Penelitian ini berangkat dari tingginya angka keluhan nyeri punggung bawah (low back pain/ LBP) para tenaga perawat di 4 bagian: ICU, IGD, Kamar Operasi dan Anesthesi Rumah Sakit Pusat Pertamina (RSPP) Jakarta, serta dukungan data penelitian tentang LBP dari luar negeri. Disinyalir sebagai penyebab adalah pola kerja keperawatan, beberapa pertimbangan selanjutnya penelitian diarahkan pada bagian ICU-A dengan alasan mendasar pada bagian ini terdapat kasus LBP yang signifkan. LBP disebabkan adanya kerusakan organ tubuh, telah terjadi kecelakaan samar atau cedera traumatik yang berulang dan akumulatif. Keluhan LBP sangat bersifat universal dan subyektif, nilai subyektifitas dapat ditingkatkan menjadi nilai obyektif. Agar keluhan tersebut dapat menjadi nilai obyektif maka perlu bukti pengukuran. Jastifikasi pengukuran didasari pada prinsip ergonomi dengan metode pendekatan psikofisik RPE (Rating of Perceived Exertion) akan menghasilkan persepsi beban kerja, dilengkapi pengukuran sampling pekerjaan yang akan menghasilkan performen kerja. Hasil ukur keduanya akan dibandingkan untuk meyakinkan kebenaran hasil ukurnya. Performen kerja menjadikan dasar ukur tingkat risiko LBP dengan metode perhitungan risiko kualitatif FLAME Model dan rekomendasi angkat NIOSH. Acuan dasar penelitian ini adalah tugas tenaga perawat pada bagian ICU-A yang terbagi elemen kerja sebagai input dalam kerangka pikir penelitian. Hasil pengukuran performen kerja perawat pada bagian ICU-A menunjukkan tingkat kerja produktif cukup tinggi (87,9%), persepsi beban kerja memperlihatkan tingkat sangat tinggi dengan konsekuensi tingkat risiko LBP yang tinggi, terutama elemen kerja mengangkat cara manual dengan beban diatas 15 Kg. Situasi pola kerja demikian sangatlah bertentangan dengan prinsip ergonomi "Fitting the task for the human", selain pertimbangan yang lebih mendasar bahwa perawat adalah aset perusahan yang perlu perlindungan terhadap segala risiko akibat kerja. Elemen kerja postur janggal dengan tangan diatas lutut dan mendorong atau menarik obyek perlu mendapat perhatian khusus karena kemungkinan besar mengandung risiko LBP. Solusi jawaban masalah ini terdapat pada teori siklus perbaikan kerja ergonomi, pelaksanaannya perlu inovasi perbaikan terhadap sistem kerja dengan pola pelaksanaan yang tidak boleh berhenti perlu kontinyunitas, sampai ditemukan pola kerja yang aman terhadap LBP. Muatan manajemen risiko menjadi prioritas dalam perlindungan tenaga perawat terhadap LBP.
This research is inspired by high symptom of low back pain (LBP) suffered by nurses at four departments: ICU, IGD, Surgery Rooms and Anesthesia at Pertamina Central Hospital Jakarta and supported by observation data on LBP from abroad. It is hypothesized that the reason of the pain is the working pattern of nurses. Further consideration on this research is focused on the ICU-A Department with the reason that it is found significant LBP cases at this department. Body's organ disorder and obscure accident or continuous and cumulative traumatic injury causes LBP. Complaint on the LBP is universal and subjective in nature; its subjectivity value can be increased to objectivity value. It is required a measurement to make the complaint objective. Measurement justification based on the ergonomic principle under Rating of Perceived Exertion (RPE) the psychophysical approach will result in working burden perception and measurement of job sampling produces working performance. Result of both measurements will be compared one another to assure of its accuracy. Working performance becomes the measurement basis of the LBP risk rate under FLAME Model qualitative risk calculation method and NIOSH recommendation. The basic reference of this research is the duties of nurses at ICU-A Department consisting of input working element in the research framework. The result of the working performance of nurses at ICU-A Department reveals that a sufficiently high productivity (87.9%), high working load perception resulting in high risk of LBP, particularly working element of manual lifting work of 15 kg load. The working situation is greatly in the contrary to the ergonomic principle "Fitting the task for the human", in addition to a more basic consideration that nurses is the company's asset which needs to be protected against all risks of work. Working element of improper body position with arm put on the knees and pushing or pulling object requires further special attentions because they attribute to the risk of LBP. The solution to this problem is contained in the theory on the improvement of the working ergonomics, the implementation of which requires innovative improvement of the working system on a continuous basis until the formulation of a safe working pattern toward LBP. Risk management shall become a high priority in the protection of nurses against LBP.

