Anak usia sekolah merupakan kelompok usia yang kritis karena rentan terhadap masalah kesehatan. Penjaringan kesehatan adalah salah satu program UKS/M untuk memelihara dan meningkatkan kesehatan siswa, mendukung tercapainya kondisi fisik dan mental yang baik agar potensi belajar dan prestasi optimal. Pengelolaan data hasil pemeriksaan penjaringan kesehatan siswa SD di Puskesmas I Denpasar Barat masih manual sehingga penulis berkeinginan membuat rancangan sistem informasi kegiatan penjaringan Kesehatan siswa Sekolah Dasar di wilayah kerja Puskesmas I Denpasar Barat kota Denpasar. Metode penelitian ini terdiri dari beberapa tahap yaitu mendata kebutuhan pengguna, menyusun prototype, mengevaluasi prototype, membuat aplikasi serta mengevaluasi efektifitas penggunaan dan kepuasan. Hasil evaluasi penggunaan aplikasi berbasis web untuk menjaring data kesehatan anak sekolah di lingkungan Puskesmas I Denpasar Barat Kota Denpasar dinilai menggunakan kuesioner dengan hasil 100% responden setuju dengan efektifitas aplikasi serta >75% responden menyatakan puas dan sangat puas dengan aplikasi tersebut. Saran pada penelitian ini adalah Puskesmas I Denpasar Barat dapat menggunakan aplikasi pada kegiatan penjaringan siswa Sekolah Dasar. Sistem ini dapat disosialisasikan kepada instansi terkait sehingga menjadi model yang dikembangkan untuk wilayah yang lebih luas, dikembangkan untuk siswa pada jenjang yang lebih tinggi serta dapat terintegrasi dengan aplikasi sejenis untuk memperoleh data yang komprehensif.
School-aged children are a critical age group because they are vulnerable to health problems. Health screening is one of the UKS/M programs to maintain and improve student health, supporting the achievement of good physical and mental conditions for optimal learning potential and achievement. Data management of health screening examination results for elementary school students at West Denpasar Community Health Center I is still manual, so the author wishes to design an information system for health screening activities for elementary school students in the work area of West Denpasar Community Health Center I, Denpasar city. This research method consists of several stages, namely recording user needs, compiling a prototype, evaluating the prototype, creating an application, and evaluating the effectiveness of use and satisfaction. The results of the evaluation of the use of a web-based application to collect health data for school children in the West Denpasar Community Health Center in Denpasar City were assessed using a questionnaire with the results of 100% of respondents agreeing with the effectiveness of the application and >75% of respondents stating they were satisfied and very satisfied with the application. The suggestion in this research is that West Denpasar Community Health Center can use the application in elementary school student screening activities. This system can be socialized to relevant agencies so that it becomes a model developed for a wider area, developed for students at a higher level, and can be integrated with similar applications to obtain comprehensive data.

