Penelitianinidilakukandengantujuanmengembangkan model budaya keselamatan pasien yang sesuai di RSIA Tumbuh Kembang Cimanggis.Penelitianiniadalahgabunganantarastudi kualitatifdankuantitatif.Penelitiankualitatifdilakukandenganwawancara mendalam untuk mengetahui asumsi, nilai, dan keyakinan pegawai terhadap keselamatan pasien sebagai dasar pemetaan budaya keselamatan pasien pegawai,sementarakuantitatifdilakukandengankuesioner untuk mengetahui gambaran faktor individu dan faktor organisasi pegawai.Dari 118 responden yang diteliti didapatkan 55,9% responden memiliki pengetahuan dan sikap yang baik, 52,5% responden memiliki motivasi baik, 57,6% responden memiliki tingkat kompetensi yang baik, 61% responden memiliki kewaspadaan situasi yang baik, 73,7% responden mengalami tingkat stress kerja yang rendah, 50,5% repondenmenyatakantingkatkelelahanyang dialami juga cukup baik. Untuk faktor organisasi diperoleh informasi 53,4% responden menyatakan kepemimpinan baik, 51,7%. Responden memandang kerja tim baik, 53,4% responden menyatakan kepemimpinan tim baik, dan 55,1% responden menyatakan pengambilan keputusuan sudah dilakukan dengan baik. Gambaran faktor lingkungan diperoleh melalui observasi dengan checklist.Semua informasi yang diperoleh akan dipergunakan sebagai bahan pertimbangan pengembangan model budaya keselamatan pasien yanng baru. Hasil temuan faktor individu, faktor organisasi, dan faktor lingkungan cukup mendukung peneliti untuk mengembangkan budaya keselamatan pasien yang dapat menunjang terciptanya standar keselamatan pasien yang optimal. Usulan pengembangan budaya tersebut kemudian dipresentasikandidalam diskusi kelompok terarah untuk mengetahui respon pegawai dan manejemen serta sasaran yang hendak ditekankan melalui budaya yang baru. Disepakati bahwa Safety, Good Communication, Team Work, Home Sweet Hospital, dan Better Everyday menjadi elemen kunci budaya keselamatan pasien yang baru yang sesuai di RSIA Tumbuh Kembang. The aim of this research is to develop a model of patient safety culture that fits RSIA Tumbuh Kembang Cimanggis. The study was a combination of qualitative and quantitative study. Qualitative research conducted with in-depth interviews to find out what assumptions, values, beliefs of patient safety that the employee have as a basis for mapping the current patient safety culture, while quantitative conducted with a questionnaire to know the description of individual factors and organizational factors. From 118 employee surveyed earned 55.9% of respondents have knowledge and good attitude, 52.5% of respondents have a good motivation, 57.6% of respondents have a good level of competence, 61% of respondents have a good awareness of the situation, 73.7% of respondents had low levels of job stress, 50.5 % respondents stating the level of fatigue is also quite good. Organizational factors obtained for 53.4% of respondents said the information good leadership, 51.7%. Respondents saw good teamwork, 53.4% of respondents said good team leadership, and 55.1% of respondents said taking decision have done well. Overview of environmental factors is obtained through the observation checklist. All information obtained will be used as a reference model of the development of new patient safety culture. The findings of the individual factors, organizational factors, and environmental factors sufficient to support researchers to develop a culture of patient safety that can support the creation of optimal patient safety standards. Proposed development of a culture is then presentate in focus groups to evaluate the employee and managment response and what kind of target are going to emphasized by the new culture. It was agreed that the Safety, Good Communication, Team Work, Home Sweet Hospital, and Better Everyday became a key element of the new patient safety culture that fits RSIA Tumbuh Kembang Cimanggis Keyword : culture, patient safety, organization.
Keselamatan pasien merupakan salah satu faktor penting di dalam pelaksanaan rumah sakit. Tujuan penelitian ini untuk melihat kesiapan penerapan keselamatan pasien di RSIA Assalam. Penelitian dilakukan dengan metode penelitian kuantitatif dan penelitian kualitatif. Metode survey dilakukan secara total sampling terhadap 59 pegawai, dan wawancara mendalam terhadap 5 informan. Hasil survey menunjukkan RSIA Assalam membudaya sedang dalam keselamatan pasien.
Dari hasil analisis menunjukkan belum siapnya RSIA Assalam dalam menerapkan budaya keselamatan pasien. Penelitian merekomendasikan penyusunan standar prosedur operasional tentang keselamatan pasien, meningkatkan jumlah pelaporan kejadian dan memberikan pendidikan dan pelatihan kepada seluruh pegawai tentang keselamatan pasien.
Patient safety is one of the important factors in the implementation of the hospital. The purpose of this study to look at the implementation of patient safety preparedness in Assalam RSIA. The research was conducted using quantitative research and qualitative research. Methods of sampling survey conducted to 59 employees total, and depth interviews with 5 informants. The survey shows RSIA Assalam being entrenched in patient safety.
