Influenza is one of the ARI diseases that receives attention because it can cause outbreaks. In Indonesia, influenza positivity rate reached 40.3%, where the identified viruses are virus A (subtype H1N1Pdm09 and AH3) and virus B (subtype Victoria). The purpose of this study is to determine the factors that influence the incidence of Influenza A and B in DKI Jakarta in 2021-2022. Cross sectional study and cox regression analysis estimating the Prevalence Ratio (PR) value were conducted by utilizing secondary data of Influenza Like Illness surveillance in DKI Jakarta in 2021-2022. The results showed that the prevalence of positive Influenza was 27.8%. The results of multivariate analysis showed that the risk factors associated with influenza A and B were age>5-64 years had a PR of 0.51 times (p-value = 0.006; 95% CI = 0.31 - 0.82), contact with sick people had a PR of 2.27 times (p-value = <0.001; 95% CI = 1.45 - 3.56), and the rainy season had a PR of 3.26 times (p-value = <0.001; 95% CI = 1.68 - 6.33) of influenza A and B infection. The rainy season is the dominant factor influencing the incidence of influenza A and B in DKI Jakarta in 2021-2022.
Failure to thrive so far using anthropometric measurements according to conventional indices measured separately, while malnutrition cannot stand alone. Measurement of failure to thrive using CIAF is needed to complement more comprehensive anthropometric failures. This study aims to determine the relationship between failure to thrive and the development of children aged 24-59 months in the stunting locus village of the Sungai Limau Health Center work area. Using a cross-sectional study design with simple random sampling techniques, chisquare analysis and logistic regression risk factor models with a sample of 105 children aged 24-59 months. Children experienced dubious development as much as 31.4%, corresponding development 68.6%, failure to grow as much as 29.5% and normal 70.5%. Bivariate results showed an association between failure to thrive with child development (p = 0.028), failure to thrive was associated with gross motor development (p = 0.002) and speech skills (p = 0.050). Other variables related to child development were maternal education (p = 0.002), maternal employment (p = 0.003), income (p = 0.003), exclusive breastfeeding (p = 0.0034), and stimulation (p = 0.0005). Multivariate analysis showed failure to thrive remained consistently associated with child development (p = 0.002). Failure to thrive was associated with dubious development after controlling for several covariate variables. The implementation of early detection programs for child growth and development requires cooperation and commitment across the health and education sectors, namely integrating posyandu and Early Childhood Education activities.
Previous studies have reported that there are long-term effects of quitting smoking on weight gain which also exacerbate prehypertension, but the effects are unclear. This study aims to determine the association of smoking and obesity on the incidence of prehypertension in young adults in Indonesia. This quantitative research is a cross-sectional analytic study using secondary data from Riskesdas 2018 with samples of 17,698 and Cox regression analysis. The results of this study explain that the prevalence of prehypertension in young adults in Indonesia is 52.61%. Multivariate analysis showed non-smokers and obese young adults had the greatest risk of developing prehypertension, which was 1.33 times. However, a decreasing effect was found in young adults who smoked and were obese on the incidence of prehypertension, which was 1.17 times, and a protective effect was found in those who smoked and were not obese (PR=0.88) due to the antagonistic interaction of smoking and obesity on prehypertension by 3.42%. Check blood pressure using applications on smartwatches and smartphones in young adults who smoke and focus on those who are obese plus increasing the implementation of Posbindu PTM in public places and promoting health through social media.
Coronavirus Disease 2019 (COVID-19) is a new infectious disease which has been a pandemic for almost three years (2020-2023). One of the risk factors for COVID-19 mortality is a history of comorbid hypertension. This study was conducted to determine the relationship between comorbid hypertension and mortality among COVID-19 confirmed cases in Indonesia for the period January-June 2022 by comprehensively controlling potential confounder variables, more complete data sources and a larger sample size. The study design was a case control study. Data sourced from the Ministry of Health's Online Hospital Information System (SIRS Online) report for the period January-June 2022. Confirmed cases of COVID-19 with comorbid hypertension have a significantly higher risk of death by 1,57 times compared to confirmed cases COVID-19 without comorbid hypertension after controlling for age, vaccination status, viral variants, and diabetes mellitus (OR=1,57; 95%CI:1,20-2,05; p=0,001). There was a difference risk of death in the non-Omicron infection group (OR=7,68; 95%CI:1,15-51,38; p=0,036) and Omicron group (OR=1,52; 95%CI:1,16-1,99; p=0,002) after controlling for age, vaccination status, and diabetes mellitus. There was a difference risk of death in the unvaccinated group (OR=1,52; 95%CI:1,02-2,26; p=0,037), 1-dose vaccine group (OR=1,85; 95%CI:0,84-4,08; p=0,125), 2-dose vaccine group (OR=1,66; 95%CI:1,08-2,54; p=0,020) and booster vaccine group (OR=0,21; 95%CI:0,03-1,48; p=0,118) after controlling for age, viral variants, and diabetes mellitus. Joint efforts from the government and community are needed in the prevention and control of comorbid hypertension to prevent mortality from COVID-19 infection.
