Indonesia is one of the ten largest coal producing country in the world. Mining characteristics which complicated and surrounded with high technology make the risk of workplace accidents in mining industry unavoidable which occur due to many factors. The purpose of this study is to describe the immediate causes and basic causes of occupational accidents on surface coal mining activities in Mining Operation Division PT X in 2015. This study refers to Domino and Loss Causation Model theory. The design uses a mixed methods research. The quantitative data using secondary data from the accident investigation reports to get the accident causes distribution, then followed by a qualitative study which analyzing occupational accident causes of property damage and lost time injury incidents. The research tools use SCAT and in-depth interview. The results show that the most contributing factor as a direct cause are improper decision making, violation, and hazardous environmental conditions. While the basic causes include: inappropriate aggression, fatigue and inadequate identification of critical safe behaviors. Improvement behavior is now performed by operators by obey the work procedures. Supervisors will upgrade supervision intensively and management will do the refreshment of working procedures to prevent similar accidents happen again in the next day.

Hand and finger injuries commonly happen in maintenance division. This happenbecause type of work in maintenance division have various stage, workingalongside a running process, using complicated machinery, and time contraints.The objective of this study was to describe occupational accident and analyzeaccident causes hand and finger injury in Maintenance Division PT X. Thisresearch use mixed method research, that start with collecting accident data and indepth interview to 6 person hand and finger injuried, supervisor and management.The most direct causes were lack of knowledge of hazards present, improperdecision making or lack of judgement, and improper use of tools. The underlyingcauses were inadequate development of policies / standards / procedures andinadequate identification of critical safe behaviors. Workers have inadequateawareness to health and safety. The roles of supervisor are reminder workersabout safety issues. Management have made and developed work procedure toprevent accident.

One of the main purpose of accident investigation is to create recommendations for corrective actions in order to prevent the recurrent accident in the future. Some reccurent accidents at PT.X indicating that the previous accident investigations have not done effectively. One of the main factors in accident investigation is the quality of investigators, because investigators conduct the whole steps of accident investigation. Based on an early interview with some of the investigators, they said that they‟ve never been trained about accident investigation. Thus, this qualitative study aims to assess the quality of accident investigators at PT.X. The aspects of quality that will be assessed in this study are competency, consistency, and the ability of investigators to be open minded. The method used in this study is descriptive qualitative with in-depth interviews, observations, and secondary data. The result showed that the overall quality of accident investigators at PT.X is categorized not good, in which the competency aspect is poor, consistency aspect is good, and the ability of investigators to be open minded aspect is good enough. Setiap kasus insiden dipicu berbagai faktor penyebab dasar dan penyebab langsung berupa faktor pekerjaan dan faktor pribadi. Mengacu pada teori Heinrich bahwa faktor pribadi adalah 80% penyebab dasar terjadinya insiden, maka faktor pribadi pekerja harus dikelola serius, antara Iain melalui pelatihan K3 yang tepat guna dan tepat sasaran. Untuk mendapatkan gambaran pelatihan K3 yang tepat guna dan tepat sasaran, tesis ini membahas bagaimana melakukan sebuah analisis kebutuhan pelatihan K3 bagi operator dan mekanik di PT. X, sebuah perusahaan pertambangan batubara terbuka. Kasus insiden tertinggi perusahaan ini menimpa operator alat berat dan mekanik. Populasi kedua jabatan ini paling tinggi dibanding jabatan lainnya dengan faktor risiko K3 yang juga paling tinggi. Mayoritas penyebab dasar insiden yang terjadi adalah faktor pribadi yang terkait dengan kurang pengetahun, stres psikologis. dan perilaku. Proses zmalisis kebutuhan pelatihan yang digunakan dalam tesis ini adalah: analisis organisasi, yaitu untuk mengambarkan dukungan organisasi atas penyelenggaraan pelatihan; analisis tugas, yaitu untuk mendapatkan gambaran jenis pelatihan yang dibutuhkan berdasarkan tugas yang dilakukan dan faktor risiko kerjanya; dan analisis personal, yaitu untuk rnendapatkan gambaran kompetensi jabatan yang dimiliiki pekerja. Selanjutnya dilakukan kategorisasi pelatihan K3 berdasarkan tujuan pelatihan. Pendekatan kualitatif digunakan dalam penelitian ini. Tujuannya adalah untuk dapat lebih mengeksplorasi faktor risiko yang dihadapi operator dan mekanik, dan mengeksplorasi jenis pelatihan K3 yang dibutuhkan kedua jabatan temebut melalui diskusi dengan level manajemen kedua jabatan tensebut dan manajemen yang mengclola pengembangan sumber daya manusia melalui pelatihan. Diskusi yang dilakul-ran borpegang pada ceklis pelatihan yang telah disiapkan peneliti, tujuarmya adalah sebagai pedornan yang mernpennudah proses diskusi. Pada akhirnya, penelitian ini dapat menggambarkan bahwa manajemen PT. X mendulcung penuh pelaksanaan pelatihan, menggambarkan jonis pelatihan yang dibutuhkan, mengetahui bahwa data kornpetensi pckerja belum dimiliki, dan dapat menghasilkan matrik pelatihan K3 bagi operator dan mckanik berdasar faktor risiko kerja yang disusun berdasarkan tujuan pelatihan. Dengan data-data yang ada dalam tesis ini, PT. X diharapkan mempertahankan hal-hal positif yang telah dimiliki dan meningkatkan dokumentasi kompetensi pekerja per individu sehingga program pengembangan per individu Iebih mudah dilaksanakan. Disamping itu, analisis kebutuhan pelatihan K3 dan pelatihan lainnya disarankan untuk dikaji ulang secara berkesinambungan.
