Abstrak:
Kerentanan rumah sakit di Indonesia dalam menghadapi bencana terlihat dari rata-rata indeks Hospital Emergency Disaster Management yang rendah (0,408) dan kegagalan infrastruktur fisik (hard resilience) saat beban puncak (surge capacity), seperti pada pandemi COVID-19 dan gempa bumi. Modal sosial (social capital) berpotensi menjadi "infrastruktur organisasi non-fisik" (soft resilience) yang mampu mensubstitusi keterbatasan fiskal dan birokrasi, namun pengoptimalannya belum terformulasikan secara terstruktur dalam manajemen bencana rumah sakit. Penelitian ini bertujuan untuk menganalisis, menyintesis, dan merekonstruksi Model Social capital melalui integrasi perspektif strategis (makro) dan operasional (mikro) guna meningkatkan kesiapsiagaan dan ketahanan rumah sakit di wilayah berisiko bencana tinggi (Provinsi Jawa Barat). Penelitian ini menggunakan desain Exploratory Sequential Mixed Methods. Tahap kualitatif melibatkan 18 informan kunci (praktisi senior dan pakar manajemen bencana) untuk mengidentifikasi 78 indikator modal sosial. Tahap kuantitatif melibatkan 110 responden dari 5 tipologi rumah sakit di Jawa Barat (RS Vertikal, RSUD, dan RS Swasta) yang dianalisis menggunakan Structural Equation Modeling (SEM-PLS). Kualitatif Tahap 2 pada 5 RS mewawancara 34 informan. Tahap akhir adalah rekonstruksi model melalui validasi konsensus ahli. Analisis SEM-PLS menunjukkan bahwa Modal Sosial berpengaruh positif dan signifikan terhadap Kesiapsiagaan (0,872) dan Ketahanan (0,672), dengan nilai R-Square Ketahanan sebesar 85,2% (kategori sangat kuat). Penelitian ini memetakan tiga lapis kapital: 1) Bonding Capital (Internal RS) berbasis organizational trust dan nilai spiritual ("Employee of Allah"); 2) Bridging Capital (Horizontal) melalui jejaring lintas faskes/organisasi profesi berbasis resiprositas; dan 3) Linking Capital (Vertikal) melalui agile governance dengan otoritas (Kemenkes/BPBD). Temuan menunjukkan adaptasi kontekstual yang khas: RS Vertikal mengandalkan bridging untuk rujukan makro; RSUD memanfaatkan kearifan lokal "Saduluran" untuk substitusi fiskal daerah; dan RS Swasta Filantropi menggunakan kekuatan identitas sosial untuk pertahanan bottom-up yang lincah. Model Social capital merupakan strategi optimasi non-fiskal yang mentransformasikan jejaring manusia menjadi kapasitas resiliensi nyata. Model ini merekomendasikan integrasi elemen penilaian modal sosial dalam Standar Akreditasi Rumah Sakit (Bab MFK) dan penguatan kurikulum sosiologi kebencanaan pada pendidikan manajemen kesehatan untuk memutus siklus repitisi kesalahan manajemen krisis.

The vulnerability of hospitals in Indonesia in dealing with disasters can be seen from the low average Hospital Emergency Disaster Management index (0.408) and the failure of physical infrastructure (hard resilience) during peak loads (surge capacity), such as in the COVID-19 pandemic and earthquakes. Social capital has the potential to become a "non-physical organizational infrastructure" (soft resilience) that is able to substitute fiscal and bureaucratic limitations, but its optimization has not been formulated in a structured manner in hospital disaster management. This research aims to analyze, synthesize, and reconstruct the Social capital Model through the integration of strategic (macro) and operational (micro) perspectives to improve hospital preparedness and resilience in high-risk disaster areas (West Java Province). This study uses the design of Exploratory Sequential Mixed Methods. The qualitative stage involved 18 key informants (senior practitioners and disaster management experts) to identify 78 social capital indicators. The quantitative stage involved 110 respondents from 5 hospital typologies in West Java (Vertical Hospitals, Hospitals, and Private Hospitals) which were analyzed using Structural Equation Modeling (SEM-PLS). Qualitative Phase 2 at 5 hospitals interviewed 34 informants. The final stage is the reconstruction of the model through expert consensus validation. The SEM-PLS analysis shows that Social capital has a positive and significant effect on Preparedness (0.872) and Resilience (0.672), with an R-Square value of Resilience of 85.2% (very strong category). This study maps three layers of capital: 1) Bonding Capital (Internal RS) based on organizational trust and spiritual values ("Employee of Allah"); 2) Bridging Capital (Horizontal) through a network of reciprocity-based health facilities/professional organizations; and 3) Linking Capital (Vertical) through agile governance with authorities (Ministry of Health/BPBD). The findings show typical contextual adaptations: Vertical RS relies on bridging for macro references; The hospital utilizes the local wisdom of "Duluran" for regional fiscal substitution; and Philanthropy Private Hospitals use the power of social identity for nimble bottom-up defenses. The social capital model is a non-fiscal optimization strategy that transforms human networks into real resilience capacity. This model recommends the integration of social capital assessment elements in Hospital Accreditation Standards (MFK Chapter) and strengthening the disaster sociology curriculum in health management education to break the cycle of repetition of crisis management errors.