Analysis of Outpatient Installation Pharmacy Waiting Time at Special Hospital of Drug Addiction Jakarta in 2023 Abstract Prescription services waiting time in outpatient installations is one of the indicators for evaluating the performance of pharmaceutical installations that affects the quality of hospital services. Hospitals need to effort that prescription services waiting time meet the Minimum Service Standards (SPM). Through the Lean method with the Value Stream Mapping approach, this study aims to determine the prescription service procedures at the outpatient installation of RSKO Jakarta, identify value added and non-value added and waste that occurs so that factors that cause waste can be analyzed which can be prevented through the strategy recommendations obtained. This is a qualitative research with data collection obtained through observing and recording the e-prescriptions services waiting time at the RSKO outpatient installation, extracting in-depth information from informants and reviewing documents. Observations were made on 20 concoction medicine recipes and 10 concoction medicine recipes. The selection of informants was carried out using a purposive sampling technique and interviews were conducted with patients to obtain value from the customer's perspective according to the principles of the Lean method. The data obtained is then analyzed to obtain the factors affecting the prescription services waiting time duration using a fishbone diagram then a scoring system is carried out by assessing the urgency, severity and growth aspects of the cause problem so that priority recommendations can be formulated. The results of research conducted in April-May 2023 found that the average waiting time for prescription drug services was 49.25 minutes (VAR 17.5%) and for concoction drugs 80.2 minutes (VAR 33%), which means that it still exceeds the SPM set by KMK No. 128 of 2009 (no concoction drug recipe < 30 minutes, concoction drug recipe < 60 minutes). Some of the factors that cause waste are inefficiency in human resources, pharmaceutical inventory systems that have not been automated, inadequate evaluation/monitoring of drug use, the absence of a separate system for emergency prescription services, prescription service SPO that has not been adjusted with the establishment of prescription response time quality standardsfor each process, networks information system that frequently down/loads repeatedly and patient’s interruption for asking information. It is hoped that in the future an improvement strategy can be carried out to improve the waiting time for prescription services; increasing HR efficiency through arrangements so that during peak hours pharmaceutical HR focuses on working on the duties and functions of prescription services, facilitating a pharmaceutical inventory system with an automated system, implementing an evaluation system for monitoring drug use more effectively so that procurement planning becomes more accurate, regulation separates prescription services from the emergency room, providing SPO in accordance with prescription service implementation, separate the information system network between patient services and office and providing reachable information for pastient (visual management).
Background. Chemotherapy is a therapeutic modality for elderly with cancer which can pose elderly, especially frail patients, to fatal side effect. To date, there is no prediction model incorporating frailty in clincal practice. This study aims to develop prediction model which includes frailty state evaluation in predicting severe chemotoxicity in elderly. Methods. A retrospective cohort study using secondary data of elderly underwent chemotherapy during 2019-2021 was conducted in Cipto Mangunkusumo Hospital. Data of determinants ( sex, age, polypharmacy, frailty status, nutritional status, depression, cognitive status, cancer type, polychemotherapy, and functional status) and the incidence severe chemotherapy side effect according to grade 3-5 CTCAE were collected. Data was analyzed to develop prediction model with Cox regression using SPSS Results. Of 193 subjects, most of them are male, with median age of 65.6 (IQR 60-82) years old. Severe chemotoxicity was found in 36% of the subjects. Prediction model consists of polypharmacy, number of chemotherapy drugs, cancer type and frailty status was developed. The model has AUC of 0.79 (95% CI 0.70-0.88), p value 0,01 Conclusion. A prognostic Model consists of polypharmacy, number of chemotherapy drugs, cancer type and frailty status can predict incidence of severe chemotoxicity in elderly with AUC 0.79 (95%CI 0.70-0.88)
One of the important business processes in hospital organizations is the management of pharmaceutical supplies. The management of pharmaceutical supplies management at RSPG Cisarua Bogor involves teams starting from the stages of planning, procurement, storage and distribution. Internal obstacles in the process of planning and procuring pharmaceutical supplies are obstacles that can be controlled by internal intervention in business processes within the hospital. This research focuses on proposed changes in the management system of pharmaceutical supply management, especially planning and procurement within RSPG Cisarua Bogor. Intervention in the procurement process using lean six sigma as an evaluation tool to determine weak points in this study only reached the improvement stage. Data collection used in-depth interviews with informants related to the process of planning and procuring pharmaceutical supplies, observation and document tracing then ended with group discussions to determine mutual agreement. The results showed that the absence of fixed procedures in the process of procuring pharmaceutical supplies made the procedure run longer and there was no benchmark of efficiency in the system. Tools in the process of procuring pharmaceutical supplies are also absent so that internal communication between departments even in the same team is low. At the improve stage of lean six sigma produced proposals for changes to standard operating procedures for routine pharmaceutical supply procurement processes, the use of procurement efficiency indicators and the use of ABC VEN as a tool in the pharmaceutical supply procurement process. Proposed improvements in the use of ABC VEN and planning efficiency indicators to address waste over production identified during the pharmaceutical supply planning process. Proposed establishment of new standard operating procedures containing timelines, tools for grouping pharmaceutical supplies based on ABC VEN and procurement efficiency indicators to overcome waste waiting in the pharmaceutical supply procurement process.
