Abstrak: Pemerintah daerah Kabupaten Kudus telah berkomitmen untuk percepatan pencegahan kasus stunting dan berupaya dalam mewujudkan Kudus zero stunting dengan menerbitkan Peraturan Bupati Kudus No. 53 tahun 2020 tentang Percepatan Pencegahan Stunting di Kabupaten Kudus. Namun pelaksanaan konvergensi di Kabupaten Kudus belum berjalan optimal. Hasil rapat kerja penilaian aksi konvergensi pencegaham stunting terintegrasi di Jawa Tengah tahun 2020, kapasitas pelaksanaan analisis situasi dan inputing serta kualitas analisis situasi dengan keterlibatan lintas sektor di Kabupaten Kudus belum optimal, rembug stunting belum dilakukan secara intensif, dan belum adanya regulasi untuk memperkuat publikasi pencegahan stunting. Selain itu pandemi COVID-19 menimbulkan adanya refocusing anggaran dari pelayanan dan program yang lain termasuk pencegahan stunting. Tujuan penelitian ini untuk menganalisis implementasi kebijakan percepatan pencegahan stunting pada masa pandemi COVID-19 di Kabupaten Kudus. Penelitian ini merupakan penelitian kualitatif. Pengambilan data dilakukan melalui wawancara mendalam kepada informan dan telaah dokumen dengan framework Edward III meliputi variabel komunikasi, sumber daya, disposisi, dan struktur birokrasi dan Van Meter dan Van Horn meliputi variabel lingkungan ekonomi, sosial dan politik. Informan penelitian yaitu perwakilan dari perangkat daerah yang tergabung dalam tim percepatan pencegahan stunting pada tingkat kabupaten, kecamatan, maupun desa/kelurahan serta Kader Pembangunan Manusia (KPM). Penelitian dilakukan bulan April hingga Mei 2022. Hasil penelitian menunjukkan bahwa implementasi kebijakan sudah berjalan namun intervensi gizi sensitif belum maksimal, komunikasi yang dilakukan terbatas dikarenakan kondisi pandemi COVID-19, sumber daya yang tersedia cukup memadai, terdapat panduan kebijakan yang telah diketahui dan dipahami oleh pelaksana kebijakan, koordinasi yang terjalin belum optimal, lingkungan ekonomi dan sosial yang belum cukup mendukung dan lingkungan politik cukup mendukung kebijakan. Kesimpulannya implementasi kebijakan percepatan pencegahan stunting pada masa pandemi COVID-19 belum berjalan dengan optimal dengan masih adanya kendala pada variabel sumber daya manusia, koordinasi, serta lingkungan ekonomi dan sosial. Dengan demikian pelaksanaan konvergensi masih perlu ditingkatkan dengan melaksanakan kegiatan secara koordinatif antar lembaga. Sosialisasi kepada masyarakat tetap digencarkan agar tujuan kebijakan dapat tercapai. Diperlukan adanya penyesuaian kebijakan dengan Perpres terbaru tantang percepatan penurunan stunting.
The local government of Kudus Regency has committed to accelerating the handling of stunting cases and is trying to realize Kudus zero stunting by issuing Regional Regulation Number 53/2020 concerning the Acceleration of Stunting Prevention in Kudus Regency. However, the implementation of convergence in Kudus has not run optimally. The results of the work meeting on the assessment of the integrated stunting prevention convergence action in Central Java in 2020, the capacity for implementing situation analysis and inputting as well as the quality of situation analysis with cross-sectoral involvement in Kudus Regency has not been optimal, the stunting discussion has not been carried out intensively, and there are no regulations to strengthen publications of stunting prevention. In addition, the COVID-19 pandemic has led to a refocusing of the budget from other services and programs, including stunting prevention. The purpose of this study is to analyze the implementation of policies to accelerate stunting prevention during the COVID-19 pandemic in Kudus Regency. This research is qualitative research. Data were collected through in-depth interviews with informants and document review with the Edward III framework covering the variables of communication, resources, disposition, and bureaucratic structure and Van Meter and Van Horn covering variables of the economic, social and political environment. Research informants are representatives from regional apparatus who are members of the stunting prevention acceleration team at the district, sub-district, and village/kelurahan levels as well as the Human Development Cadre (KPM). The study was conducted from April to May 2022. The results show that policy implementation has been running but sensitive nutrition interventions have not been maximized, communication is limited due to the COVID-19 pandemic, the available resources are adequate, and there are policy guidelines that are known and understood by policy implementers, coordination is not optimal. , the economic and social environment is not yet sufficiently supportive and the political environment is sufficiently supportive of policies. In conclusion, the implementation of the policy to accelerate stunting prevention during the COVID-19 pandemic has not run optimally, with constraints on human resource variables, coordination, as well as the economic and social environment. Thus, the implementation of convergence still needs to be improved by carrying out activities in a coordinated manner between institutions. Socialization to the community continues to be intensified so that policy objectives can be achieved. This policy need to be adjusted with the latest Presidential Regulation on accelerating stunting reduction.