Abstrak:
ABSTRAK Kegemukan dan obesitas dapat terjadi pada setiap strata ekonomi dan usia sebagai faktor penyebab PTM. Masalah status gizi lainnya seperti stunting atau pendek memiliki pengaruh pada prestasi belajar anak. Usaha Kesehatan Sekolah (UKS) merupakan upaya preventif mengatasi masalah status gizi anak di sekolah dengan meningkatkan kesadaran status gizi siswa. Sulitnya mendapatkan data kesehatan siswa dikarenakan belum adanya pencatatan kesehatan setiap siswa. Pencatatan yang masih manual oleh Puskesmas hanya terkait jumlah status gizi siswa, sehingga cukup sulit melakukan pemantauan siswa yang memiliki masalah kesehatan. Rancangan sistem informasi usaha kesehatan ini dibuat sebagai alat untuk melakukan pemantauan status gizi siswa SD dan membantu menentukan rencana kegiatan selanjutnya. Penelitian ini menggunakan metode prototipe untuk mengembangkan rancangan sistem informasi UKS. Keterlibatan pengguna dalam pengembangan sistem informasi ini untuk menyesuaikan dengan kebutuhan informasi. Proses pengumpulan informasi dilakukan dengan melaksanakan kegiatan wawancara kepada pihak yang terlibat secara langsung dengan pelaksanaan kegiatan uks. Hasil informasi yang ada kemudian digunakan untuk analisis untuk membuat rancangan sistem informasi usaha kesehatan sekolah. Informasi ditampilkan dalam bentuk grafik masalah status gizi siswa di sekolah dan data status gizi setiap siswa. Sistem ini dapat diakses secara daring (online) untuk melihat kebutuhan informasi setiap pengguna. Harapannya sistem ini dapat diterapkan sehingga dapat membantu dalam melakukan pemantauan status gizi siswa di tingkat sekolah dasar. The design of this health information system was created as a tool for monitoring the nutritional status of elementary students and helping to determine the next activity plan. This research uses prototype method to develop UKS information system design. The involvement of users in the development of this information system to adjust to the information needs. The process of gathering information is done by conducting interviews to the parties directly involved with the implementation of UKS activities. The results of the later information were used for the analysis to make the school health business information system design. Information is displayed in graphical form of the problem of nutritional status of students in school and data of each student's nutritional status. This system can be accessed online to see the information needs of each user. The hope of this system can be applied so that it can assist in monitoring the nutritional status of students at the primary school level. Penelitian ini bertujuan untuk merancang aplikasi sistem informasi program usaha kesehatan sekolah (UKS) yang mampu menghasilkan informasi sesuai dengan kebutuhan esensial di tingkat institusi. Penelitian ini merupakan penelitian kualitatif dengan mengunakan metode RAD (Rapid Application Development)yang meliputi tahapan perencanaan kebutuhan, rancangan pemakai, dan kostruksi. Penelitian ini menghasilkan prototipe aplikasi sistem informasi manajemen program UKS berbasis web terintegrasi dari tingkat sekolah sampai ke tingkatDinas Kesehatan Kota Bogor. Sistem Pencatatan dan Pelaporan Puskesmas (SP3) merupakan bagian dari SIK, yang telah diakui sebagai sumber data yang berasal dari Puskesmas. SP3 telah diberlakukan tahun 1981 dengan SK Menkes No.63/Menkes/SK/II/1981 dengan Petunjuk pelaksanaan SK dari Dirjen Binkesmas No.143/Binkesmas/Dj/III/1981. Dalam perjalanannya sistem pencatatan dan pelaporan Puskesmas (SP3) mengalami pemantapan, hasilnya tertuang dalam SK. Dirjen Binkesmas No. 590/BM/DJ/Info/V/96 tanggal 10 Mei 1996 dengan dua komitmen terpenting menurut penulis yaitu menghindari format lain diluar SP3 dan pembinaan SP3 berjenjang. Hasil penjajakan awal di lapangan di Kab. Karawang, tenyata ditemukan beberapa kendala pemanfaatan SP3 belum berjalan seperti yang diharapkan, diantaranya tumpang tindihnya laporan SP3 dengan laporan program lain, mekanisme alur pelaporan SP3 dan UPTD Puskesmas ke Dinas Kesehatan belum tertata dengan baik, ketidak tahuan tentang system operation prosedur (SOP), tidak adanya bimbingan SP3 secara berjenjang. Penulis meneliti apakah yang menyebabkan hambatan alur pelaporan dari Puskesmas ke Dinas Kesehatan dan tidak adanya proses analisa data, umpan balik serta pembinaan teknis secara berjenjang. Tujuan penulisan ini diharapkan tersedianya jaringan informasi kesehatan untuk kelancaran dan kelangsungan SP3 dan