Abstrak: Objectives: We evaluated the cost-effectiveness of fee-based and free testing strategies at an HIV voluntary counseling and testing (VCT) program integrated into a community-based AIDS service organization in Moshi, Tanzania.

Methods: We waived the usual fee schedule during a 2-week free, advertised VCT campaign; analyzed the number of clients testing per day during prefree, free, and postfree testing periods; and estimated the cost-effectiveness of limited and sustained free testing strategies.

Results: The number of clients testing per day increased from 4.1 during the prefree testing interval to 15.0 during the free testing campaign (P<.0001) and remained significantly increased at 7.1 (P < .0001) after resumption of the standard fees. HIV seroprevalence (16.7%) and risk behaviors were unchanged over these intervals. Modeled over 1 year, the costs per infection averted with the standard fee schedule, with a 2-week free VCT campaign, and with sustained free VCT year-round were $170, $105, and $92, respectively, and the costs per disability-adjusted life year gained were $8.72, $5.40, and $4.72, respectively.

Conclusions: The provision of free VCT enhances both the number of clients testing per day and its cost-effectiveness in resource-limited settings.

Abstrak:
Salah satu strategi yang dilakukan pemerintah Indonesia untuk mencapai target Ending AIDS 2030 (95-95-95) adalah melakukan skrining HIV berbasis komunitas (Community-based Screening/CBS) dengan menggunakan alat Oral-Fluid Test (OFT). Sebagai salah satu Principal Recipient (PR) hibah The Global Fund, Indonesia AIDS Coalition (IAC) berperan sebagai mitra pelaksana Kemenkes RI dalam melaksanakan program-program penanggulangan HIV/AIDS termasuk program CBS HIV untuk populasi Pekerja Seks Perempuan (PSP). Penelitian ini bertujuan untuk mengetahui gambaran implementasi program serta tantangan dan hambatan dalam pelaksanaan CBS HIV pada PSP di IAC. Desain penelitian yang digunakan adalah deskriptif dengan pendekatan kualitatif dan kuantitatif. Pengumpulan data dilakukan melalui Focus Group Disccusion (FGD) dengan 7 orang penerima program dan wawancara mendalam dengan 7 orang pelaksana program. Penelitian ini menemukan secara umum, capaian CBS HIV pada PSP di IAC tahun 2022 masih rendah. Tidak adanya standar kompetensi dan belum meratanya pelatihan bagi petugas lapangan menjadi hambatan utama dalam aspek komunikasi. Tidak adanya insentif bagi petugas menurunkan semangat petugas khususnya relawan. Hambatan lainnya adalah kurangnya perencanaan dana distribusi di awal program yang menyebabkan terlambatnya pelaksanaan program. Tidak adanya SOP atau buku pedoman juga menghambat petugas dalam pencarian informasi. Kemenkes RI dan IAC serta mitra pelaksana lainnya diharapkan dapat pengembangan keterampilan petugas lapangan, membuat SOP dan buku pedoman yang dapat diakses dan dipahami oleh seluruh petugas, mempertimbangkan insentif finansial maupun non-finansial bagi petugas, serta meningkatkan koordinasi terkait pencatatan dan pelaporan program untuk meningkatkan keberhasilan program CBS HIV di Indonesia.

One of the strategies undertaken by the Indonesian government to achieve the target of Ending AIDS 2030 (95-95-95) is to carry out community-based screening for HIV (CBS) using the Oral-Fluid Test (OFT) kit. As one of the Principal Recipients (PR) grants from The Global Fund, the Indonesia AIDS Coalition (IAC) acts as an implementing partner for the Ministry of Health of Indonesia in implementing HIV/AIDS prevention programs including the CBS HIV program for the female sex worker (FSW) population. This study aims to describe the implementation program as well as challenges and obstacles in the implementation of CBS HIV on FSW in IAC. The research design used is descriptive with a qualitative and quantitative approach. Data collection was carried out through Focus Group Discussion (FGD) with 7 program recipients and in-depth interviews with 7 program implementers. This study found that in general, the achievement of CBS HIV in FSW at IAC in 2022 is still low. The absence of competency standards and uneven staff training are the main obstacles in the communication aspect. The absence of incentives for officers reduces the enthusiasm of officers, especially volunteers. Another obstacle was the lack of planning of distribution funds at the beginning of the program which caused delays in program implementation. The absence of standardized procedure or guidebooks also hindered officers from seeking information. The Ministry of Health of Indonesia and IAC and other implementing partners are recommended to be able to develop the skills of field staff, provide standardized procedures and guidebook that are accessible and understandable to all officers, consider financial and non-financial incentives for officers, and improve coordination regarding program recording and reporting to improve the implementation of CBS HIV program in Indonesia.