Abstrak:
Prevalensi stunting di Indonesia berada pada posisi 115 dari 151 negara di dunia pada tahun 2020. Secara nasional, Indonesia menunjukkan penurunan namun masih diperlukan penurunan untuk mencapai target di tahun 2024. Menurut hasil SSGI tahun 2021 prevalensi stunting di Provinsi Jawa Barat sebesar 24,5%. Berdasarkan Buku Profil Informasi Kesehatan Kabupaten Bogor 2019, prevalensi stunting di Puskesmas Ragajaya sebesar 41,98% lebih tinggi daripada prevalensi Kabupaten Bogor yaitu sebesar 19,08%. Ibu hamil merupakan salah satu kelompok sasaran dalam pencegahan stunting. Pencegahan stunting dapat dilakukan sejak 1000 Hari Pertama Kehidupan (HPK) yaitu dari masa kehamilan hingga usia 2 tahun. Stunting dapat terjadi akibat asupan nutrisi ibu hamil kurang. Theory of Planned Behavior (TPB) mencakup strategi untuk meningkatkan kepercayaan ibu terhadap kemampuannya dalam memenuhi kebutuhan nutrisi selama kehamilan. Perilaku pencegahan stunting dapat dipengaruhi berbagai faktor seperti sikap, norma subjektif, dan persepsi kontrol perilaku. Penelitian ini bertujuan untuk mengetahui hubungan sikap, norma subjektif, dan persepsi kontrol perilaku dengan perilaku pencegahan stunting pada ibu hamil di wilayah kerja Puskesmas Ragajaya. Desain penelitian ini cross-sectional. Sampel penelitian sebanyak 90 ibu hamil. Pengambilan data dilakukan pada bulan Desember 2022. Hasil penelitian menunjukkan bahwa sebanyak 65.5% ibu hamil memiliki perilaku pencegahan stunting yang baik. Hasil analisis bivariat menunjukkan sikap (p=0.004), norma subjektif (p=0.045), dan persepsi kontrol perilaku (p=0.001) berhubungan dengan perilaku ibu hamil dalam mencegah stunting. Saran bagi fasilitas kesehatan diharapkan memberikan pendidikan kesehatan secara rutin sebagai salah satu langkah preventif baik melalui pelayanan konsultasi kesehatan ibu hamil, berbagai platform media sosial, serta kegiatan – kegiatan diskusi terkait dengan kesehatan ibu hamil terutama mengenai gizi ibu hamil dalam pencegahan stunting.

In 2020, the prevalence of stunting in Indonesia will be the 115th highest out of 151 countries in the world. Nationally, Indonesia is showing a decline, but a gradual decrease is still needed to reach the target in 2024. According to the results of SSGI in 2021, the prevalence of stunting in West Java Province is 24.5%. Based on the 2019 Bogor Regency Health Information Profile Book, the prevalence of stunting in the Ragajaya Health Center is 41.98%, which is higher than the Bogor Regency prevalence of 19.08%. Pregnant women are one of the target groups in stunting prevention. Stunting can be prevented from the first 1000 days of life (HPK), or from pregnancy to the age of two. Stunting can occur due to the insufficient nutritional intake of pregnant women. The Theory of Planned Behavior (TPB) includes strategies to increase the mother's confidence in her ability to meet nutritional needs during pregnancy. Stunting prevention behavior can be influenced by various factors, such as attitudes, subjective norms, and perceptions of behavior control. This study aims to determine the relationship between attitudes, subjective norms, and perceptions of behavior control with stunting prevention behavior in pregnant women in the working area of the Ragajaya Health Center. The research design is cross-sectional. The research sample consisted of 90 pregnant women. Data collection was carried out in December 2022. The results showed that 65.5% of pregnant women had good stunting prevention behavior. The results of the bivariate analysis showed that attitude (p = 0.004), subjective norm (p = 0.045), and perceived behavioral control (p = 0.001) were related to pregnant women's behavior in preventing stunting. Suggestions for health facilities are expected to provide routine health education as one of the preventive measures both through pregnant women's health consultation services, various social media platforms, and discussion activities related to the health of pregnant women, especially regarding nutrition for pregnant women in preventing stunting.