Low Back Pain merupakan keluhan muskuloskeletal yang sering dialami perawat akibat paparan risiko ergonomi. Penelitian ini bertujuan menganalisis gambaran keluhan LBP serta hubungan faktor risiko fisik, psikososial, dan individu terhadap LBP pada perawat di Rumah Sakit X. Penelitian menggunakan desain cross-sectional dan teknik purposive sampling dengan jumlah sampel 72 perawat. Keluhan LBP diukur menggunakan Nordic Musculoskeletal Questionnaire (NMQ), faktor fisik dengan QEC (postur tidak netral, manual handling, beban kerja, durasi aktivitas kerja), dan faktor psikososial dengan ENSS (Stres kerja dan kurangnya dukungan sosial), faktor individu dengan kuesioner karakteristik individu (usia, jenis kelamin, aktivitas fisik, dan BMI). Analisis dilakukan menggunakan uji Chi-square dan perhitungan Odds Ratio (OR). Hasil menunjukkan prevalensi LBP dalam 7 hari terakhir sebesar 38,9%. Sebagian besar variabel tidak berhubungan signifikan dengan LBP. Namun, beban kerja berhubungan signifikan dengan LBP (p=0,034), dengan risiko meningkat pada beban kerja sedang (OR=2,29), tinggi (OR=5,2), dan sangat tinggi (OR=18). Dengan demikian, rumah sakit perlu mengembangkan program pencegahan LBP yang berfokus pada pengendalian beban kerja serta disesuaikan dengan kondisi kerja dan karakteristik individu perawat.
Low back pain is a musculoskeletal complaint frequently experienced by nurses due to exposure to ergonomic risks. This study aims to analyze the characteristics of low back pain complaints and the relationship between physical, psychosocial, and individual risk factors and low back pain among nurses at Hospital X. The study used a cross-sectional design and purposive sampling technique, with a sample size of 72 nurses. LBP complaints were measured using the Nordic Musculoskeletal Questionnaire (NMQ), physical factors using the QEC (non-neutral posture, manual handling, workload, duration of work activity), psychosocial factors using the ENSS (work stress and lack of social support), and individual factors using the individual characteristics questionnaire (age, gender, physical activity, and BMI). Analysis was performed using the Chi-square test and Odds Ratio (OR) calculation. The results showed a prevalence of low back pain (LBP) in the past 7 days of 38.9%. Most variables were not significantly associated with LBP. However, workload was significantly associated with LBP (p=0.034), with the risk increasing with moderate (OR=2.29), high (OR=5.2), and very high (OR=18) workloads. Therefore, hospitals need to develop LBP prevention programs that focus on workload management and are tailored to the working conditions and individual characteristics of nurses.

Abstrak:
Pekerjaan (manual handling) yang dilakukan oleh pekerja unit mixing dan supplyingdi line PPIC PT X berisiko untuk menimbulkan keluhan subjektif nyeri punggungbawah. Faktor risiko yang menjadi fokus penelitian adalah faktor risiko manualhandling dengan menambahkan faktor risiko individu sebagai faktor pendukung.Pengukuran faktor risiko manual handling dilakukan menggunakan metode REBAsedangkan faktor risiko individu diperoleh dengan wawancara. Hasil penelitianmenjelaskan terdapat dua aktivitas yang memiliki risiko ergonomi tinggi antara lainadalah aktivitas mengambil kantung sak dari palet dan mengangkat kantung sak kedalam mesin mixing. Hasil penelitian juga mendapatkan faktor kebiasaan merokokdan kebiasaan olahraga menjadi faktor risiko individu yang dapat memperberattingkat keluhan nyeri punggung bawah. Pengadaan alat vacuum dan pneumaticscissor lift akan mengurangi faktor risiko manual handling sehingga tingkat risikodan keluhan low back pain akan menurun.
