Abstrak:
Kelahiran prematur merupakan penyebab kematian neonatal ketiga terbanyak di Indonesia. Kelahiran prematur dapat meningkatkan morbiditas dan mortalitas neonatal/bayi sehingga dapat berdampak pada indikator kesehatan Indonesia. Penelitian ini bertujuan untuk mengidentifikasi determinan kelahiran prematur di wilayah perkotaan dan perdesaan Indonesia. Data berasal dari Survei Kesehatan Indonesia 2023 dengan menggunakan desain studi potong lintang. Sampel penelitian ini adalah perempuan umur 10-54 tahun yang pernah kawin, mempunyai pengalaman reproduksi dalam kurun waktu lima tahun terakhir untuk anak terakhir dengan total sampel 32.288 responden. Penelitian ini menggunakan uji Chi Square dan regresi logistik ganda dalam analisisnya. Hasil penelitian ini menunjukkan bahwa determinan kelahiran prematur di wilayah perkotaan dan pedesaan yaitu,  kunjungan ANC, kehamilan kembar, hipertensi, dan perdarahan antepartum. Usia, ibu, ketuban pecah dini, dan metode persalinan hanya berhubungan di wilayah perkotaan, sedangkan plasenta previa hanya berhubungan di wilayah perdesaan. Kehamilan kembar menjadi variabel yang berhubungan paling dominan dengan kelahiran prematur di wilayah perkotaan dan perdesaan Indonesia

Preterm birth is the third leading cause of neonatal mortality in Indonesia. It significantly contributes to neonatal morbidity and mortality, thereby affecting the overall health indicators of the country. This study aims to identify the determinants of preterm birth in urban and rural areas of Indonesia. The data were obtained from the 2023 Indonesian Health Survey using a cross-sectional study design. The study sample consisted of 32,288 ever-married women aged 10–54 years who had given birth in the last five years. Data were analyzed using chi-square tests and multivariable logistic regression. The results showed that antenatal care (ANC) visits, multiple pregnancies, hypertension, and antepartum hemorrhage were significant determinants of preterm birth in both urban and rural areas. Maternal age, premature rupture of membranes, and mode of delivery were only associated with preterm birth in urban areas, while placenta previa was only significant in rural areas. Multiple pregnancies were the most dominant factor associated with preterm birth in both settings.

Abstrak:
Kematian pada periode neonatal masih menjadi permasalahan dalam kesehatan masyarakat. Pada tahun 2021 Kementerian Kesehatan melaporkan sebesar 73,1% kematian anak dibawah 5 tahun di Indonesia terjadi pada masa neonatal. World Health Organization (WHO) mengungkapkan bahwa penyebab terbanyak kematian neonatal adalah kelahiran prematur dan salah satu faktor risiko dari kelahiran prematur sebagai penyebab kematian neonatal terbanyak adalah faktor usia ibu, dimana ibu hamil usia remaja lebih berisiko untuk melahirkan bayi prematur. Penelitian ini merupakan penelitian analitik dengan desain kohort retrospektif yang menganalisis hubungan antara kehamilan usia remaja dengan kelahiran prematur. Penelitian ini dilakukan di Rumah Sakit Umum Daerah (RSUD) Cengkareng Jakarta Barat menggunakan data rekam medis pada pasien yang masuk dengan tanda persalinan di RSUD Cengkareng pada periode 1 Januari 2022 – 31 Desember tahun 2022. Populasi dalam penelitian ini adalah seluruh ibu hamil yang masuk rumah sakit dengan tanda persalinan di RSUD Cengkareng Jakarta Barat pada tahun 2022. Sampel terdiri dari dua kelompok, yaitu kelompok ekspose dan kelompok non-ekspose. Kelompok ekspose merupakan kelompok ibu hamil yang datang dengan tanda persalinan pada rentang usia remaja (10-19 tahun), sementara kelompok non-ekspose merupakan kelompok ibu hamil yang datang dengan tanda persalinan pada rentang usia reproduksi sehat (20-34 tahun). Pengambilan sampel dilakukan menggunakan teknik simple random sampling dengan hasil perhitungan akhir sampel sebanyak masing-masing 141 responden pada kelompok ekspose dan kelompok non-ekspose. Data dianalisis menggunakan uji chi-square dan cox regression. Hasil analisis penelitian menunjukkan terdapat hubungan yang bermakna antara kehamilan usia remaja dengan kelahiran prematur dengan nilai asosiasi RR adjusted 3,37 [95% CI (2,02-5,62); nilai-p 0,000)]. Diperlukan peningkatan pengetahuan mengenai kesehatan reproduksi terutama pada kelompok remaja untuk mencegah terjadinya kehamilan usia dini

Neonatal mortality is still a public health problem. The Indonesian Ministry of Health reported that 73.1% of deaths of children under 5 years in Indonesia occurred during the neonatal period. The World Health Organization (WHO) revealed that the most common cause of neonatal mortality is preterm birth and one of the risk factors for preterm birth is the mother's age, which pregnant adolescents are more at risk of it. This study is an analytical study with a retrospective cohort design that analyzes the relationship between adolescent pregnancy and preterm birth. This study was conducted at Cengkareng Regional General Hospital West Jakarta using medical record data in the period of 1st January 2022 until 31st December 2022. The population in this study is all pregnant women who were admitted to the hospital with signs of labor. The sample of this study consisted of two groups, the exposed group and the non-exposed group. The exposed group was a group of pregnant women who came with signs of labor in the adolescent age range (10-19 years), while the non-exposed group was a group of pregnant women who came with signs of labor in the reproductive age range (20-34 years). Sampling was carried out using a simple random sampling technique with the final sample count 141 respondents in each of the group. Data were analyzed using chi-square and cox regression tests. The analysis results show a significant relationship between adolescent pregnancy and preterm birth with association values RR adjusted 3,37 [95% CI (2,02-5,62); p-value 0,000)]. It is necessary to increase knowledge about reproductive health of adolescent to prevent early pregnancy.

Abstrak: The purpose of this study was to investigate the impact of income inequality and parental socioeconomic status on several birth outcomes in Japan. Data were collected on birth outcomes and parental socioeconomic status by questionnaire from Japanese parents nationwide (n = 41,499) and then linked to Gini coefficients at the prefectural level in 2001. In multilevel analysis, z scores of birth weight for gestational age decreased by 0.018 (95% confidence interval (CI): -0.029, -0.006) per 1-standard-deviation (0.018-unit) increase in the Gini coefficient, while gestational age at delivery was not associated with the Gini coefficient. For dichotomous outcomes, mothers living in prefectures with middle and high Gini coefficients were 1.24 (95% CI: 1.05, 1.47) and 1.23 (95% CI: 1.02, 1.48) times more likely, respectively, to deliver a small-for-gestational-age infant than mothers living in more egalitarian prefectures (low Gini coefficients), although preterm births were not significantly associated with income distribution. Parental educational level, but not household income, was significantly associated with the z score of birth weight for gestational age and small-for-gestational-age status. Higher income inequality at the prefectural level and parental educational level, rather than household income, were associated with intrauterine growth but not with shorter gestational age at delivery.
