Abstrak:
Kecelakaan transportasi darat menjadi penyebab kematian urutan ke-8 di dunia dengan jumlah kematian mencapai 1,35 juta orang per tahun (WHO, 2021). Data Korlantas Polri tahun 2019 ? 2021 menunjukkan kasus kecelakaan transportasi darat di Indonesia berada di atas angka 100 ribu setiap tahunnya. Untuk mengetahui penyebab kecelakaan secara komprehensif, penelitian ini mengidentifikasi faktor kegagalan laten dan kegagalan aktif yang berkontribusi pada kasus kecelakaan transportasi darat di Indonesia. Penelitian ini menggunakan data sekunder yaitu 320.084 laporan kasus kecelakaan lalu lintas tahun 2019 ? 2021 milik NTMC Korlantas Polri. Metode yang digunakan adalah kualitatif dengan melakukan analisis berdasarkan Swiss Cheese Model. Hasil penelitian ini menunjukkan faktor kegagalan laten pada kecelakaan transportasi darat di Indonesia tahun 2019 ? 2021 yaitu kondisi awal kendaraan rem tidak berfungsi (5,7%), terjadi di jalan dengan kondisi baik (92,5%), terjadi pada kondisi cuaca cerah (92,3%), dan kondisi pencahayaan terang/jelas (79,1%). Faktor kegagalan aktif pada kecelakaan kecelakaan transportasi darat di Indonesia tahun 2019 - 2021 adalah usia pengendara berusia 22 ? 29 tahun (15,7%), pengendara berpendidikan SLTA/sederajat (48,4%), pengendara yang tidak memiliki SIM (28,3%), dan pengendara yang ceroboh terhadap lalu lintas dari depan (19,0%).
Road transportation accidents are the eighth leading cause of death worldwide, reaching 1.35 million people annually (WHO, 2021). Data from the NTMC Korlantas Polri for 2019-2021 shows that the number of road transportation accidents in Indonesia is above 100 thousand every year. This study identifies latent and active failure factors contributing to Indonesia's land transportation accidents to determine the causes of accidents comprehensively. This study uses secondary data with 320,084 traffic accident case reports in 2019-2021 belonging to the NTMC Korlantas Polri. The method used is qualitative by analyzing based on the Swiss Cheese Model. The results of this study indicate latent failure factors in road transportation accidents in Indonesia in 2019-2021, namely the initial condition of the brake vehicle not functioning (5.7%), occurring on roads with suitable conditions (92.5%), occurring in sunny weather conditions ( 92.3%), and bright/clear lighting conditions (79.1%). Active failure factors in land transportation accidents in Indonesia in 2019 - 2021 are the age of drivers aged 22 ? 29 years (15.7%), drivers with a senior high school education/equivalent (48.4%), drivers who do not have a driver's license (28.3 %), and drivers who are careless towards traffic from the front (19.0%).

Abstrak:
Tesis ini membahas tentang Kinerja Sistim Manajemen Keselamatan TransportasiDarat (SMKTD) yang terdiri dari 5 (lima) pilar yaitu Manajemen Pengemudi,Manajemen Risiko Perjalanan, Manajemen Peralatan dan Kendaraan, ManajemenHSSE, Manajemen Kontraktor dan insiden mobil tangki. Penelitian ini bertujuanuntuk mengetahui hubungan kinerja masing-masing dan keseluruhan pilar padaSistem Manajemen tersebut dengan penurunan insiden di PTMS. Penelitianbersifat deskriptif analitikal dengan menggunakan metode statistik analisisunivariat, bivariat, multivariat dan dilengkapi dengan analisis kualitatif. Hasilpenelitian menyarankan agar dapat dilakukan awareness kepada semua pihak ataspelaksanaan Sistem Manajemen Keselamatan Transportsi Darat melalui seminar,pelatihan dan pembinaan pengemudi, ketersediaan pelaporan data insiden yangakurat dan penyediaan media informasi yang efektif sehingga diharapkan dapatmeningkatkan kepedulian dan konsistensi dalam pelaksanaan Sistem ManajemenKeselamatan Transportasi Darat.Kata kunci:Sistim Manajemen Keselamatan Transportasi Darat, Manajemen Pengemudi,Manajemen Risiko Perjalanan, Manajemen Peralatan dan Kendaraan, ManajemenHSSE, Manajemen Kontraktor, awareness.

Abstrak:
Niaga minyak dan gas khususnya distribusi BBM memiliki potensi risikokecelakaan kerja yang tinggi. Proses kerja ini memiliki potensi risiko kecelakaanlalu lintas dan risiko lainnya yaitu ledakan, kebakaran dan lain-lain dari BBM itusendiri. Oleh karena itu perlu dilakukan pengendalian melalui Sistem ManajemenKeselamatan Transportasi Darat (SMKTD. Tujuan dari studi ini untuk mengetahuigambaran penerapan Sistem Manajemen keselamatan transportasi darat diPerusahaan Niaga BBM dan LPG PT.X TBBM Plumpang tahun 2016. Studi inibersifat deskriptif kualitatif melalui penilaian checklist, wawancara responden danobservasi lingkungan kerja.Penelitian ini menghasilkan pelaksanaan sistem sudahcukup baik namun perlu dilakukan pengawasan dan audit berkala sertapembaharuan baik pada prosedur maupun dokumen terkait lainnya.Kata kunci: distribusi BBM, Sistem Manajemen Keselamatan Transportasi Darat(SMKTD), kecelakaan lalu lintas.
Distribution of oil and gas via land transportation has a potential risk, such as roadaccident, explosion and fire due to oil and gas chemical properties. Therefore, it isnecessary to control the risk through implementation of Land Transport SafetyManagement System (LTSMS). This study was aimed to describetheimplementation of LTMSM at one of Oil and Gas distributor PT.X TBBMPlumpang 2016. This is a descriptive and qualitative assessment study using achecklist, interview respondents and work area observation. This research hasshown that the LTSMS has been properly implemented,but supervision, periodicaudits and written documents need to be improved.Keywords: oil and gas distribution, Land Transport Safety Management System(LTSMS), road accidents.
