Abstrak:
Jumlah kasus HIV / AIDS setiap tahunnya mengalami peningkatan, dan salah satu faktor yang menyebkan peningkatan kasus HIV adalah dengan adanya peningkatan jumlah penularan di kalangan pengguna NAPZA suntik. Masalah tersebut mendorong dilakukannya penelitian ini untuk melihat faktor-faktor yang berhubungan dengan status HIV pada pengguna NAPZA suntik di Klinik PTRM Rumah Sakit Ketergantungan Obat Cibubur Tahun 2014. Desain penelitian yang digunakan adalah cross-sectional dengan sampel 46 pasien NAPZA suntik di Klinik PTRM. Hasil penelitian menunjukkan status HIV ( + ) sebesar 63%, diketahui 87% penasun adalah laki-laki, 58.7% berusia 34 tahun, 71,7% memiliki tingkat pendidikan SMA, 58.7% menikah, 69.6% memiliki tingkat pengetahuan HIV yang baik, 63% penasun telah menyuntik 9 tahun, 50% penasun pertama kali menyuntik di usia < 19 tahun, 69.6% penasun menyuntik 3 kali sehari, 87% penasun berbagi jarum suntik, 43.5% penasun melakukan sterilisasi dengan air bersih, 60.9% penasun melakukan seks berisiko rendah, 80.4% penasun memanfaatkan LJSS, 52.2% telah mengikuti terapi metadon 4 tahun, 58.7% penasun mendapatkan NAPZA dari 2 sumber yang berbeda. Hasil uji Chi Square menyatakan terdapat hubungan yang bermakna antara usia pertamakali menyuntik (PR 1.8; P Value = 0.02), berbagi jarum suntik (PR 4.2; P Value =0.02), dan sterilisasi jarum menggunakan air bersih (PR 5.5; P Value = 0.006) dengan status HIV. Oleh karena itu perlu dikembangkan lagi akses terhadap jarum suntik steril bagi penasun.

Abstrak:
Penggunaan ARV yang semakin meluas sebagai pengobatan pada pasien HIV memberikan harapanhidup yang lebih baik bagi ODHA. Penelitian ini bertujuan untuk mengetahui faktor-faktor yangmempengaruhi kesintasan hidup pasien HIV/AIDS yang mengakses ARV di Puskesmas Jakarta Barattahun 2015-2017. Desain penelitian ini adalah kohort retrospekstif menggunakan data rekam medispasien HIV. Sampel penelitian adalah pasien HIV yang mengakses ARV di Puskesmas Jakarta Baratpada tahun 2015-2017, yaitu sebanyak 480 pasien. Hasil penelitian ini menunjukkan probabilitaskesintasan pasien HIV selama 1, 2 dan 3 tahun pengamatan sebesar 95,2%, 91,7% dan 78,3% denganinsidens rate sebesar 4 per 1000 orang-bulan. Hasil analisis dengan regresi cox menunjukkan bahwapasien dengan koinfeksi TB lebih cepat 2,62 kali untuk meninggal dibandingkan pasien tanpakoinfeksi TB (HR = 2,621; 95%CI: 1,027-6,407, nilai P = 0,035). Untuk pasien HIV dengan tingkatadherensi minum ARV < 95% lebih cepat 9,29 kali untuk meninggal dibandingkan pasien dengantingkat adherensi ≥ 95% (HR= 9,298; 95%CI: 4,653-18,660, nilai P = 0,000).Kata kunci:HIV, ARV, Kesintasan

KaplanMeier and Cox Proportion hazard regression were used to calculate the survival time and itsassociated factors, respectively. The cumulative survival rates of those receiving ARV in 1,2 and 3years were 95,2%, 91,7% and 78,3%. Multivariate Cox Regression analysis showed that patients withTB-coinfection were at a higher risk of death from AIDS-related diseases (HR = 2,621; 95%CI:1,027-6,407, nilai P = 0,035) than patients without TB-coinfection. Abstrak:
Angka kasus infeksi HIV terus meningkat dan telah menginfeksi sekitar 36,7 juta penduduk dunia dan 2 juta diantaranya anak-anak dibawah usia 15 tahun. Tingginya prevalensi HIV pada populasi penasun menunjukkan bahwa Penasun merupakan populasi kunci yang penting dalam penyebaran HIV di Indonesia. Penelitian ini untuk mengetahui determinan yang mempengaruhi terjadinya infeksi HIV pada Penasun di Indonesia tahun 2015 dengan menggunakan data sekunder STBP Penasun pada 6 kota besar yang dilakukan oleh Kementerian Kesehatan RI. Sebanyak 1231 penasun diwawancara dan diambil sampel darahnya kemudian di lakukan analisis multivariat regresi cox dan didapatkan prevalensi HIV sebesar 28,8%, 653 (55,2%) pernah menyuntik bersama, 1116 (90,7%) menggunakan jarum bekas pakai, 336 (35%) sedang terapi PTRM. Dari penelitian ini diketahui bahwa pasangan seks dengan sesama penasun beresiko terkena HIV 2,22 kali dibanding yang tidak (95% CI 1,152-4,281, nilai p=0,017), persepsi diri merasa beresiko terkena HIV 1,68 kali kali beresiko terkena HIV (95% CI 1,176-2,406, nilai p=0,004), sharing needle memiliki resiko 1,90 kali terinfeksi HIV (95% CI 0,68-5,35), status sifilis dan riwayat IMS memiliki resiko 1,2 kali terkena HIV dan perilaku penggunaan kondom menunjukkan hasil bermakna (nilai p=0,006) dapat mencegah terkena HIV. Program pengendalian HIV di Indonesia diharapkan dapat lebih memfokuskan intervensi pada populasi kunci (penasun) edukasi pengetahuan HIV, pengobatan dan perilaku preventif.

