Abstrak:

Prevalensi diabetes melitus mengalami tren meningkat, baik di tingkat global maupun di
Indonesia. Prevalensi DM berdasarkan pemeriksaan kadar gula darah di Indonesia meningkat dari tahun 2013 (6,9%), 2018 (10,9%), hingga 2023 (11,7%). DM adalah salah satu penyakit tidak menular (PTM) yang bisa dicegah melalui perubahan gaya hidup, salah satunya adalah aktivitas fisik, yang menjadi kunci dalam pencegahan dan pengurangan beban PTM. Penelitian ini bertujuan untuk mengetahui hubungan antara aktivitas fisik dengan kejadian DM pada penduduk usia 15 tahun ke atas di Indonesia pada tahun 2023, dengan distratifikasi oleh variabel perancu usia, jenis kelamin, status obesitas, tempat tinggal, status pekerjaan, dan status ekonomi. Penelitian ini menggunakan data Survei Kesehatan Indonesia (SKI) 2023 dengan desain studi potong lintang. Analisis yang dilakukan adalah analisis univariat, bivariat, dan stratifikasi. Berdasarkan analisis bivariat, aktivitas fisik berhubungan signifikan dengan DM, dengan aktivitas fisik kurang lebih berpeluang (OR=1,36; 95% CI= 1,23 – 1,51) untuk memiliki DM. Variabel perancu usia, jenis kelamin, status obesitas, dan status pekerjaan berhubungan signifikan dengan DM dan digunakan untuk stratifikasi. Setelah distratifikasi, hubungan aktivitas fisik dengan DM tetap signifikan, tetapi nilai OR berbeda berdasarkan modifikasi efek dari variabel usia dan jenis kelamin. Penduduk usia 40 tahun ke atas (OR=1,52; 95% CI= 1,35 – 1,71) lebih berpeluang untuk memiliki DM jika kurang melakukan aktivitas fisik. Upaya intervensi pengelola program kesehatan perlu difokuskan untuk merancang dan mengimplementasikan kebijakan aktivitas fisik dan skrining DM yang komprehensif dengan bekerja sama dengan sektor luar kesehatan dan fokus untuk meningkatkan partisipasi aktivitas fisik pada penduduk berusia 40 tahun ke atas.


There is an upward trend of diabetes mellitus prevalency, both globally and in Indonesia.  DM prevalency based on blood glucose examination in Indonesia shows an upward trend,  from 2013 (6,9%), 2018 (10,9%), up to 2023 (11,7%). DM is a noncommunicable disease  (NCD) that could be prevented by lifestyle change, which one of them is physical activity,  a key in NCD prevention and burden reduction.  This study aims to identify the  association between physical activity and DM in Indonesia’s population aged 15 years  and over in 2023, with stratification based on confounding variable age, sex, obesity  status, residential area, employment status, and economic status. This study utilizes  Survei Kesehatan Indonesia (SKI) 2023 data with cross-sectional study design.  Univariate, bivariate, and stratification analysis were conducted. Based on bivariate  analysis, physical activity is significantly associated with DM, with the less active group  having higher odds (OR=1,36; 95% CI= 1,23 – 1,51) for developing DM. Confounding  variables age, sex, obesity status, and employment status significantly associated with  DM and will be used in stratification. After stratification, the association between  physical activity and DM hold its significance, but the OR differs based on effect  modification by age and sex variables. Population aged 40 years and over (OR=1,52; 95%  CI= 1,35 – 1,71) higher odds to develop DM if they’re physically inactive. Interventions  effort made by health program organizer needs to be focused on designing and  implementing comprehensive physical activity and DM screening policy with partners  outside the health sectors and focusing on increasing participation in physical activity  among population aged 40 years and over.

