Tesis ini membahas tentang penilaian risiko yang dilakukan di Stasiun Pengumpul Gas Y PT X. Metode penilaiaan risikonya menggunakan penggabungan 2 (dua) analisis yaitu analisis risiko proses melalui metode Hazops dan analisis risiko peralatan melalui metode risk based inspection (RBI). Penelitian ini adalah penelitian observational dengan pendekatan analisis risiko semikuantitatif. Hasil penelitian ini memberikan gambaran tingkat risiko proses dan peralatan di Stasiun Pengumpul Gas Y, dimana tingkat risiko tertinggi ada pada proses dan peralatan tanki penampung kondensat. Tingkat risiko tersebut diperoleh dari kombinasi kemungkinan kejadian (likelihood) dan konsekuensi/keparahan dampak (severity). Selain itu dihasilkan butir-butir rekomendasi untuk mengendalikan risiko sampai pada risiko yang dapat diterima. Kata kunci : Risiko, likelihood, severity/konsekuensi.
The focus of this research is risk assessment carried out at Gas Gathering Station Y in PT X. Risk assessment method applied are combination between process hazard analysis by using Hazops method and equipment risk analysis by using risk based inspection (RBI) method. The research is observational by semiquantitative analysis approach. The research gives description regarding process and equipment risk level at Gas Gathering Station Y, which is the highest risk level is on the process and equipment named condensate storage tank. Risk level is calculated by combining likelihood and severity/consequence value. The research also recommend several risks control to reduce risk to be acceptable risk. This study assess the consequences of the chemical release from the pressurized Liquefied Petroleum Gas (LPG) storage tank at PT. LMI. This study is kuantitative. Simulation of release using ALOHA (Areal Locations of Hazardous Athmospheres) software based on primary and secondary datas on the normal operation. ALOHA give result in a distance range of dispersion, fire and explosion. Each distance range is classified into three categories: red, orange and yellow based on the level of severity. These study's result is indicate that schools, medical clinic, factories, and homes around PT. LMI being the high-risk population affected by the release of chemicals from the LPG storage tank.

Occupational health and safety issues at SMEs were caused by lack of riskmanagement. The purpose of this study was to assess level of OHS risk at five (5)of metal-based workshop at Gunung Sari Village, Bogor, 2016. W.T Fine methodwas used for analyzing the risk level by scoring the level of consequence,exposure, and likelihood of each risk. The results showed that ergonomics andchemical hazard were categorised as very high risk level.

Risiko kecelakaan kerja dapat dicegah atau diminimalisir dengan melakukan manajemen risiko kecelakaan di tempat kerja.Salah satu tahapan dalam manejemen risiko adalah penilaian risiko.Analisis risiko digunakan untuk mengevaluasi risiko apakah risiko tersebut memerlukan tindak lanjut atau tidak. Permasalahan dalam penelitian ini adalah adanya risiko yang tinggi dalam kegiatan Bongkar Muat pada gudang tempat penimbunan sementara PT SLI, oleh sebab itu perlu dilakukan kegiatan analisis. Jenis penelitian ini adalah penelitian kualitatif dengan strategi studi kasus.Penelitian ini dilakukan dengan mengidentifikasi bahaya dan risiko, kemudian melakukan analisis data agar dapat diketahui tingkat risiko dengan mengacu pada standar Australia/ new Zealand atau AS/NZs 4360 : 2004. Berdasarkan hasil identifikasi Risiko maka peneliti mengevaluasi risiko kegiatan ini dan didapati untuk proses Mengangkat, sebanyak 9 potensi bahaya terdapat 6 potensi yang dinilai sebagai risiko Tinggi (High). Pemilahan barang terdapat 3 potensi dengan risiko Tinggi (High).Memindahkan dan Memasukkan terdapat 4 dinilai dengan risiko Tinggi (High). Saran yang dapat diberikan yaitu perusahaan harus menyediakan alat dan fasilitas yang dibutuhkan seperti, Forklift, Roll cage, Rak, APD dan juga pengendalian secara Administratif. The risk of workplace accidents can be prevented or minimized by performing the risk management of accidents in the workplace. One of the stages in the management of risk is the risk assessment. Risk analysis is used to evaluate whether the risk are requiring follow-up or not. Problems in this study is that there are some high risk in the activities of loading-unloading at PT SLI warehouse, therefore it is necessary to analysis activities. This type of research is a qualitative research strategy case study. This research was conducted by identifying hazards and risks, then perform the data analysis in order to know the level of risk with reference to the standard Australian / New Zealand or AS / NZS 4360: 2004. The result after Identification and evaluated the risk of these activities were found such as, for Lifthing activities there are nine potential hazards, and 6 potential areassessed as high risk (High). For shorting activitiesthere are 3 potential with High risk. Moving and Entering found that there are 4 was assessed as High risk.Suggestion which can be given that the company should provide the necessary tools and facilities such as, Forklift, Roll cage, rack, PPE and also the Administrative control. This study focused on hazard identification and risk analysis at SME for furniture making at Kelapa Dua, Depok in 2017. This study aimed to know risk level of processes in making furniture. Risk assessment done by using semi quantitative risk analysis of W.T. Fine 1971 by scoring consequences, exposure, and likelihood.
