Data of good quality on Minimum Service Standard of Maternal and Child Health (MSS MCH) is required to be utilized as a basis for planning. One of the data assessment models that has been developed is the Routine Data Quality Assessment (RDQA) model, which is an adaptation of the WHO model adopted by Pusdatin. No research has been conducted in Depok City until recently. Therefore, this study aimed to assess SPM data on maternal health, specifically indicators such as the K4 and Linakes, in Depok City. The assessment was conducted by considering the indicators of completeness, timeliness, internal consistency, external consistency, and accuracy as well as the organizational factors that influence them. The results showed that data completeness in Depok City was good, timeliness could not be optimally analyzed, internal consistency was relatively good although there was some inconsistent data in some puskesmas, external consistency was good, and lastly, inaccuracy was found in one of the health centers in Depok City.. In addition, this study also found organizational issues surrounding data collection that could potentially affect data quality.

ABSTRACT The quality and accuray of immunization data is a problem that is often found in health centers and the Department of Health, despite high immunization coverage but in the absence of immunization data quality assessment is routinely reported that the percentage of coverage is not necessarily high data quality is also high. This study aims to describe the data quality routine immunization based methods Data Quality Self Assessment (DQS) in the city of Padang Panjang in 2013. Research is a qualitative research method that uses DQS from WHO. The study was conducted at the Department of Health and all health centers in the city of Padang Panjang is Gunung health center, Kebun Sikolos, Koto Katik and Bukit Surungan. Data obtained from the conducted interviews, observation and document reviews. Immunization data quality component data usage shows that the quality is still low, the data underutilized for suggestions for improvement of recording and reporting of immunization. The accuracy of the data found the village to the health center coverage more than succesfuly verified (overreporting) for reporting the results of DPT-HB3 and Measles. For lack of reporting still occurs accessory reporting date. Monitoring system do not optimal for evaluating databases must addres satisfying immunization program, for the system of recording and reporting of immunization data better.;The quality and accuray of immunization data is a problem that is often found in health centers and the Department of Health, despite high immunization coverage but in the absence of immunization data quality assessment is routinely reported that the percentage of coverage is not necessarily high data quality is also high. Kata kunci: Alur pencatatan dan pelaporan, integrasi data, kematian, kualitas data, registrasi vital, sebab kematian Vital registration is actually the best source of information for development in health. However, in Indonesia the vital registration has not gone well. It is estimated that underreporting of up to 40% for the reporting of death which is a vital event. In addition, the management of vital events including death data is separated by several institutions. The purpose of this study is to confirm the quality of death data, to know the flow of recording and reporting of death data and cause of death data abad to know the integration of death data in Depok. This research is a qualitative research with case study design. The results showed that the death data in Kelurahan (village office) is a valid and complete data as a measure of death data. Primary Health Care facility can coordinate with Kelurahan to obtain death data so that verbal autopsy can be done to get the cause of death. If this is done then the Health Department will obtain death and death data for all deaths at the district / city level as a basis for health program intervention. Key words: Recording and reporting flow, data integration, data quality, death, vital registration, cause of death