Every analysis on any incident is triggered by various factors, either basic causes or direct causes, including occupational and personal ones. According to Heinrich, personal factors contribute 80% of basic causes of incidents. Hence, personal factors of employees should be genuinely managed by, among outliers, providing ejective and ejicient training on occupational health and safety. In order to provide a description on an ejective and ejicient training on occupational health and salty, this thesis will discuss how to analyze requirements of occupational health and safety training for operators and mechanics in PT X an open coal mining company. Most incidents in the company are related to heavy-duty operators and mechanics. Besides having the highest risk in term of occupational health and safety, the two are the most populated positions. The majority of incidents happened because of personal factors, e.g. lack of knowledge, psychological stress, and unsafe behavior. The processes involved in the analysis on training requirements include: organizational analytsis, i.e. describing organizational support for training; task analysis, i.e. desiring types of required training based on tasks performed and their occupational risks; and personal analysis, i.e. describing occupational competency acquired by the workers. Then, classification on occupational health and safety training based on training objectives will follow. This study tales a qualitative approach. The objective is to allow greater exploration on risk factors facing the operators and mechanics, and to explore types of occupational health and safety training needed by the two positions through discussion with management-level personnel of the two positions and the management staffs' responsible fur human resource development. The discussion rowers to the training checklist prepared by researcher, providing guidelines for the discussion. Eventually, this study can illustrate how the management ofP1'f X has been fully supportive to the training, describe the types of training required, show that data on employees competency are not yet available, and generate occupational health and safety training matrix operators and mechanics based on the occupational risk factors as listed in training objectives. It is expected that the data provided in the thesis will allow PII X to maintain positives already found in the company and to improve documentation on competency of each individual employee, which will make programs on individual development better executed. Also. it is recommended that the analysis on occupational health and safety training requirements is continuously revisited.
The safety performance of a company shows how well the company is in protecting employee safety and reducing losses due to work accidents (Sullivan, 2000). Safety performance in a company or organization is influenced by various factors, one of which is the human factor. In the loss causation model, (Bird & Germain, 1996) explains that there are five elements as factors that cause accidents, namely lack of control due to inadequate programs, program standards and compliance with standards, basic causes consisting of personal factors and work factors, causes direct actions consisting of non-standard acts and conditions, and the resulting harm to people, property and processes. PT. X is a Mining Service Company engaged in blasting. From the data of safety accidents that occurred at PT. X over the past 10 years, it was found that 60% of the main causes of these accidents were due to the human factor. The research in this thesis aims to determine the effect of human factors on the safety performance of PT.X and to analyze the most influential human factors in improving the safety performance of PT. X as measured by the level of work accidents. Based on the loss causation model and the basic causes of work accidents at PT. X period 2009-2013, there were six (6) human factors studied, namely competence, fitness to work, compliance with procedures, safety communication, safety awareness, and safety behavior. The result of the research is that competence and communication are the most influential human factors in improving the safety of PT. X.
This research examines work-related accidents in the mining industry categorized as injury cases, using the Human Factors Analysis and Classification System in Mining Industry (HFACS-MI). The mining industry is known as a high-risk sector, where serious incidents such as major injuries and fatalities frequently occur despite investigations and preventive measures. The study aims to identify the main causal factors of workplace accidents based on the HFACS-MI framework, which classifies human errors and systemic weaknesses within mining organizations. Using data from injury-related accident cases in 2024, both qualitative and quantitative analyses were conducted to assess contributing factors from the operator level up to the organizational level.The findings reveal that the majority of accidents were triggered by unsafe acts, particularly skill-based errors, indicating deficiencies in workers' basic competencies. Additionally, latent failures, such as inadequate supervision and organizational inefficiencies, were also found to play a significant role.