Abstrak:

Keselamatan pasien merupakan kewajiban rumah sakit dan bagian integral dari akreditasi sejak 2008. Namun, berbagai permasalahan masih sering ditemukan, sehingga keberlanjutan perbaikan menjadi tantangan. Penelitian ini bertujuan merumuskan model konseptual strategi peningkatan keselamatan pasien. Penelitian menggunakan pendekatan mixed method dengan desain convergent parallel. Data kuantitatif berasal dari Riset Fasilitas Kesehatan 2019 (523 rumah sakit) dan data akreditasi (917 rumah sakit), dianalisis menggunakan uji chi-square, regresi logistik, dan analisis jalur. Data kualitatif dikumpulkan melalui wawancara mendalam dan telaah dokumen dari enam rumah sakit, dinas kesehatan provinsi, dan Perhimpunan Rumah Sakit Seluruh Indonesia (PERSI) wilayah di Sumatera Utara dan Bali, dengan total 95 informan. Analisis tematik menggunakan perangkat NVivo, dengan kerangka Malcolm Baldrige dan model implementasi Van Meter-Van Horn, meliputi ukuran dan tujuan kebijakan, sumber daya, kepemimpinan, perencanaan strategis, fokus tenaga kerja, fokus operasi, fokus pelanggan, pengukuran, analisis, dan manajemen pengetahuan, komunikasi antar organisasi, serta peran akreditasi. Hasil kuantitatif menunjukkan bahwa pelaporan insiden keselamatan pasien berhubungan signifikan dengan lokasi (Jawa-Bali), status akreditasi, jumlah tempat tidur (> 200), kelas rumah sakit (A dan B), evaluasi pelayanan, audit internal, serta keaktifan komite keselamatan pasien dan pengendalian infeksi. Hasil kualitatif menunjukkan bahwa implementasi kebijakan keselamatan pasien sudah berjalan, namun bervariasi tergantung kepemilikan dan ketersediaan sumber daya. Semua dimensi yang diteliti berpotensi menjadi faktor pendukung maupun penghambat tergantung pengelolaannya. Kepemimpinan yang kuat, fasilitas yang memadai, serta budaya keselamatan yang ditanamkan secara konsisten memperkuat implementasi, sedangkan lemahnya komitmen dan keterbatasan dana menjadi kendala. Hambatan juga muncul dalam pelaporan insiden, baik dari sisi organisasi maupun individu. Penelitian ini menghasilkan model konseptual strategi peningkatan keselamatan pasien yang mencakup integrasi keselamatan pasien dalam perencanaan strategis, penguatan kepemimpinan, peningkatan kapasitas staf, alokasi anggaran memadai, monitoring dan evaluasi berkelanjutan, serta pelibatan pasien. Model ini diharapkan dapat mendorong peningkatan keselamatan pasien secara menyeluruh dan berkelanjutan di rumah sakit.


 