tersedianya SOP. Rancangan penelitian dengan menggunakan metode kualitatif, dengan pendekatan evaluatif terhadap aspek review program SP3 yang saat ini berjalan di Dinas Kesehatan Kabupaten Karawang. Cara pengukurannya yaitu melalui hasil observasi langsung dengan menggunakan "Cheklist" dan melalui hasil olahan brain storming, wawancara mendalam serta work shop. Hasil yang diperoleh penelitian yaitu tersedianya mekanisme alur pelaporan SP3 dan SOPnya. Tersedianya tenaga pengelola SP3 dan sarana komputer di lingkungan Dinas Kesehatan maupun di UPTD Puskesmas, sangat memungkinkan untuk dikembangkannya jaringan informasi kesehatan pada SP3. Restukturisasi Organisasi dan Tata Kerja Dinas Kabupaten Karawang, memacu untuk menata kembali TUPOKSI para pengelola SP3 di tingkat Kabupaten maupun di tingkat UPTD Puskesmas. Untuk mengoptimalkan jaringan informasi kesehatan pada SP3 maka penulis menyarankan: Dinas Kesehatan Kabupaten Karawang dalam mengoptimalkan pelaksanaan SP3 dengan sistem jaringan informasi berbasis komputer maka diperlukan adanya peningkatan dan penambahan tenaga operator, teknisi komputer serta peningkatan kualitas komputer. Dan untuk para peneliti lain diharapkan adanya penelitian lanjutan tentang kebutuhan data esensial SP3 dan pembuatan software SP3 untuk tingkat Puskesmas yang langsung akses ke tingkat Kabupaten dan ke tingkat Propinsi.
Health Information Network Design on Recording and Reporting System of Public Health Care at Karawang Regency Government Health ServiceThe recording and reporting system of Public Health Care (SP3) is a part of Health Information System (SIK) which has been accepted as data resource of the public health care. SP3 has been accepted legally in 1981 by health minister deuce No. 63/Menkes/SK/II/1981 completed with operational direction from Public Health Guide General Directorate No.143/Binkesmas/Dj/III/1981. The application on recording and reporting system of Public Health Care has been developed and its result written in Public Health Guide General Directorate Decree No. 590/BM/DJ/Info/V/96 dated on May, 10, 1996. According to writer the last decree consists of two commitments i. e. to avoid other forms and to practice Recording and Reporting System of Public Health (SP3) itself gradually. The early result before the field in Karawang Regency, really has been found some hindrances in the usage of recording and reporting system of Public Health Care (SP3). It has not been running smoothly as Ls, expected. Among of them are found some complicated records and reports which missed with other matters, the mechanism of SP3 current from Public Health Care to government Health Service has not been well, the ignorance about procedure operation system (SOP) and no guidance on SP3 gradually. The has researched that the cause of hindrance on reporting current from Public Health Care to Government health service and the absence of data analyzing process are feed back to gradual technical guidance. This writing target expects the Health Information Network is available; the continuation of SP3 and the SOP is available. The design researches by using qualified method with evaluation tsipproach on recording and reporting system of Public Health Care are applied. Recently research designing by using qualified method with approach to SP3 Program Review Aspect is running well in Government Service of Karawang Regency. How to measure are through live observation result with using checklist and by brain storming result, by intensive interviews and by workshop. The result of the research gets the availability of current mechanism for SP3 and for SOP. The availabilities of skillful men and computers for SP3 in Government Health Service and at Public Care must have great possibility for developing of health information network at SP3. The organization Restructure on Government in Karawang Regency will give spirit to set up again the man for SP3 in Regency level and at Public Health Care level. To maximize the health information network at SP3 the writer suggests to Government Health Service of Karawang Regency in order to maximize SP3 application by computer base information network system of course are needed the increase and the addition of operators, computer technicians, and the increase of computer quality itself. And to other researchers, the writer expects the existence of continuous researches about SP3 software products for Public Health Care which has access straightly to regency and province level.