Stunting di Kabupaten Bantaeng masih menjadi tantangan dengan prevalensi 15,8%. Keberhasilan dalam menekan angka ini menjadi dasar untuk menelaah peran strategis KPM sebagai garda terdepan dalam program konvergensi. Tujuan penelitian ini adalah untuk menelaah implementasi peran KPM dalam upaya pencegahan stunting serta mengidentifikasi faktor pendukung dan penghambat efektivitasnya di Kabupaten Bantaeng. Penelitian ini menggunakan desain kualitatif fenomenologi dengan total 24 informan yang terdiri dari 7 KPM, 3 kepala desa, 5 tenaga kesehatan, 3 kader posyandu, dan 6 perwakilan masyarakat sasaran. Pengumpulan data dilakukan melalui wawancara mendalam dan telaah dokumen , yang kemudian dianalisis secara tematik. Hasil penelitian menunjukkan peran KPM sangat efektif dalam memfasilitasi akses terhadap layanan KIA, sanitasi, dan jaminan kesehatan, berkat kolaborasi lintas sektor dan dukungan penuh pemerintah desa. Namun, masih kurang optimal dalam intervensi yang bersifat edukatif untuk mengubah perilaku, seperti pada aspek konseling gizi dan partisipasi PAUD. Faktor penghambat utamanya adalah norma sosial yang kuat di masyarakat dan keterbatasan sumber daya untuk program edukasi yang terstruktur. Penelitian ini menyimpulkan implementasi peran KPM efektif pada intervensi yang didukung program pemerintah yang konkret dan terukur, namun lemah dalam mengubah pengetahuan dan norma sosial. Lebih lanjut, ditemukan pula kesenjangan antara kualifikasi pendidikan formal KPM dengan kompleksitas tugas edukasi dan konseling gizi yang harus diemban secara efektif. Disarankan agar Pemerintah Daerah mengembangkan kampanye sosialisasi untuk mengubah norma sosial dan mengalokasikan dana untuk program edukasi terstruktur. Selain itu, Dinas Pemberdayaan Masyarakat dan Desa (DPMD) perlu mengevaluasi kembali standar rekrutmen KPM dan merancang pelatihan terstandar dan berkelanjutan untuk meningkatkan kapasitas KPM.


 

Stunting remains a challenge in Bantaeng Regency, with a prevalence of 15.8%. This study explores the strategic role of Human Development Cadres (KPM) in the stunting prevention program and identifies supporting and hindering factors affecting their effectiveness. Using a qualitative phenomenological approach, the study involved 24 informants: 7 KPM, 3 village heads, 5 health workers, 3 Posyandu cadres, and 6 community representatives. Data collection was done through in-depth interviews and document reviews, followed by thematic analysis. Findings show that KPM are effective in improving access to maternal and child health services, sanitation, and health insurance due to strong cross-sector collaboration and village government support. However, their role is less optimal in educational interventions, particularly in nutrition counseling and increasing participation in early childhood education (PAUD). Key challenges include persistent social norms and limited resources for structured educational programs. There is also a gap between KPM’s formal education levels and the complex tasks of health education and counseling. The study concludes that KPM play an effective role in interventions backed by clear government programs but face difficulties in changing behaviors and social norms. It recommends that local governments strengthen social norm change campaigns and allocate funding for structured education. Furthermore, the Community and Village Empowerment Office (DPMD) should review recruitment standards and provide ongoing, standardized training to enhance KPM capacity.