Patient safety is a mandatory obligation for hospitals and has been an integral part of hospital accreditation since 2008. However, various patient safety issues are still frequently found, making the sustainability of improvements a major challenge. This study aims to formulate a conceptual model of patient safety improvement strategies. A mixed-methods approach with a convergent parallel design was employed. Quantitative data were obtained from the 2019 Rifaskes (523 hospitals) and accreditation records (917 hospitals), and analyzed using chi-square tests, logistic regression, and path analysis. Qualitative data were collected through in-depth interviews and document reviews from six hospitals, provincial health offices, and the Indonesian Hospital Association (PERSI) in North Sumatra and Bali Provinces, involving a total of 95 informants. Thematic analysis was conducted using NVivo software, guided by the Malcolm Baldrige framework and the Van Meter–Van Horn policy implementation model. Quantitative findings showed that the reporting of patient safety incidents was significantly associated with location (Java–Bali), accreditation status, bed capacity (>200 beds), hospital class (A and B), presence of service evaluations, internal audits, and the activity of patient safety and infection control committees. Qualitative results indicated that while policy implementation was underway, it varied depending on hospital ownership and available resources. All dimensions could act as either enablers or barriers depending on how they were managed. Strong leadership and adequate facilities enhanced implementation, while weak commitment and limited funding were key constraints. Incident reporting also faced challenges at both organizational and individual levels. This study produced a conceptual model for improving patient safety through the integration of safety into strategic planning, strengthened leadership, staff capacity building, sufficient budget allocation, continuous monitoring and evaluation, and enhanced patient engagement. The model is expected to support comprehensive and sustainable patient safety improvements in hospitals

Abstrak:

Ketersediaan obat esensial menjadi tantangan saat menghadapi Pandemi COVID-19 di Indonesia. Evaluasi JEE (Joint External Evaluation) 2023 menunjukkan koordinasi lintas sektor masih lemah dalam mencegah, mendeteksi dan menanggapi keadaan darurat kesehatan. Tahun 2021 sebanyak 30% kabupaten/kota belum memenuhi target 85% obat esensial tersedia. Tingkat ketersediaan ini dipengaruhi oleh implementasi kebijakan nya. Penelitian ini bertujuan menganalisis kesiapsiagaan pemerintah daerah dalam menjamin ketersediaan obat esensial pada masa pandemi dan membangun model konseptual resiliensinya.

Penelitian ini dilakukan dengan pendekatan kualitatif di Provinsi Jawa Barat, Kota Depok dan Kabupaten Bogor, serta di Provinsi Bali, Kota Denpasar dan Kabupaten Badung. Pengumpulan data wawancara mendalam, observasi dan data laporan dari instansi. Instrumen wawancara telah divalidasi dengan nilai I-CVI >0,83. Implementasi kebijakan dan penyusunan model resiliensi pemerintah daerah di analisis menggunakan model Van Meter Van Horn.

Kesiapsiagaan pemerintah daerah saat Pandemi COVID-19 pada awalnya belum siap namun dengan adaptasi dan peningkatan kapasitas telah lebih siap. Semua aktor telah menjalankan perannya sehingga tingkat ketersediaan obat esensial indikator selama pandemi COVID-19 memenuhi target capaian indikator di puskesmas maupun kabupaten/kota tahun 2020-2022. Faktor-faktor yang mendukung tingkat ketersediaan obat esensial indikator adalah adanya dukungan pemerintah daerah, dukungan kebijakan, kepemimpinan, ketersediaan anggaran BTT, kolaborasi lintas sektor dan ketepatan perencanaan obat. Faktor-faktor penghambat antara lain terbatas anggaran daerah, SDM, sarana prasarana, ketergantungan industri terhadap bahan baku impor, SIM belum terintegrasi dan ego-sektoral. Terdapat 37 indikator yang terbagi 22 indikator utama dan 15 pendukung pada dimensi ukuran dan tujuan kebijakan, sumber daya, komunikasi antar organisasi, karakteristik badan pelaksana, lingkungan sosial, ekonomi dan politik serta disposisi pelaksana yang dapat membangun resiliensi pemerintah daerah dalam mejamin ketersediaan obat esensial. Pemerintah daerah perlu meningkatkan kapabilitas SDM, penguatan surveillance obat, pemastian buffer stock, kolaborasi lintas sektor, SIM terintegrasi, serta penerapan kebijakan BLUD puskesmas dapat meningkatkan implementasi kebijakan pemerintah daerah dalam menjamin ketersediaan obat esensial.


