Abstrak: Purpose: Research examining the relationship between human immunodeficiency virus (HIV)-related risk behavior and negative affective states has netted mixed findings. The present study examined the potential moderating role of gender and sexual orientation on negative affect and HIV-related risk behavior.

Design: Measures of depression, anxiety, and HIV-related risk behavior were collected from participants at the time of pretest counseling. Analysis of variance was used to examine HIV-related risk behavior as a function of negative affect levels and gender and behavioral sexual orientation.

Setting: The study was conducted through a community-based, anonymous HIV counseling and testing site.

Subjects: Participants included 185 consecutively recruited men and women presenting for HIV testing.

Measures: Participants completed a structured interview regarding HIV-related risk behavior, the Center for Epidemiological Studies Depressed Mood Scale, and the Beck Anxiety Inventory.

Results: Greater HlV-related risk behavior was associated with high levels of anxiety and moderate or high levels of depression for bisexual women. Greater HIV-related risk behavior was also associated with low levels of anxiety and mild to moderate levels of depression for bisexual men.

Conclusions: The relationship between negative affective states and HIV-related risk behavior may vary as a function of gender and behavioral sexual orientation. Better understanding of the relationship between negative affect and risk behavior may be achieved through consideration of these sources of variation.

Abstrak:
Latar Belakang:. Rekam medis elektronik (RME) merupakan teknologi informasi yang bertujuan untuk mengumpulkan, menyimpan, memproses, dan mengakses data dalam sistem informasi manajemen RS. Grup RS Hermina mulai menerapkan RME paling pertama pada November 2022. Untuk menjaga keberlangsungan serta meningkatkan pengembangan RME, grup RS Hermina perlu melakukan analisis implementasi RME. Tujuan: Studi ini bertujuan untuk menganalisis implementasi penggunaan RME di grup RS Hermina. Metode: Penelitian ini menggunakan metode potong lintang di grup RS Hermina selama bulan April hingga Mei 2024 dengan melibatkan 49 RS di Indonesia. Terdapat 12.429 karyawan tenaga medis yang terdiri dari bidan/ perawat, dokter umum, dokter gigi, dan dokter spesialis, 149 karyawan direksi/ manajemen RS, dan 277 karyawan rekam medis. Penelitian dilakukan melalui penyebaran kuesioner Human Organization Technology net benefit (HOT-fit) digital sesuai kriteria inklusi dan eksklusi. Responden pengguna diambil dengan proportional sampling. Sampel responden pengguna terdiri dari bidan/ perawat, dokter umum, dokter gigi, dan dokter spesialis sebanyak 200 orang. Sampel responden manajemen diambil dengan total sampling yaitu sebesar 149 orang. Sementara itu, seluruh perwakilan responden rekam medis dari masing-masing RS Hermina diambil untuk mengetahui widget mana yang masih dikerjakan secara manual. Data yang diperoleh dilakukan analisis univariat, bivariat, hingga multivariat dengan kemaknaan p<0,05. Penelitian ini telah lolos dari komite etik riset FKM UI. Hasil: Kuesioner HOT-fit modifikasi dilakukan uji validitas dengan hasil seluruh pernyataan valid dan reliabel. Setelah dilakukan kriteria inklusi dan eksklusi didapatkan responden yang berpartisipasi dalam penelitian ialah 392 responden. Dari sisi pengguna, sebagian besar memberikan respon positif (>50% setuju atau sangat setuju) pada pernyataan yang ada di faktor manusia, organisasi, teknologi, dan manfaat dari implementasi RME. Hasil analisis multivariat menunjukkan ketiga faktor memiliki hubungan bermakna dengan manfaat efektivitas RME. Pada manajemen, terdapat 34 responden yang melakukan pengisian kuesioner. Hasil jawaban manajemen menunjukkan respon positif (>50% setuju atau sangat setuju) pada pernyataan yang ada di faktor organisasi, teknologi, dan manfaat dari implementasi RME). Diskusi: Implementasi RME di grup RS Hermina dapat dikatakan efektif dilihat dari cara mendapatkan informasi lebih cepat dan efisien, mengurangi angka kesalahan penulisan diagnosis, kemudahan konsultasi, alat legal untuk klaim BPJS, serta meningkatkan keamanan data pasien. Dari hasil penelitian, hal yang dapat ditingkatkan meliputi RME dikurangi tingkat error melalui alokasi dana untuk peningkatan jaringan dan perlu standar prosedur operasional jika terjadi kendala RME. Selain itu, tampilan menarik, sederhana, dan mudah digunakan dapat diperbaiki melalui adanya focused group discussion antara pengguna dengan vendor penyedia RME. Kesimpulan: Implementasi RME di grup RS Hermina sudah efektif dan ketiga faktor baik faktor manusia, organisasi, maupun terknologi memiliki hubungan dengan efektivitas RME. Kuesioner HOT-fit modifikasi ini dapat digunakan untuk menilai analisis implementasi RME di masing-masing grup RS Hermina.

Background: Electronic medical record (EMR) is an information technology for collecting, saving, processing, and accessing data in hospital management information system. The Hermina Hospital Group began implementing EMR firstly in November 2022. To maintain sustainability and enhance the EMR development, the Hermina Hospital Group needs to conduct an analysis of EMR implementation. Objective: This study aims to analyze the implementation of EMR usage in the Hermina Hospital Group. Methods: This study used a a cross-sectional method in the Hermina Hospital Group from April to May 2024 involving 49 hospitals around Indonesia. There were approximately 12,429 medical staffs consisting of midwives/nurses, general practitioners, dentists, and specialists, 149 hospital management/ director staffs, and 277 medical record staffs.. The study was carried out through the distribution of the modified Human Organization Technology Net Benefit (HOT-fit) digital questionnaire according to inclusion and exclusion criteria. User respondent samples were taken using proportional sampling. User respondent samples consisted of 200 midwives/nurses, general practitioners, dentists, and specialists. Management respondent samples were taken with a total sampling of 149 people. Meanwhile, all representative respondents of medical records from each Hermina Hospital were taken to determine which widgets were still being manually processed. Data obtained were analyzed univariately, bivariately, and multivariately with significance p50% agree or strongly agree) to the statements in the human, organizational, technological, and benefits factors of EMR implementation. Multivariate analysis showed that these three factors had a significant relationship with the effectiveness benefits of EMR implementation. In management, there were 34 respondents who completed the questionnaire. The management response results showed positive responses (>50% agree or strongly agree) to the statements in the organizational, technological, and benefits factors of EMR implementation. Discussion: The implementation of EMR at Hermina Hospital Group can be considered effective based on quicker and more efficient information retrieval, reduced diagnostic error rates, ease of consultation, legal compliance for BPJS claims, and enhanced patient data security. Areas for improvement identified from the study include reducing EMR error rates through increased network funding and establishing standard operational procedures for EHR challenges. Additionally, improving user interface attractiveness, simplicity, and usability could be achieved through focused group discussions between users and EHR vendors. Conclusion: EMR implementation at Hermina Hospital Group has been effective with all three factors—human, organizational, and technological—demonstrating a relationship with EHR effectiveness. This modified HOT-fit questionnaire can be used to reproduce for analysis of EMR implementation in each Hermina Hospital group.