From this study we also found that having IDUs sex partner increased risk to get HIV infection for about 2,22 times as compared those did not have (95% CI of PR: 1,152- 4,281, p-value=0,017), Self perception that ones being at risk of HIV increased risk to get HIV infection 1,68 times (95% CI 1,176-2,406, nilai p=0,004), sharing needle increased risk to get infected by HIV 1,90 times (95% CI 0,68-5,35), status of Sifilis infection and STI (sexual transmitted infection) both increased risk 1,2 times and condom use had protective effect and showed signficant association (p-value=0,006). HIV control programs in Indonesia were expected to be more focus on intervention toward key population of IDUs in areas of education to increase knowledge of HIV, therapy and behavior intervention.

Dilaporkan terdapat 38,4 juta orang yang hidup dengan HIV pada tahun 2021 dan terdapat sebanyak 58 juta kasus kasus kronis Hepatitis C pada tahun 2019. Pengguna NAPZA suntik merupakan populasi yang paling rentan untuk terinfeksi kedua virus ini akibat jalur transmisi kedua virus ini yang sangat besar melalui jarum suntik tidak steril. Kedua penyakit ini dapat terjadi secara bersamaan yang menyebabkan percepatan progres keduanya menjadi kronis. Penelitian ini bertujuan untuk melihat faktor-faktor yang berhubungan dengan kejadian koinfeksi HIV/HCV untuk mencegah penyebaran yang lebih lanjut dengan melakukan analisis bivariat dengan menggunakan chi-square dan melihat crude prevalence rate. Studi cross-sectional dari data STBP 2018-2019 di tujuh kabupaten/kota Jawa Barat populasi Penasun dilakukan dan didapatkan bahwa positivity rate koinfeksi HIV/HCV pada Penasun mencapai sebesar 9%. Ditemukan bahwa terdapat hubungan antara usia, jenis kelamin, tingkat pendidikan, status perkawninan, riwayat dipenjara, usia pertama kali menggunakan NAPZA suntik, lama menggunakan NAPZA suntik, pernah menggunakan alat suntik tidak steril, usia pertama kali berhubungan seksual, hubungan seksual satu tahun terakhir, penggunaan kondom dengan pasangan tetap, pengetahuan komprehensif HIV, akses LASS, dan akses PTRM dengan kejadian koinfeksi HIV/HCV. Dari hasil tersebut diperlukan intervensi yang tepat untuk mencegah dan menanggulangi tingginya kejadian koinfeksi HIV/HCV pada Penasun.
There are 38.4 million people reported living with HIV in 2021 and there are as many as 58 million cases of chronic hepatitis C cases in 2019. Injecting drug users are the most vulnerable population to be infected with these two viruses due to the transmission routes of these two viruses via unsterile needles. These two diseases can occur simultaneously which causes the accelerated progress of both infections to become chronic. This study aims to look at the factors associated with the incidence of HIV/HCV coinfection to prevent further spread by conducting bivariate analysis using chi-square and looking at the crude prevalence rate. A cross-sectional study of 2018-2019 IBBS data in seven districts/cities of West Java of the IDU population was conducted and it was found that the positivity rate of HIV/HCV coinfection in IDU reached 9%. It was found that there was a relationship between age, gender, education level, marital status, history of imprisonment, age at first injecting drug use, duration of injecting drug use, ever using unsterile injecting equipment, age at first sexual intercourse, sexual intercourse in the past year, use of condoms with regular partners, comprehensive knowledge of HIV, access to sterile syringe service, and access to methadone treatment, with HIV/HCV coinfection. From these results, appropriate interventions are needed to prevent and overcome the high incidence of HIV/HCV co-infection among IDU.

Penelitian ini bertujuan untuk mengetahui hubungan factor sosiodemografi, pengetahuan dan persepsi, perilaku penggunaan napza, perilaku berisiko seks, dan perilaku pencegahan dengan kejadian HIV pada kelompok kunci pengguna narkotika suntik (penasun) di Indonesia. Penelitian ini menggunakan data sekunder STBP 2018-2019. Sampel yang digunakan adalah penasun yang ada diseluruh Indonesia yang sesuai dengan kriteria inklusi dan eksklusi dengan jumlah sampel sebanyak 1.364. data dianalisis secara univariat dan bivariat. Prevalensi HIV sebanyak 13,7%.