The availability of essential medicines has been a challenge during the COVID-19 pandemic in Indonesia. The 2023 Joint External Evaluation (JEE) showed that inter-sectoral coordination remains weak in preventing, detecting, and responding to health emergencies. In 2021, 30% of districts/cities had not met the target of 85% availability of essential medicines. This availability rate is influenced by the implementation of policies. This study aims to analyze the preparedness of regional governments to ensure the availability of essential medicines during the pandemic and to develop a conceptual model of their resilience. This study used a qualitative approach in West Java Province, Depok City, Bogor District, Bali Province, Denpasar City, and Badung District. Data collection involved in-depth interviews, observations, and reports from relevant agencies. The interview instrument was validated with an I-CVI value >0.83. The Van Meter Van Horn model analyzed policy implementation and the development of regional government resilience models. Regional government preparedness during the COVID-19 pandemic was initially inadequate, but through adaptation and capacity building, it has improved. All actors have fulfilled their roles, ensuring that the availability of essential medicines during the COVID-19 pandemic met the target indicators at health centers and districts/cities from 2020 to 2022. Factors supporting the availability of essential medicines include regional government support, policy support, leadership, availability of Unexpected Costs Unexpected Costs budget, cross-sectoral collaboration, and accurate medicine planning. Hindering factors include limited local budgets, human resources, infrastructure, industry dependence on imported raw materials, unintegrated SIM, and sectoral egoism. There are 37 indicators divided into 22 main indicators and 15 supporting indicators in the dimensions of policy size and objectives, resources, inter-organizational communication, characteristics of implementing agencies, social, economic, and political environment, and the disposition of implementers that can build the resilience of regional governments in ensuring the availability of essential medicines. Regional governments need to enhance human resource capabilities, strengthen drug surveillance, ensure buffer stock, promote cross-sectoral collaboration, integrate SIM, and implement Regional Public Service Agency policies for community health centers to improve the implementation of regional government policies in ensuring the availability of essential medicines.

Abstrak: Kelelahan kerja berdampak besar terhadap kinerja, keselamatan, dan kesehatan tenaga kesehatan rumah sakit. Penelitian ini mengembangkan model sistem manajemen risiko kelelahan berbasis pendekatan kualitatif dan kuantitatif melalui studi literatur, FGD, wawancara, dan observasi. Hasilnya adalah model ICHAFIT (Integrated Collaboration Healthcare Adaptability for Fatigue Intervention and Tracking) dengan lima elemen utama dan strategi pencegahan berbasis data. Terdapat 24 indikator valid dan reliabel untuk menilai implementasinya. Model ini berfungsi sebagai kerangka konseptual dan alat praktis, serta menghasilkan policy brief untuk advokasi kebijakan nasional.
Work fatigue significantly affects hospital workers' performance, safety, and health. This study developed a fatigue risk management model using qualitative and quantitative approaches through literature review, FGD, interviews, and observations. The result is the ICHAFIT model (Integrated Collaboration Healthcare Adaptability for Fatigue Intervention and Tracking) comprising five key elements and a data-driven prevention strategy). It includes 24 valid and reliable indicators to assess implementation. ICHAFIT serves as both a conceptual framework and practical tool, and also produced a policy brief to support national advocacy for fatigue risk management in hospitals.

Abstrak: Tuberkulosis masih merupakan masalah kesehatan masyarakat yang utama di dunia, termasuk di Indonesia. Menemukan dan menyembuhkan pasien merupakan cara terbaik dalam upaya pencegahan penularan TB, dengan menerapkan strategi DOTS. Sejalan dengan kebijakan pembangunan nasional, pelaksanaan strategi pengendalian TB nasional diprioritaskan pada daerah terpencil, perbatasan dan kepulauan terutama yang belum memenuhi target penemuan kasus dan keberhasilan pengobatan. Terdapat lima provinsi dengan TB paru tertinggi dan dua tertinggi yaitu Provinsi Jawa Barat (0.7%), Papua (0.6%). Akses pelayanan kesehatan pasien TB menunjukan ketidakmerataan, dimana hanya ada di wilayah perkotaan dan berada pada ekonomi tinggi. Permasalahan dalam penelitian ini adalah masih ditemukan pasien TB yang tidak mendapatkan akses pelayanan kesehatan. Keterbatasan akses pelayanan kesehatan pasien TB dapat disebabkan oleh kondisi individu yang berbeda-beda serta adanya perbedaan kondisi fisik (geografis). Tujuan penelitian ini adalah mendapatkan model spasial akses pelayanan kesehatan di provinsi Jawa Barat dan Papua.
 
Penelitian ini menggunakan desain potong lintang dan menggunakan data yang berasal dari Riset Kesehatan Dasar 2013. Lokasi penelitian di 2 provinsi yaitu di provinsi Jawa Barat dan provinsi Papua. Analisis penelitian dengan menggunakan regresi logistik untuk melihat pengaruh karakteristik individu terhadap akses pelayanan kesehatan dan analisis spasial statistik menggunakan Geographically Weighted Regression (GWR) untuk melihat spasial akses pelayanan kesehatan. Akses pelayanan kesehatan adalah pasien TB yang melakukan pemeriksaan dahak, foto rontgen dan mendapatkan obat anti TB.
 
