Abstrak: Skripsi ini membahas tentang rancangan pengembangan rekam medis di klinik Avenue 8 Dental Care. Klinik ini berada di dua lokasi berbeda dengan jenis pencatatan rekam medis berbeda, Klinik Jakarta menggunakan rekam medis elektronik dan klinik Bintaro menggunakan rekam medis manual. Penggunaan dua sistem yang berbeda antara dua klinik menciptakan dua database berbeda dan tidak saling terhubung sehingga dokumen rekam medis pasien menjadi terpisah diantara kedua sistem. Tujuan dari skripsi ini adalah membuat rancangan pengembangan rekam medis elektronik di klinik Avenue 8 Dental Care yang bermanfaat untuk meningkatkan mutu pelayanan di Klinik Avenue 8 Dental Care. Penelitian ini menggunakan metode kualitatif dan rancangan sistem dilakukan dengan menggunakan tahapan System Development Life Cycle (SDLC). Pengumpulan data dilakukan dengan melakukan wawancara mendalam dan observasi. Hasil akhir penelitian ini berupa rancangan rekam medis elektronik  di klinik Avenue 8 Dental Care terutama di klinik Bintaro yang masih menggunakan rekam medis manual.

The study discussed the design of the development of medical records at the Avenue 8 Dental Care clinic. The clinic is located in two different locations with different types of medical records, the Jakarta Clinic uses electronic medical records and the Bintaro clinic uses manual medical records. The use of two different systems between the two clinics creates two different databases and are not interconnected so that the patient's medical record documents become separate from the development of electronic medical records at the Avenue 8 Dental Care clinic that are useful for between the two systems. The purpose of this study is to design the development of electronic medical records at the Avenue 8 Dental Care Clinic that are useful to improve the quality of service at the Avenue 8 Dental Care Clinic. This research uses qualitative methods and system design is done using the System Development Life Cycle (SDLC) stage. Data collection is done by conducting in-depth interviews and observations. The final results of this study were in the form of electronic medical records at the Avenue 8 Dental Care clinic, especially at the Bintaro clinic, which still uses manual medical records.

Abstrak: Rekam medis merupakan sumber informasi kesehatan yang memerlukan manajemen data yang baik, informasi merupakan hal yang penting dalam penentuan kebijakan atau pengambilan keputusan. Pengelolaan rekam medisrawat jalan di UPT Puskesmas Kecamatan Cileungsi masih manual yang mengakibatkan banyak keterbatasan dan permasalahan. Permasalahan ini mengakibatkan ketidak mampuan mengolah data menjadi informasi yang dibutuhkan dengan cepat dan akurat. Tujuan penelitian adalah membuat rancangan sistem informasi rekam medis elektronik rawat jalan yang dapat memudahkan pengolahan data untuk dibuat menjadi informasi yang dibutuhkan. Pengembangan sistem informasi menggunakan metode Incremental dan Iterative,pengumpulan data dengan cara observasi, telaah dokumen dan wawancara.Penelitian ini menghasilkan desain logis sistem informasi rekam medis elektronikrawat jalan sesuai kebutuhan pengguna yang dapat menghasilkan informasi secaracepat dan akurat. Kecepatan arus informasi di puskesmas sangat membantu manajemen dalam pengambilan keputusan dan menjalankan fungsi perencanaan,monitoring dan evaluasi. Sistem informasi ini dapat di implementasikan dengan beberapa kondisi yang dapat menunjang terselenggaranya sistem dengan baik,yaitu tersedianya perangkat keras dan perangkat lunak sesuai dengan kebutuhansistem informasi yang dikembangkan secara bertahap, pelatihan petugas serta dukungan manajemen berupa dana, kebijakan dan prosedur. Medical record is a source of health information management that requires gooddata, information is crucial in the determination of policy or decision making.Outpatient medical record management in UPT Puskesmas Kecamatan Cileungsimanuals still lead to many limitations and problems. This problem resulted in theinability of the processing of data into information you need quickly andaccurately. The purpose of the research is to make the design of electronicmedical record information system outpatient which can facilitate the processingof data to be made into the needed information. Information systems developmentusing Iterative, Incremental method and data collection by means of observation,review of documents and interviews. This research resulted in the logical designof the information system of the electronic medical record outpatient, accordingthe needs of users who are able to generate information quickly and accurately.Speed of information flow in primary health care help management in decisionmaking and running the functions of planning, monitoring and evaluation. Thisinformation system can be implemented with some conditions that can supportthis system properly, namely the availability of hardware and software accordingto the needs of information systems developed gradually, officer training andmanagement support in the form of funds, policies and procedures.Keywords: Electronic; Information; Medical Record; Primary Health Care;Systems.
