Abstrak:
Penelitian ini membahas tentang penilaiain risiko keselamatan dan kesehatankerja pada proses kerja casting di PT. X, Cikupa Tangerang tahun 2016.Penilaian risiko dilakukan untuk mendapatkan nilai risiko yang terdapat padaproses kerja casting di PT.X, Cikupa Tangerang. Identifikasi bahaya danrisiko dilakukan menggunakan Job Safety Analysis (JSA). Analisis tingkatrisiko menggunakan ukuran standard kualitatif yang dimodifikasi dariAS/NZS 4360:2004. Penelitian ini menggunakan desain studi cross sectionaldengan pendekatan observasi dan wawancara. Nilai risiko dihasilkan dariperkalian antara probabilitas dan konsekuensi yang mungkin ditimbulkan.Hasil penelitian menyatakan terdapat 94 risiko pada proses casting di PT.X,Cikupa Tangerang.

Abstrak:
Skripsi ini membahas tentang analisis risiko pada sepuluh tahapan proses yang terdapat pada bagian Low Pressure Die-Casting, PT X plant Onepack Cikarang. Penelitian ini menggunakan desain penelitian deskriptif yang mengacu pada Australian Standar/New Zealand Standard (AS/NZS) 4360 : 2004. Metode yang digunakan dalam analisis risiko adalah metode semi-kuantitatif formula matematika W.T.Fine. Tujuan penelitian adadalah mendapatkan tingkat risiko K3 pada proses kerja LPDC PT X Plant Onepack Cikarang. Hasil penelitian adalah jenis-jenis risiko dan tingkat risiko yang ditentukan dari nilai risiko yang dimiliki pada masing-masing dari sepuluh tahapan proses dibagian LPDC tersebut.

Abstrak:
Kompleksitas industri manufaktur mesin dan perlengkapan melibatkan beragam jenis proses produksi, salah satunya adalah pekerjaan pengelasan. Proses pekerjaan pengelasan merupakan salah satu proses industri terpenting dalam menggabungkan komponen logam atau baja. Beragam bahaya ditemukan pada proses pengelasan seperti percikan logam las, percikan api las, emisi asap las, dan manual handling selama pekerjaan berlangsung menimbulkan risiko keselamatan dan kesehatan kerja bagi pekerja. Penelitian ini bertujuan untuk mengidentifikasi dan menilai risiko keselamatan dan kesehatan kerja pada proses pekerjaan pengelasan di area fabrikasi PT XYZ. Tahap identifikasi bahaya dan risiko dilakukan secara kualitatif melalui observasi, kegiatan brainstorming, wawancara, dan telaah dokumen perusahaan menggunakan metode Hazard and Operability Study (Studi HAZOP). Setelah itu, dilakukan analisis penilaian risiko semi kuantitatif dengan menilai perkalian dari kriteria probability dan severity berdasarkan matriks penilaian risiko PT XYZ. Hasil penelitian menunjukkan bahwa terdapat 12 tahapan proses pekerjaan pengelasan. Teridentifikasi 52 bahaya dengan dengan 49 risiko dari proses pekerjaan pengelasan. Tingkat risiko yang didapatkan meliputi 23 risiko dengan kategori sangat tinggi, 9 risiko dengan kategori tinggi, dan 17 risiko dengan kategori sedang. Penilaian risiko melibatkan pengendalian risiko sehingga nilai tingkat risiko dapat diturunkan melalui pengendalian yang telah ada dan rekomendasi pengendalian yang diberikan peneliti.
The complexity of the machinery and equipment manufacturing industry involves various types of production processes, one of which is welding work. Welding is one of the most important industrial processes in joining metal or steel components. Various hazards found in the welding process such as welding metal sparks, welding sparks, welding fumes emissions, and manual handling during work pose risks to occupational safety and health for workers. This study aims to identify and assess occupational safety and health risks in the welding work process in the PT XYZ fabrication area. The hazard and risk identification stage is carried out qualitatively through observation, brainstorming activities, interviews, and reviewing company documents using the Hazard and Operability Study (HAZOP Study). After that, a semi-quantitative risk assessment analysis was carried out by assessing the multiplication of probability and severity criteria based on PT XYZ's risk assessment matrix. The results showed that there were 12 stages of the welding work process. Identified 52 hazards with 49 risks from the welding work process. The level of risk obtained includes 23 risks with very high categories, 9 risks with high categories, and 17 risks with moderate categories. Risk assessment involves risk control so that the value of the risk level can be reduced through existing controls and control recommendations provided by researchers.

Abstrak:
Penelitian ini membahas tentang analisis risiko K3 di proses kerja Die Casting Plan 3 PT. X pada bulan Mei-Juni 2016. Penelitian ini menggunakan desain penelitian deskriptif dengan AS/NZS ISO 31000:2009 sebagai standar untuk proses penilaian risiko K3. Metode yang digunakan dalam penilaian risiko adalah semi-kuantitatif formula matematika W. T Fine. Identifikasi risiko menggunakan metode Job Hazard Analysis (JHA). Tujuan penelitian ini untuk mengetahui tingkat risiko K3 pada proses kerja Die Casting. Hasil penelitian menunjukkan bahwa ditemukan sebanyak 58 risiko pada proses kerja Die Casting dimana pada penilaian risiko awal (basic risk) sebanyak 48,27% risiko dengan level very high, 24,14% risiko dengan level risiko substantial, 18,97% risiko dengan level risiko priority 3, dan 8,62% risiko dengan level risiko priority 1. Kemudian risiko dinilai kembali dengan mempertimbangkan pengendalian yang sudah ada (existing control) menjadi 46,55% risiko dengan level risiko priority 3, 24,13% risiko dengan level risiko substantial, 13,80% risiko dengan level risiko priority 1, 8,62% risiko dengan level risiko very high, dan 6,90% risiko dengan level risiko acceptable. Sedangkan dari keempat proses kerja Die Casting terdapat 5 risiko terbesar dengan level risiko yang belum acceptable, yaitu very high, priority 1 dan substantial. Oleh karena itu, diberikan rekomendasi untuk pengendalian dari 5 risiko terbesar tersebut dari masing-masing proses yang bersifar engineering control, administrative control atau personal protective control.