Kesehatan dan Keselamatan Kerja (K3) merupakan aspek krusial dalam menjamin perlindungan tenaga kerja, khususnya di lingkungan laboratorium yang memiliki tingkat risiko tinggi terhadap paparan bahan mikrobiologi, bahan Kimia maupun lingkungan. Penelitian ini bertujuan untuk merancang sistem informasi K3 yang dapat diterapkan di Balai Besar Biomedis dan Genomika Kesehatan (BB Binomika) Kementerian Kesehatan Indonesia guna mendukung pengelolaan manajemen biorisiko dan K3 secara efektif dan efisien. Permasalahan utama yang diidentifikasi adalah pengelolaan data K3 yang masih dilakukan secara manual, sehingga menghambat proses monitoring dan pengambilan keputusan. Penelitian ini menggunakan pendekatan Software Development Life Cycle (SDLC) dengan metode prototyping untuk merancang sistem informasi yang mencakup identifikasi bahaya serta penilaian risiko kecelakaan kerja, pengelolaan kelengkapan APD, penyediaan pelatihan K3, Pengelolaan limbah laboratorium, serta pengelolaan insiden terkait kecelakaan kerja. Hasil dari penelitian ini berupa prototipe sistem informasi K3 yang dapat menjadi dasar pengembangan lebih lanjut. Implementasi sistem ini diharapkan dapat membantu pengelolaan K3 secara lebih komprehensif, efisien, dan efektif. Melalui penyediaan data real-time dan visualisasi dalam bentuk dashboard.
Occupational Health and Safety (OHS) is a crucial aspect in ensuring the protection of workers, especially in laboratory environments that have a high risk of exposure to microbiological materials, chemicals and the environment. This study aims to design an OHS information system that can be applied at the Center for Biomedical and Health Genomics (BB Binomika) of the Indonesian Ministry of Health to support effective and efficient management of biorisk and OHS. The main problem identified is the management of OHS data which is still done manually, thus hampering the monitoring and decision-making process. This study uses the Software Development Life Cycle (SDLC) approach with the prototyping method to design an information system that includes hazard identification and risk assessment of work accidents, management of completeness of PPE, provision of OHS training, laboratory waste management, and management of incidents related to work accidents. The results of this study are in the form of a prototype of an OHS information system that can be the basis for further development. The implementation of this system is expected to help OHS management more comprehensively, efficiently and effectively. Through the provision of real-time data and visualization in the form of a dashboard.

Indonesia sedang mengalami masalah Triple burden of malnutrition, namun aplikasi pemantauan gizi saat ini belum mengakomodir pengukuran antropometri oleh guru di sekolah. Selain itu, program Makan Bergizi Gratis (MBG) juga memerlukan sistem pengawasan keamanan pangan rutin berbasis Hazard Analysis Critical Control Point (HACCP) guna mencegah kasus keracunan. Penelitian ini bertujuan mengembangkan sistem informasi berbasis website untuk pencatatan status gizi siswa dan pelaporan keamanan pangan MBG di sekolah X, Kota Tangerang Selatan. Pengembangan sistem menggunakan pendekatan System Development Life Cycle (SDLC) dengan metode prototyping, melalui tahapan perencanaan, pembuatan sistem informasi, uji coba, dan perbaikan. Data kualitatif diperoleh melalui wawancara mendalam terhadap petugas UKS, pengawas gizi SPPG, dan ahli gizi puskesmas yang dipilih secara purposive sampling. Sistem yang dihasilkan mampu mengolah data antropometri siswa dan pelaporan keamanan pangan secara real-time. Output sistem menyediakan informasi status gizi (grafik dan spasial koordinat sekolah), narasi pelaporan HACCP, tes organopletik, serta fitur pemberian feedback harian terhadap MBG yang diterima sekolah.