Abstrak:
Hipotensi ortostatik adalah turunnya tekanan darah sistolik (TDS) 20 mmHg atau turunnya tekanan darah diastolik (TDS) 10 mmHg pada saat perubahan posisi, dari posisi tidur ke posisi tegak. Berbagai faktor yang berhubungan dengan hipotensi ortostatik, seperti usia, obat anti hipertensi, hipertensi, strok dan diabetes melitus masih diperdebatkan. Sampai saat ini belum ada data mengenai prevalensi hipotensi ortostatik di Indonesia. Belum diketahui pula faktor prediktor hipotensi ortostatik. Sebagian besar penelitian hipotensi ortostatik yang ada di luar negeri dilakukan pada subjek berusia lanjut, Tujuan penelitian ini adalah untuk mendapatkan prevalensi hipotensi ortostatik di Indonesia dan faktor prediktor terjadinya hipotensi ortostatik pada orang berusia 40 tahun ke atas di Indonesia. Penelitian ini merupakan bagian penelitian survei epidemiology hipertensi di Indonesia. Empat ribu empat ratus tiga puluh enam subjek berusia 40-94 tahun didapatkan secara random dari berbagai praktek dokter di berbagai kabupaten di Indonesia. Data dikumpulkan dengan melakukan serangkaian anamnesis (riwayat penyakit diabetes melitus, hipertensi, dan strok serta penggunaan obat anti hipertensi) dan pemeriksaan tekanan darah pada posisi tidur dan duduk setelah 1-3 menit. Regresi logistik ganda dilakukan untuk mendapatkan faktor prediktor hipotensi ortostatik yang paling bermakna. Subjek yang mengalami hipotensi ortostatik sebesar 561 subjek (12,65%). Analisis bivariat menunjukkan bahwa riwayat diabetes melitus, riwayat strok, tekanan darah sistolik tinggi dan tekanan darah diastolik tinggi mempengaruhi terjadinya hipotensi ortostatik. Usia dan penggunaan obat anti hipertensi tidak mempengaruhi terjadinya hipotensi ortostatik. Obat agonis alfa-2 sentral dan kerja sentral lain merupakan satu-satunya obat anti hipertensi yang mempengaruhi terjadinya hipotensi ortostatik. Hasil analisis multivariat mendapatkan tekanan darah sistolik tinggi dan tekanan darah diastolik tinggi sebagai prediktor hipotensi ortostatik. Sedangkan riwayat diabetes melitus dan riwayat strok tidak mempengaruhi terjadinya hipotensi ortostatik. Penggunaan obat anti hipertensi merupakan faktor protektif terjadinya hipotensi ortostatik. Penelitian ini memastikan bahwa usia bukan merupakan prediktor terjadinya hipotensi ortostatik. Adanya komorbiditas seperti hipertensi (tekanan darah sistolik atau diastolik tinggi) merupakan prediktor terjadinya hipotensi ortostatik. Sedangkan obat anti hipertensi merupakan faktor protektif terjadinya hipotensi ortostatik. Penatalaksanaan hipertensi yang baik dan pemilihan obat anti hipertensi yang tepat amat diperlukan untuk mencegah terjadinya hipotensi ortostatik. Daftar bacaan : 32 (1978 - 2001)

The Prevalence And Predictor Of Orthostatic Hypotension Among 40 Years And Above Adult Population In Indonesia.Various factors associated with orthostatic hypotension such as age, drug induced hypotension, hypertension and diabetes mellitus have still been questioned and debatable with one another. No big scale population study done in this matter. Furthermore, most of previous studies were conducted in subjects 65 years or older, only a few were done in subjects from younger to older adults. The purpose of this study is to find the prevalence and predictor of orthostatic hypotension among 40 years and above adult population in Indonesia. This study is a part of Indonesian hypertension epidemiologic survey. A random sample of 4436 subjects aged 40-94 years was obtain from various municipilities in every big island in Indonesia. Orthostatic testing, as well as assesment of history of medical conditions (diabetes mellitus, stroke, and hypertension), blood pressure measurement and use of anti-hypertensive medications were performed. Blood pressure measurements were obtained by trained doctors with the subjects in supine position and after they had been seated for 1-3 minute. A stepwise logistic regression was used to determine significant predictor of orthostatic hypotension. A total of 561 persons (12.6%) experienced orthostatic hypotension. Bivariate analysis showed that orthostatic hypotension was influenced by history of diabetes mellitus, history of stroke, high systolic and diastolic blood pressure. Orthostatic hypotension was not influenced by age and the use of anti hypertension medicine. Central ccz-agonist and other centrally acting drug is the only anti hypertension medicine which influences orthostatic hypotension. Multivariate analysis showed that high systolic and diastolic blood pressure were predictor factors of ortostatic hypotension, while history of diabetes mellitus and stroke were not. The use of anti-hypertensive medicine was protective factor for orthostatic hypotension . This study confirms the conclusion that age by itself is not a predictor for orthostatic hypotension. In fact, the existence of comorbidities in the subjects such as hypertension (high systolic and diastolic blood pressure) is a predictor factor, while the use of anti-hypertensive medication is a protective factor. Proper management of hypertension and preference of anti hypertension medicine is a must to prevent orthostatic hypotension. References : 32 (1978 - 2001)