Akses pelayanan kesehatan pasien TB di provinsi Papua masih rendah. Karakteristik individu yang mempengaruhi akses pelayanan kesehatan adalah asuransi kesehatan, pekerjaan, menikah, mengetahui ketersediaan fasilitas kesehatan. Model spasial akses pelayanan kesehatan menghasilkan dua jenis variabel pembentuknya, yaitu adanya variabel lokal dan variabel global. Variabel lokal adalah variabel yang mempunyai pengaruh unsur kewilayahannya terhadap akses pelayanan kesehatan, sedangkan variabel global merupakan variabel yang berpengaruh di tingkat provinsi.
 
Masih rendahnya pasien TB yang melakukan akses pelayanan dapat disebabkan oleh sulitnya pasien TB dalam mencapai fasilitas kesehatan, terutama di wilayah dengan perbedaan geografis. Sehingga perlunya ada kebijakan dalam menyiapkan sarana dan prasarana kesehatan pasien TB, yaitu dengan mulai memasukan tenaga kesehatan terlatih di bidang tuberkulosis pada seluruh fasilitas pelayanan kesehatan.
 

Tuberculosis is a major public health problem in the world, including in Indonesia. Finding and curing the patients are the best way of preventing transmission of TB by implementing the DOTS strategy. Implementation of the national TB control strategy prioritized in remote, border and island especially TB patients who do not meet the target case detection and treatment success. There are two of provinces with the highest and second highest TB namely west Java province (0.7%) and Papua (0.6%). Accessibility to health services of TB patients showed inequality, which only exist in urban areas and at high economic status. The problem in this research is find the of TB patients who do not get accessibility to health services. Limited accessibility to health services of TB patients could be caused by conditions different individuals as well as differences in physical conditions (geographic). The purpose of this study is to setup a spatial model of accessibility to health services in the province of West Java and Papua.
 
This study used a cross-sectional design and data derived from the Basic Health Research in 2013 (RISKESDAS). Research sites in the provinces of West Java and Papua. Research analysis applied logistic regression to determine the effect of individual characteristics of accessibility to health services and statistical spatial analysis using the Geographically Weighted Regression (GWR) for a model of spatial accessibility to health services.
 
Accessibility to health care is the patient of TB sputum examination, x-rays and getting anti-TB. Accessibility to health services of TB patients in the province of Papua remains low. Individual characteristics that affect accessibility to health care are health insurance, employment, marriage, the availability of health facilities. Spatial models of accessibility to health services generate two types of constituent variables, the local variables and global variables. Local variables are variables that influence the spatial element of accessibility to health services, while global variables are variables that influence at the provincial level.
 
The low TB patients who do accessibility services may be caused by the difficulty in the of TB patients to health facilities, especially in the areas with geographical differences. Thus the need for a policy in preparing health facilities TB patients, i.e. to start entering trained health personnel in the field of tuberculosis in the entire health care facility.

