Coronavirus Disease 2019 (COVID-19) merupakan penyakit infeksi baru yang selama hampir tiga tahun (2020-2023) menjadi pandemi. Salah satu faktor risiko kematian COVID-19 adalah riwayat komorbid hipertensi. Penelitian ini dilakukan untuk mengetahui hubungan komorbid hipertensi dengan kematian kasus konfirmasi COVID-19 di Indonesia periode Januari-Juni 2022 dengan mengontrol variabel potensial confounder secara komprehensif, sumber data lebih lengkap dan jumlah sampel lebih besar. Desain studi yang digunakan adalah kasus kontrol. Data bersumber dari laporan Sistem Informasi Rumah Sakit Online (SIRS Online) Kementerian Kesehatan periode Januari-Juni 2022. Kasus konfirmasi COVID-19 dengan komorbid hipertensi memiliki risiko kematian sebesar 1,57 kali lebih tinggi secara bermakna dibandingkan dengan yang tidak memiliki komorbid hipertensi setelah dikontrol oleh variabel usia, status vaksinasi, varian virus, dan diabetes melitus (OR=1,57; 95%CI:1,20-2,05; p=0,001). Ada perbedaan risiko kematian pada kelompok infeksi bukan Omicron (OR=7,68; 95%CI:1,15-51,38; p=0,036) dan Omicron (OR=1,52; 95%CI:1,16-1,99; p=0,002) setelah dikontrol variabel usia, status vaksinasi, dan diabetes melitus. Ada perbedaan risiko kematian pada kelompok belum vaksin (OR=1,52; 95%CI:1,02-2,26; p=0,037), vaksin 1-dosis (OR=1,85; 95%CI:0,84-4,08; p=0,125), vaksin 2-dosis (OR=1,66; 95%CI:1,08-2,54; p=0,020) dan vaksin booster (OR=0,21; 95%CI:0,03-1,48; p=0,118) setelah dikontrol variabel usia, varian virus, dan diabetes melitus. Perlu upaya bersama dari pemerintah dan masyarakat dalam pencegahan dan pengendalian komorbid hipertensi untuk mencegah kematian COVID-19.

Corona Virus Disease 2019 (COVID-19) merupakan suatu penyakit menular yang disebabkan oleh Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2). COVID-19 merupakan penyakit nomor satu dalam daftar kasus terbanyak dirawat inap sekaligus penyebab kematian tertinggi di Rumah Sakit Umum Daerah (RSUD) Tarakan pada tahun 2022. Oleh karena itu, penting dilakukannya penelitian terkait faktor-faktor yang berhubungan dengan kematian pasien COVID-19 di Layanan Rawat Inap RSUD Tarakan tahun 2022. Penelitian ini menggunakan desain kohort retrospektif dengan data sekunder dari laporan Electronic Health Records RSUD Tarakan. Variabel yang diteliti adalah karakterisik pasien, penyakit penyerta, komplikasi, dan tatalaksana kasus COVID-19. Jumlah sampel yang memenuhi kriteria untuk dianalisis sebanyak 550. Hasil analisis multivariat dengan cox regression time constant menemukan faktor risiko dalam kejadian kematian pasien COVID-19 di Layanan Rawat Inap RSUD Tarakan tahun 2022 adalah derajat keparahan (aRR 2,95; 95%CI 1,61-5,39), penyakit ginjal kronik (aRR 3,19; 95%CI 1,62-6,31), serta penggunaan ventilasi mekanik invasif <96 jam (aRR 3,50; 95%CI 1,97-6,22) dan ≥96 jam (aRR 2,44; 95%CI 1,24-4,80). Sedangkan faktor protektifnya adalah penggunaan antivirus favipiravir (aRR 0,33; 95%CI 0,16-0,70). Berdasarkan hasil tersebut, peneliti menyarankan rumah sakit untuk memperbaiki clinical pathway dan standar operasional prosedur yang memperhatikan faktor risiko dan protektif kematian COVID-19.

In 2022, COVID-19 was the leading cause of inpatient admissions and deaths at Tarakan Regional General Hospital (RSUD Tarakan). This highlights the importance of investigating factors related to mortality of COVID-19 inpatient at RSUD Tarakan in 2022. This study uses a retrospective cohort design with secondary data from the hospital's Electronic Health Records (EHR). The variables studied included patient’s characteristics, comorbid, complications, and the management of COVID-19 cases. A total of 550 samples met the criteria for analysis. Multivariate analysis using cox regression with time constant identified several risk factors for COVID-19-related deaths at RSUD Tarakan in 2022, including severity (aRR 2.95; 95%CI 1.61-5.39), chronic kidney disease (aRR 3.19; 95%CI 1.62-6.31), and the duration of invasive mechanical ventilation for <96 hours (aRR 3.50; 95%CI 1.97-6.22) and ≥96 hours (aRR 2.44; 95%CI 1.24-4.80). Meanwhile, protective factors include the use of favipiravir (aRR 0.33; 95%CI 0.16-0.70) and remdesivir (aRR 0.69; 95%CI 0.38-1.26). Based on these findings, the researchers recommend that hospital improve clinical pathways and standard operating procedures that take into account the risk and protective factors for COVID-19 mortality

Laporan WHO pada akhir bulan April 2021th menyebutkan bahwa terjadi peningkatan angka kematian COVID-19 dalam 6 minggu secara global dan dalam 3 minggu di regional Asia Tenggara. Sejalan dengan itu, angka kematian COVID-19 di Indonesia merupakan yang tertinggi di Asia Tenggara. Laporan kematian akibat COVID-19 juga terus terjadi setiap harinya di Kota Depok berkisar 1-10 kematian per hari. Berbagai studi dari dalam maupun luar negeri menyebutkan bahwa usia, diabetes mellitus, dan hipertensi memiliki asosiasi yang paling kuat terhadap kejadian kematian akibat COVID. Hasil penelitian ini juga menunjukkan bahwa jumlah gejala klinis memodifikasi hubungan antara hipertensi dengan kematian penderita COVID-19. Diketahui bahwa kombinasi antara penderita COVID19 yang hipertensi dan gejala klinis >3 meningkatkan risiko kematian 18 kali (pvalue 0,01) dibandingkan penderita COVID-19 tanpa hipertensi dan gejala klinis ≤3
