidak terkait pekerjaan, yaitu status gizi (IMT gemuk dan obesitas), keluhan kesehatan, dan kualitas tidur terhadap kelelahan subjektif pada operator. Hasil analisis statistik inferensial juga menunjukkan bahwa terdapat hubungan yang signifikan antara faktor risiko terkait pekerjaan, yaitu masa kerja, temperatur, kebisingan, getaran, dan pencahayaan, terhadap kelelahan subjektif pada operator. Sementara itu, hasil analisis inferensial menggunakan uji regresi logistik ganda model prediksi menunjukkan bahwa kualitas tidur merupakan variabel yang paling dominan berhubungan dengan kelelahan subjektif pada operator.

Penelitian ini menganalisis faktor kegagalan yang terjadi terhadap insiden di jalur hauling PT.X, dimana insiden di jalur hauling merupakan insiden berulang dengan persentase tertinggi selama dua tahun terakhir. Fokus penelitian merupakan analisis dari faktor individu ke organisasi, yaitu kondisi laten dan kegagalan aktif. Metode yang digunakan dalam penelitian ini dibuat berdasarkan Human Factor Analysis Classification System in Mining Industry (HFACS-MI) dengan teknik telaah dokumen dan wawacara sebagai pengumpulan data. Hasil penelitian mengungkapkan bahwa kondisi laten lebih banyak ditemukan, khususnya organizational influence dan unsafe leadership yang berinteraksi dengan kegagalan aktif, dimana skill based error paling banyak ditemukan dalam penelitian. Interaksi dari kedua faktor kegagalan tersebut meningkatkan risiko terjadinya insiden di jalur hauling.
This study analyzes the factors contributing to incidents on the hauling route of PT.X, where incidents on the hauling route have been recurring with the highest percentage over the past two years. The research focuses on analyzing factors from individual to organizational levels, specifically latent conditions and active failures. The methodology employed in this study is based on the Human Factor Analysis Classification System in Mining Industry (HFACS-MI), utilizing document review and interviews as data collection techniques. The research findings reveal a predominance of latent conditions, particularly organizational influences and unsafe leadership, interacting with active failures, where skill-based errors are most frequently identified. The interaction of these two failure factors increases the risk of incidents on the hauling route.