From the analysis of the readiness of the application of patient safety culture in hospitals shows RSIA Assalam unprepared to implement patient safety culture. Study recommends the creation of standard operating procedures on patient safety, increase the number of reporting events and providing education and training to all employees about patient safety.
Tesis ini membahas perlunya RS Tugu Ibu Depok memiliki remunerasi yang baik dan dapat menjadi salah satu opsi menyelesaikan masalah kesejahteraan finansial pegawai yang kurang memadai sehingga mampu meningkatkan kinerja pegawai pada khususnya dan mutu pelayanan pada umumnya. Penelitian ini adalah penelitian kualitatif dengan desain deskriptif analitik. Pengumpulan data yang dilakukan berdasarkan hasil wawancara mendalam. Hasil yang didapat menggambarkan bahwa saat ini indikator remunerasi yang berlaku kecuali dokter adalah pendidikan dan masa kerja, jabatan yang disandang, dan risiko kerja yang berlaku untuk unit radiologi saja. Sedangkan pengembangan model remunerasi yang dapat dilakukan RS Tugu Ibu Depok 2 yaitu : tingkat kegawatdaruratan dan risiko kerja yang mencakup seluruh pegawai. Untuk dasar penetapan remunerasi diharapkan sesuai dengan ketentuan pemerintah, kemampuan finansial internal rumah sakit, dan pesaing (rumah sakit lain). Diharapkan ada penelitian selanjutnya terhadap indikator lain seperti kehadiran atau absensi.
The focus of this study is discussed the necessity for Tugu Ibu Hospital to have a good remuneration also become one of the option to solved the lack of employees financial welfare, so it could increase employees performance particulary and the standard of service generally. This research is qualitative analytical descriptive. The data were collected by in depth interview. Theresults describe that the current remuneration indicators which applicable, except the physician, are education and years of service, position, and the risk of work applicable only for radiological unit.There were 2 indicators of remuneration could be applicable on Tugu Ibu Hospital, there were : emergency level and the risk of work that cover all employees. For basic determination of remuneration is expected accordance with government regulations, hospital financial capability, and competitors (other hospitals). The researcher expect further studies on other indicators such as the presence or absence.

This study discusses the perceptions of nursing staff on the patient safety culture at RSIA SamMarie Basra in 2017. The purpose of this study is to get a description of the perceptions of nursing staff on patient safety culture that can reflect the quality level of patient safety implementation at RSIA SamMarie Basra. This research is a quantitative research with survey method and cross sectional approach. The results showed that 53,1% of nursing staff at RSIA SamMarie Basra had a good perception of the patient safety culture. There is no significant relationship between individual characteristics on the nursing staff perceptions on patient safety culture. The researcher recommends that all workers in the hospital commit to the implementation of non-punitive response so that the patient safety culture in hospitals can be improved.

Analisis budaya keselamatan pasien menghasilkan 4 dimensi kategori budaya sedang yang memerlukan perbaikan keselamatan pasien serta 7 dimensi kategori budaya baik yang menjadi kekuatan dalam keselamatan pasien. Pengorganisasian ketenagaan, beban kerja dan pola kerja; konseling pasien; keterbukaan komunikasi; dan respons terhadap kesalahan menjadi kelemahan budaya keselamatan pasien yang menjadi prioritas perbaikan. Tingkat pelaporan kejadian masih rendah dan harus mendapat perbaikan.

This research uses sequential explanatory design started from quantitative analysis using questionnaire The Pharmacy Survey on Patient Safety Culture (PSOPSC) from AHRQ followed by focus group discussion to formulate strategy to build patient safety culture.
Analysis of patient safety culture resulted in 4 dimensions of moderate cultural categories that require improvement and 7 dimensions of good cultural categories that be strength of the patient safety culture. Staffing, Work Pressure and Pace; Patient counseling; Communication openness; and Response to Mistakes is weakness of the patient safety culture that become priority improvement. Level of incident reporting is still low and need improvement.

it is necessary to measure patient safety culture especially among nurses.This study aims to describe the nurses perception about patient safety culture andhow the implementation of patient safety goals at Setia Mitra Hospital. Thisdescriptive research conducted with quantitative and qualitative approaches.Population in this study is all nurses in Setia Mitra hospital that consist of 77 nurseswith sample 68 nurses.The result of study shows the nurse's perception about patient safety in SetiaMitra hospitals is 72.1% good. Dimension with the most good nurse perception ishospital handoffs and transitions dimension by 75% and at least perceived good ismanagement support dimension of 1.5%. Statistically, age (p value = 0.048), workperiod (p value = 0.016) and the level of nurses (p value = 0.049) have arelationship with the nurses perception on patient safety culture (p <0.05). In theimplementation of patient safety goals in Setia Mitra Hospital, namely 55.15% with agood understanding. It is recommended that patient safety team in the Setia MitraHospital further optimized its duties and functions so that the patient safety culture inhospitals can be improved.Keywords : Patient Safety Culture, Nurse Perception, Patient Safety Goal.
