One of the factors that can increase the success of the family planning program is the increasing number of women who use contraception and the reduced number of unmet need for contraception. It is known that the percentage of unmet need for contraception in NTT (17.6%) and DIY (6.3%) is the highest and lowest among the ten main national family planning provinces. This study was conducted to determine whether there are determinants of the unmet need for contraception in NTT and DIY. This study is a quantitative study with a cross-sectional design and uses secondary data from the Indonesian Demographic and Health Survey (IDHS) in 2017. The number of samples in this study was 1,043 women (513 in NTT and 530 in DIY). The results showed that the proportion of women experiencing unmet need for contraception in NTT was 17.6%, of which 9.8% were for thinning and 7.7% for restriction. Meanwhile, the proportion of women who experienced unmet need for contraception in Yogyakarta was 6.3%, of which 1.8% for thinning and 4.4% for restriction. Factors related to unmet need for contraception in NTT were discussions with husbands (RO = 1.790; 95%CI = 1.24-2.58). Meanwhile, there are no factors related to the unmet need for contraception in DIY.
The problem that is feared will have a major impact on the success of the family planning program in controlling the population in Indonesia is the incidence of discontinuation of contraceptive use. The 2017 IDHS data reported that around 29% of women with various contraceptive methods decided to stop using contraceptives after 12 months of use. This study aim to describe the comparison of determinants of contraceptive discontinuation between the Western Region of Indonesia (Sumatera) and the Eastern Region of Indonesia (Nusa Tenggara, Maluku, Papua). This study uses Indonesia Demography Health Survey (IDHS) 2017. The population for this study is a women of childbearing age 15-49 years old. The dependent variable in this study is the contraceptive discontinuation, while the independent variable of this study are age, parity, fertility preferences, level of education, occupation, area of residence, wealth index, discontinued contraceptive method, internet use, and mobile phone ownership. Multivariable logistic regression was used to identify the predictors of contraceptive discontinuation. The proportion of respondent who discontinue using contraceptive was 45,7% (Sumatera) and 41,2% (Nusa Tenggara, Maluku and Papua). The most common reason for discontinuation in Sumatra and Nusa Tenggara, Maluku, Papua is because of side effects/health problems. The results of the multivariate analysis showed that the variable level of education (OR 2,63) was the largest determinant of contraceptive discontinuation in Sumatra, followed by area of residence (OR 1,13). Meanwhile, the biggest determinant of discontinuation of contraceptive use in Nusa Tenggara, Maluku, Papua is the area of residence (OR 1,42). Counseling and education related to contraceptive methods and side effects/health problems that may arise need to be intensified, especially in the group with higher education levels and living in urban areas.
Indonesia as the fourth most populous country in the world has a family planning policy, which is managed by the National Family Planning Population Agency (BKKBN), in an effort to control population numbers. The BKKBN has six strategic indicators for the 2020-2024 period, namely Total Fertility Rate (TFR), Modern Contraceptive Prevalence Rate (mCPR), Unmet need for family planning, Age Specific Fertility Rate (ASFR) 15-19 years, Family development index (iBangga) and Median Age of First Marriage for Women (MUKP). Nationally, unmet need has not met the target and when viewed by province, there are disparities. The purpose of this research is to quantify the social inequity of unmet need for contraception in Indonesia in 2012 and 2017. This research is a quantitative study with a cross-sectional design based on data from the 2012 and 2017 IDHS. The sample in this study was couples of childbearing age (PUS) who lived together and were sexually active in the last 4 weeks. The number of samples in this study were 22477 (2012) and 24173 (2017) couples. Formation of family planning service access variables using Principal Component Analysis (PCA). The inequality analysis used was the help of the Health Equity Assessment Toolkit (HEAT) developed by the World Health Organization (WHO) and grouping analysis was also carried out using a hierarchical method. The results of the research on access to family planning services are most influenced by contraceptive information provided by doctors. In general, there has been a decline in the absolute value of unmet need for contraception in Indonesia from 2012 to 2017. However, when viewed from the population (confident interval), there is no difference in unmet need from 2012 and 2017. Inequality in unmet need for contraception in Indonesia in 2017 still occurs with dimensions parity (> 2 children) and husband's age (> 45 years), followed by area of residence (rural) and social economy (top). There was a change in the priority areas of unmet need from 2012 (12 provinces) to 2017 (14 provinces). From these results it is assumed that adjacent areas do not always have similar characteristics. That is, unmet need is not influenced by territory. Variables that intersect the decline in unmet need and inequality are husband's age (> 45 years), parity (> 2 children), socioeconomic status and area of residence. If this is seen again with the cluster analysis, the variables of husband's age and parity are included in the priority province criteria. Priority provinces in 2017 have the oldest husband's average age interval (37.71 – 40.52 years) among other groups and also have the highest parity of >2 children (2.09 – 3.01 children) in their cluster members.