Indonesia is facing a triple burden of malnutrition, yet current nutritional monitoring applications have not accommodated independent anthropometric measurements by school teachers. Additionally, the Makan Bergizi Gratis (MBG) program requires a routine food safety surveillance system based on Hazard Analysis Critical Control Point (HACCP) to prevent poisoning incidents. This study aims to develop a website-based information system for recording student nutritional status and reporting MBG food safety at X School, South Tangerang City. System development utilized the System Development Life Cycle (SDLC) approach with the prototyping method, encompassing planning, system creation, testing, and refinement phases. Qualitative data were obtained through in-depth interviews with UKS (School Health Unit) staff, SPPG nutritional supervisors, and public health center (Puskesmas) nutritionists selected via purposive sampling. The resulting system is capable of processing student anthropometric data and food safety reporting in real-time. The system's output provides nutritional status information (visualized through graphs and spatial school coordinates), HACCP reporting narratives, organoleptic tests, and a daily feedback feature regarding the MBG meals received by the school.

Latar Belakang : Hasil penelitian survey cepat status kesehatan gigi dan mulut tahun 2004 di Kabupaten Sumedang pada murid kelas VI, didapat prevalensi karies yaitu sebesar 64 %. Rata rata DMF-T sebesar 1,78. Nilai PTI sebesar 5,26 %. Target yang ingin dicapai Departemen kesehatan tahun 2010 adalah prevalensi karies 50 %. Hasil Evaluasi kegiatan UKGS tahun 2003, didapat bahwa puskesmas yang tidak melaporkan kegiatan UKGS sebesar 50 %. Penyebab dari masalah tersebut adalah kegiatan UKGS belum berjalan dengan baik.
Tujuan : Tujuan dari penelitian ini adalah mengembangkan basis data di puskesmas dan Dinas Kesehatan yang bermanfaat sebagai alat bantu dalam menghasilkan informasi yang dapat dijadikan acuan dalam menentukan kebijakan, perencanaan, monitoring dan evaluasi program dalam rangka menurunkan prevalensi karies dan meningkatkan cakupan UKGS.
Metoda : Metoda yang digunakan adalah metoda pendekatan System Development Life Cicle (SDLC) dengan cara observasi dan wawancara mendalam dengan informan.
Hasil : Pengembangan sistem dimulai dari penetapan kebutuhan sistem, pemodelan sistem serta penetapan perangkat lunak dan keras yang digunakan dengan harapan dapat menghasilkan informasi dalam menyusun perencanaan, monitoring dan evaluasi program UKGS.Sistem informasi UKGS dikembangkan dengan rancangan formulir input dan tampilan output yang berisi cakupan UKGS, Indikator, grafik dan Peta sebaran tenaga Kesehatan gigi di Kabupaten Sumedang.
Kesimpulan : Pelaksanaan sistem informasi ini agar berjalan dengan baik dan berkelanjutan membutuhkan komitmen dan kebijakan yang kuat dari penentu kebijakan, aturan yang jelas tentang organisasi pelaksana, motivasi yang kuat dari pelaksana dan dukungan dana yang berkesinambungan.

Background: The result from a quick survey on dental health at Sumedang district in 2004 shows that 64 % prevalency caries affected children from the 6th grade students, DMF-T average 1.78 and PTI value at 5,2 %. However, In 2010 the Departement of Health targetting prevalency caries < 50 %. Indeks DMF-T average < 1.00 and PTI > 50%. On the evaluation in report of UKGS activities in 2003, it is said that 50% of Puskesmas did not report their UKGS activities. The reason for this was because the expected UKGS activities was not running well.
Objective : The purpose of this research is to develop information data base in Puskesmas and Dinas Kesehatan, as a result will be useful as a guide line in making policy, planning, monitoring and evaluating program in effort to reduce the presentage of prevalency of caries. In addition increase UKGS objectivities.
Methodes: The method used in this research is called development life cycle system (SDLC) by observing and extensivelly interviewing the informant.