Abstrak:
Latar belakang: Kemudahan akses pelayanan kesehatan bagi peserta Jaminan Kesehatan Nasional (JKN) merupakan salah satu upaya untuk meningkatkan derajat kesehatan penduduk Indonesia. Kemudahan akses ini terwujud dengan bertambahnya fasilitas kesehatan yang melayani peserta JKN. Indikator kemudahan akses terlihat dari bertambahnya jumlah peserta yang berkunjung ke fasilitas kesehatan baik di tingkat pelayanan rawat jalan maupun rawat inap. Kunjungan peserta JKN per 1.000 penduduk dikenal dengan isitilah rate sebagai salah satu indikator utilisasi pelayanan kesehatan untuk menjaga kesinambungan program JKN. Tujuan: penelitian ini bertujuan menganalisis faktor yang mempengaruhi rate rawat jalan tingkat lanjutan (RJTL) maupun rawat inap inap tingkat lanjutan (RITL) dan pemodelan prediksi rate RJTL dan rate RITL. Data yang digunakan berasal dari database BPJS Kesehatan dan Survei Sosial Ekonomi Nasional (SUSENAS) tahun 2016 – 2019 yang diolah berdasarkan faktor predisposing, faktor enabling, dan faktor need dimana semua data digunakan dalam penelitian atau total sampling. Metode: analisis data panel dinamis yang ditujukan untuk membuat model prediksi rate RJTL dan rate RITL. Hasil: model prediksi yang digunakan pada rate RJTL dan rate RITL adalah estimator First Difference Generalized Method of Moment (FDGMM). Kesimpulan: rate RJTL dipengaruhi oleh variabel nilai tagihan klaim dibayar per kunjungan RJTL; jumlah rumah sakit kelas A, B, C, D; jumlah peserta pria; jumlah peserta berusia > 50 tahun; jumlah peserta dengan jumlah anggota keluarga > 3 orang; jumlah peserta berpengeluaran di bawah garis kemiskinan; jumlah peserta dengan penyakit tidak menular; rasio fragmentasi; rasio rujukan; dan jumlah peserta berpendidikan SMP. Sedangkan, rate RITL dipengaruhi oleh variabel nilai tagihan klaim dibayar per kunjungan RITL; jumlah rumah sakit kelas A, B, C, D; jumlah peserta pria; jumlah peserta berusia > 50 tahun; jumlah peserta dengan jumlah anggota keluarga > 3 orang; rate readmisi; jumlah peserta berpendidikan SMP; dan jumlah peserta berpendidikan Perguruan Tinggi. Saran: hasil penelitian menyarankan agar Pemerintah Daerah turut mendukung pemenuhan sarana prasarana pelayanan kesehatan agar masyarakat dapat menjangkau pelayanan kesehatan dengan mudah, mengelola perencanaan penambahan rumah sakit sesuai kebutuhan; Kementerian Kesehatan dapat memberikan regulasi terkait pemenuhan dan pemerataan fasilitas kesehatan maupun tenaga medis, terutama pada daerah dengan keadaan geografis yang sulit; BPJS Kesehatan dapat menggunakan model prediksi rate RJTL dan rate RITL sebagai alat bantu dalam menilai kebutuhan penambahan kerjasama dengan rumah sakit. Peneliti selanjutnya dapat melakukan penelitian dengan faktor utilisasi yang lebih luas dan lengkap serta melakukan kajian yang lebih mendalam pada satu wilayah tertentu dengan mempertimbangkan pengaruh aspek geografis, seperti jarak antar fasilitas kesehatan, luas wilayah dan kondisi akses ke fasilitas kesehatan.

Background: easy access to health services for participants of the National Health Insurance (JKN) is one of the efforts to improve the health status of the Indonesian population. This accessibility is achieved through an increase in health facilities serving JKN participants. The indicator of accessibility can be observed from the rising number of participants visiting health facilities, both at the outpatient and inpatient levels. The rate of visits by JKN participants per 1.000 population is considered an indicator of health service utilization, which contributes to the continuity of the JKN program. Objective: this study aims to analyze the factors that influence the advanced level of outpatient care (RJTL) and inpatient care (RITL) and to model the prediction of RJTL rates and RITL rates. The data used is derived from the BPJS Kesehatan database and the 2016-2019 National Socioeconomic Survey (SUSENAS), which are processed based on predisposing factors, enabling factors, and need factors. All data is utilized in the research, employing total sampling. Method: dynamic panel data analysis is employed to develop prediction models for RJTL rates and RITL rates. Results: the prediction model used for the RJTL rate and RITL rate is the First Difference Generalized Method of Moment (FDGMM) estimator. Conclusion: RJTL rate is influenced by several variables: value of claims bills paid per RJTL visit, number of class A, B, C, and D hospitals, number of male participants, number of participants aged over 50 years, number of participants with more than 3 family members, number of participants with expenditures below the poverty line, number of participants with non-communicable diseases, fragmentation ratio, referral ratio, and number of participants with junior high school education. On the other hand, the RITL rate is affected by value of claim bills paid per RITL visit, number of class A, B, C, and D hospitals, number of male participants, number of participants aged over 50 years, number of participants with more than 3 family members, readmission rate, number of participants with junior high school education, and number of participants with university education. Recommendations: the results of this study suggest that the Regional Government should also support the fulfillment of health service infrastructure so that partisipant can reach health services easily, manage plans for adding hospitals as needed; The Ministry of Health can provide regulations regarding the fulfillment and equity of health facilities and medical personnel, especially in areas with difficult geographical conditions; BPJS Kesehatan can use RJTL rate prediction model and RITL rate as a tool in assessing the need for additional collaboration with hospitals. Future researchers can conduct research with broader and more complete utilization factors and conduct more in-depth studies in a particular area by considering the influence of geographical aspects, such as the distance between health facilities, area size and conditions of access to health facilities.