Penelitian ini adalah penelitian operasional riset dengan pendekatan masalah melalui analisis deskriftif mengenai Renizana Pengembangan Unit Tumbuh Kembang Anak di Rumah Sakit Jiwa dan Ketergantungan Obat Sembada Medan 2010. Hasil penelitian menunjukkan posisi rencana ini adalah grow and build. Akan tetapi faktor internal rumah sakit masih dalam posisi yang lemah. Strategi terpilih yang dihasilkan dari Consensus Decision Making Group (CDMG) berdasarkan analisis IE Matriks dan SWOT Matriks rencana pengembangan ini menunjukkan langkah~langkah penguatan terhadap faktor internal dan integrasi terhadap faktor eksternal. Pentingnya komitmen yang kuat dari seluruh "top management" Rumah Sakit Jiwa dan Ketergantungan Obat Sembada untuk mengimplementasikan rencana pengembangan ini. Penetapan terapi dan tarif layanan merupakan dasar dari rencana pengembangan ini. Perlu dilakukan langkah-langkah dalam menghadapi citra rumah sakit yang telah terbentuk di masyarakat.
This research is an operational research with descriptive analysis approach, it's about Development Planning Chi1d?s Growth and Development Unit at Sembada Mental and Drugs Abuse Hospital, Medan in 2010. The result of this research showed the position of this plan is in grow and build. However hospital internal factor is in weak position. The chosen strategies through Consensus Decision Making Group (CDMG) based on IE matrix and SWOT matrix analysis of this development planning, shows steps for strengthen intemal and integration for external that are market development and product development. To implement this development planning, strong commitment is essential fiom all top management of Sembada Mental and Drugs Abuse Hospital. Arrangement for therapy and tariffs service are fundamental of this development planning. In order to anticipate the hospital image that already grown in the society few steps have to be conduct.
ABSTRAK Budaya keselamatan pasien baru mulai tumbuh di RSU Manuaba setelah dicanangkan dan dibentuknya Tim Keselamatan Pasien Rumah Sakit RSU Manuaba tahun 2009. Belum berjalan dengan baiknya sistem pelatihan yang ada terutama dalam konsep keselamatan pasien menggambarkan belum adanya upaya rumah sakit dalam meningkatkan mutu rumah sakit terutama mutu SDMnya. Oleh karena itu peneliti ingin mengetahui kesiapan perawat dalam menerapkan konsep keselamatan pasien di Rumah Sakit. Penelitian ini menggunakan menggunakan desain operational research dengan pendekatan kualitatif jumlah sampel sebanyak 51 orang, yang merupakan jumlah total perawat RSU Manuaba. Hasil penelitian ini menunjukkan bahwa bahwa untuk saat ini perawat belum siap untuk menerapkan konsep keselamatan pasien di rumah sakit, kebijakan keselamatan pasien dan SOP sudah ada tapi belum disosialisasikan dengan baik dan berkelanjutan. Ada perubahan nilai pengetahuan dan sikap perawat ketika diukur sebelum dan sesudah diberikan pelatihan. Anggaran mengenai pendidikan dan pelatihan keselamatan pasien belum ada. Pihak rumah perlu meningkatkan pendidikan dan pelatihan mengenai konsep keselamatan pasien agar terwujud budaya keselamatan pasien di lingkungan perawat RSU Manuaba. Patient safety culture has just begun at Manuaba General Hospital after declerated and performed patient safety team at 2009. The Patient Safety concept had not worked out properly,the hospital had not strongly forced to improve quality of human resources. The study investigated the preparation of nurses in managing patient safety concept at Manuaba General Hospital. The study used research operational design with qualitative method, total sample were 51 nurses who worked at Manuaba General Hospital. The result showed nurses had not ready to implemantation patient safety concept, patient safety policy and standart operational procedure had established but not been good and continously sosialized. There was proggression of knowledge and attitude of nurses pre and post training.There was no budget for patient safety training. The manangement of Hospital need to increase education anda training of patient safety to create it concept as a culture in Manuaba General Hospital. Patient safety is very basic in the service to patients in the hospital. As a firststep in improving patient safety in hospitals is by measuring the patient's safety culture.The purpose of this research is to know the relationship of transformational leadership,teamwork, and individual awareness to patient safety culture at Prima Medika Hospital(RSPM) Denpasar. The method used is mix method. The quantitative approach usescross-sectional research. Questionnaires were distributed to a sample of 218respondents. In a qualitative approach, further searches on things that remainunanswered, to complement the explanation of the results of quantitative research.Statistical analysis using Structural Equation Modeling (SEM), with STATA-SE 12.1program. In-depth interviews were conducted with the RSPM Board of Directors andrelevant employees, to confirm the results of the study. The results of the analysis showthat the independent variables are Transformational Leadership, Individual Awareness,and Team Cooperation, and all three are related significantly to the dependent variableof Patient Safety Culture (p <0.001). The individual consciousness variable had thegreatest relation (77,8%) with patient safety culture compared to other variable. It isrecommended to improve patient safety culture in RSPM, it is necessary to conductroutine patient safety training, improve effective communication, risk competence andawareness, learn from mistakes, and evaluate workload, in order to create desiredpatient safety culture.Keywords: patient safety culture, structural equation modeling, transformationalleadership, teamwork, individual awareness.