Results: In developing a system it should begin with an implication of required model and hardware and software. This system will be used to obtain information of arranging a plan, monitoring and evaluation of UKGS program. UKGS information system is developed with the input design form and output sheet that contains UKGS covarage, indicator, graphic and distribution map of dental health personnel in Sumedang District.
Conclution: Good and continuity of system implementation need the policy and commitment of decision maker, clear regulation of system organization, good motivation of the system operator and continuity of financial support.

Tujuan : Tujuan dari penelitian ini adalah mengembangkan basis data di puskesmas dan Dinas Kesehatan yang bermanfaat sebagai alat bantu dalam menghasilkan informasi yang dapat dijadikan acuan dalam menentukan kebijakan, perencanaan, monitoring dan evaluasi program dalam rangka menurunkan prevalensi karies dan meningkatkan cakupan UKGS.
Metoda : Metoda yang digunakan adalah metoda pendekatan System Development Life Cicle (SDLC) dengan cara observasi dan wawancara mendalam dengan informan.
Hasil : Pengembangan sistem dimulai dari penetapan kebutuhan sistem, pemodelan sistem serta penetapan perangkat lunak dan keras yang digunakan dengan harapan dapat menghasilkan informasi dalam menyusun perencanaan, monitoring dan evaluasi program UKGS.Sistem informasi UKGS dikembangkan dengan rancangan formulir input dan tampilan output yang berisi cakupan UKGS, Indikator, grafik dan Peta sebaran tenaga Kesehatan gigi di Kabupaten Sumedang.
Kesimpulan : Pelaksanaan sistem informasi ini agar berjalan dengan baik dan berkelanjutan membutuhkan komitmen dan kebijakan yang kuat dari penentu kebijakan, aturan yang jelas tentang organisasi pelaksana, motivasi yang kuat dari pelaksana dan dukungan dana yang berkesinambungan.

Background: The result from a quick survey on dental health at Sumedang district in 2004 shows that 64 % prevalency caries affected children from the 6th grade students, DMF-T average 1.78 and PTI value at 5,2 %. However, In 2010 the Departement of Health targetting prevalency caries < 50 %. Indeks DMF-T average < 1.00 and PTI > 50%. On the evaluation in report of UKGS activities in 2003, it is said that 50% of Puskesmas did not report their UKGS activities. The reason for this was because the expected UKGS activities was not running well.
Objective : The purpose of this research is to develop information data base in Puskesmas and Dinas Kesehatan, as a result will be useful as a guide line in making policy, planning, monitoring and evaluating program in effort to reduce the presentage of prevalency of caries. In addition increase UKGS objectivities.
Methodes: The method used in this research is called development life cycle system (SDLC) by observing and extensivelly interviewing the informant.
Results: In developing a system it should begin with an implication of required model and hardware and software. This system will be used to obtain information of arranging a plan, monitoring and evaluation of UKGS program. UKGS information system is developed with the input design form and output sheet that contains UKGS covarage, indicator, graphic and distribution map of dental health personnel in Sumedang District.
Conclution: Good and continuity of system implementation need the policy and commitment of decision maker, clear regulation of system organization, good motivation of the system operator and continuity of financial support.

Abstrak:
Suatu rancangan sistem informasi pelaporan monitoring dan evaluasi yangterkomputerisasi diciptakan untuk menggantikan sistem lama yang masih manual. Metodelogi menggunakan tahapan dalam metode System Development Life Cycle (SDLC) yang dimulai dari tahap analisis,perancangan database, perancangan interface hingga implementasi. Studiini dibatasi hanya sampai pada tahap desain sistem. Hasil pengembangansistem monitoring dan evaluasi pelaksanaan kegiatan dan realisasi anggaran kesehatan di Dinas Kesehatan Provinsi Bengkulu memudahkan penggunauntuk mendapat data alokasi anggaran, realisasi, capaian kegiatan , dan sisaanggaran.