Implementation of the JKN program should be able to increase people's access to health services. However, disparities in health services still exist. The study aims to analyze the utilization of health services based on regional status for JKN participants in Indonesia from 2019 to 2021 and also examine the factors that influence the utilization of inpatient healthcare for JKN participants in Indonesia. This research uses Susenas secondary data for 2019, 2020 and 2021. Data were analyzed bivariately and multivariately using the Binary Regression method using a logit model. The results showed that regional status is significantly related (p-value 0.000 <0.05) to health utilization of JKN participants from 2019 to 2021. Respondents who live in urban areas in 2019 have 1,141 times, 1,127 times in 2020 and 1,127 times in 2021 higher odds ratio than respondents who live in rural areas. Age, gender, educational status, marital status, employment status, economic status and province are significantly related (p-value 0.000 <0.05) to the utilization of inpatient healthcare for JKN participants.
Hospitals have an obligation to prevent patient safety, one of which is to prevent the risk of patients falling. Compliance with efforts to prevent the risk of patients falling in the emergency room and children's room is still not up to standard Compliance is influenced by hedonic goals, gain goals, and normative goals. The purpose of this study was to analyze factors related to compliance with fall risk prevention efforts at Wonosari Hospital Yogyakarta. This study used explanatory sequential mixed method with cross sectional research design. The results of the study were that there was a significant relationship between motivation, reward, SOP and compliance with efforts to prevent the risk of patients falling at Wonosari Hospital Yogyakarta. The dominant factor in the study was motivation. This study recommends conducting regular and continuous momitoring and evaluation, the existence of reward punishment associated with service services, providing greater opportunities for self-development for nurses, modify the patient's bed by closing the hole in the handril of the pediatric patient's bed, supervise the head of the room, reward and punishment for PJ shifts, senior shift PJs sharing knowledge and guiding junior nurses, and attaching risk assessments of falling patients at the nurse station.
Quantitative research with a cross-sectional design (Cross Sectional) will examine the achievements of health service programs for people at risk of getting HIV using the Malcolm Baldrige approach at the Depok City Health Center. The purpose of this study was to determine the performance of the health service program for people at risk of getting HIV using the Malcolm Baldrige approach. The population of this study were all Community Health Centers in Depok City, totaling 38 Health Centers. The research instrument used questionnaires and interviews which were made based on standard references from Malcolm Baldrige's criteria which had been translated into Indonesian by previous researchers and adapted to the research objectives. There are 6 independent variables, namely leadership, strategic planning, focus on customers, measurement analysis and knowledge management, focus on workforce and process management, while the dependent variable is achievement of minimum service standards in health services for people at risk of getting HIV. Data analysis used univariate data analysis to look at the frequency distribution of performance results, bivariate data analysis looked at the relationship between 6 (six) Malcolm Baldrige criteria and the achievement results of the minimum service standard for the Health Service Program for people at risk of getting HIV at the Depok City Health Center and multivariate analysis to find the most common factor Dominantly affect the achievement of minimum HIV service standards. The majority of Univariate research results fall into the category of lacking leadership (53.07%), strategic planning (46.21%) focus on customers (43.84%), measurement, analysis and knowledge management (44.21%), workforce (47.85%) and process management (47.49%) while the results of the minimum service standards for people at risk of getting HIV (69.86%) are in the sufficient category. In the bivariate analysis all independent variables have a strong and positive relationship to the dependent variable. In the multivariate analysis there is a strong correlation between HIV MSS achievement and leadership and customer focus variables (R=0,749) and R square =0,561 where the most dominant variable is Customer Focus (48.5%).