ABSTRAK Kesetaraan gender adalah suatu konsep yang masih diupayakan oleh pemerintah Indonesia untuk mencapai kondisi yang ideal. Disparitas gender, dimana pemberdayaan perempuan belum maksimal, secara langsung maupun tidak langsung mempengaruhi hal-hal dimana wanita memegang peranan baik sebagai pengambil keputusan maupun sebagai pelaku di berbagai bidang dalam kehidupan bermasyarakat. Keterlibatan wanita dalam berbagai aspek kehidupan harus diperhitungkan, terlepas apakah kesetaraan gender dan pemberdayaan perempuan sudah mencapai kondisi yang ideal atau tidak. Kondisi pemberdayaan perempuan Indonesia di setiap provinsi berbeda-beda. Pemberdayaan perempuan sendiri diukur melalui sikap menolak ‘kumpul’ dengan suami pada kondisi tertentu, keterlibatan dalam pengambilan keputusan rumah tangga, dan sikap istri atas pemukulan suami terhadap istri. Berdasarkan SDKI 2007, kondisi pemberdayaan perempuan di Indonesia, apabila dilihat dari sikap setuju terhadap pemukulan suami terhadap istri, persentase Provinsi NTT masih lebih tinggi bila dibandingkan dengan DI. Yogyakarta. Kemudian untuk sikap setuju dengan semua alasan penolakan ‘kumpul’ dengan suami untuk kondisi tertentu, persentase yang setuju untuk semua alasan di DI. Yogyakarta sebesar 81,9 %, sedangkan di NTT sebesar 63%. Maka dapat disimpulkan bahwa kondisi pemberdayaan wanita di DI. Yogyakarta lebih baik daripada di NTT. Selain itu, TFR DI. Yogyakarta pada tahun 2007 sebesar 1,8, dan NTT sebesar 4,2. Desain penelitian ini cross sectional dengan menggunakan data Survei Demografi dan Kesehatan Indonesia (SDKI) 2007 dengan memilih 1654 responden perempuan yang memiliki anak lahir hidup dan masih terikat dalam ikatan perkawinan. Analisis Structural Equation Modelling (SEM) digunakan untuk melihat hubungan sikap menolak ‘kumpul’ dengan suami pada kondisi tertentu, keterlibatan dalam pengambilan keputusan rumah tangga, dan sikap istri atas pemukulan suami terhadap istri dengan jumlah anak lahir hidup. Hasil penelitian menunjukkan semakin tidak setuju dengan sikap menolak ‘kumpul’ dengan suami pada kondisi tertentu maka semakin tinggi fertilitas, semakin rendah keterlibatan perempuan dalam pengambilan keputusan rumah tangga maka semakin tinggi fertilitas. Dari persamaan yang terbentuk, sikap menolak ‘kumpul’ dengan suami pada kondisi tertentu, keterlibatan dalam pengambilan keputusan, dan umur kawin memiliki pengaruh paling besar terhadap fertilitas (R2=0,049).
ABSTRACT Gender equality is a concept that is still being pursued by the Indonesian government in order to achieve the ideal conditions. Gender disparity, where the empowerment of women is not maximized yet, affects the things in which women play a role both as decision makers and subjects in various fields, either directly or indirectly. The involvement of women in various aspects of life must be taken into account, regardless of whether gender equality and empowerment of women have reached the ideal condition or not. Indonesian women empowerment conditions in each province vary. Empowerment of women is measured by their refusal to have sexual intercourse with their spouses, involvement in household decision-making, and the wives’ acceptance of physical abuse committed by their husbands. According to 2007 IDHS, the condition of women's empowerment in Indonesia; measured by acceptance of husband’s physical abuse, shows that NTT province’s rate is still higher compared to DI. Yogyakarta’s. On the other hand, concerning the agreement to all the reasons for refusing sexual act with the husbands to certain conditions, the percentage of respondents who agreed to all the reasons in DI. Yogyakarta reaches 81.9%, while in NTT province is 63%. It can be concluded that the condition of women empowerment in DI. Yogyakarta is better than in NTT. In addition, the TFR IN. Analysis of Structural Equation Modeling (SEM) is used to analyze the relationship between refusal to have sexual intercourse with their spouses in certain circumstances, involvement in household decision-making, the wives’ stand regarding physical abuse committed by their husbands and the number of babies born alive. The results show that the less of refusal to have sexual act with spouses in certain circumstances, the higher the fertility rate ; the less women’s involvement in household decision-making, the higher the fertility rate. Based on the equations formed, the refusal act to have sexual intercourse with husband in certain circumstances, involvement in decision-making, and the marriageable age have the most impact on fertility (R2 = 0.049).
Human Immunodeficiency Virus (HVI) is still a global public health problem, where by the end of 2020 it is estimated that there are around 37.7 million people in the world living with HIV. In Indonesia until March 2021, there were 427,201 people with HIV of which 89.7% occurred in the fertile age (15-49 years). The Province of Jakarta became the province with the highest number of people with the HIV, which was 71,473 people. The high level of discriminatory behavior towards people with HIV has an impact on the reluctance to do HIV tests and seek treatment and tends to hide their disease status. The purpose of this study was to determine the determinants of discriminatory behavior toward people with HIV in Jakarta Province. This study used a cross-sectional study design with a sample of 1,354 respondents, male and female aged 15-49 years, had heard of HIV, and domiciled in Jakarta Province. The multivariate logistic regression was applied to determine the determinants of discriminatory behavior. The results of this study indicate that the discriminatory behavior against people with HIV in Jakarta Province is 30.3%. Based on the multivariate logistic regression model, the younger age, lower educational level, un-exposed to mass media, and lack of comprehensive knowledge are variables significantly related to discriminatory behavior towards people with HIV (p-value less than 0.10). Age is the most related variable, age 15-24 years have a risk of 1.58 (95% CI = 1.12 - 2.16) to discriminate against towards people with HIV compared to age 35-49 years after being controlled by education, exposure to information sources, and comprehensive knowledge. It recommended that intensify the dissemination of information on HIV/AIDS, especially related to the transmission of HIV/AIDS, by strengthening the collaboration of various parties to increase knowledge and reach of society in DKI Jakarta Province.
