The traffic accident rate is still high, there was 1,3 million people die caused roadaccident. The high number of accidents resulting in death, permanent disability,property damage is large enough to be one of the obstacles in the development ofbusiness, especially in logistics and transportationPT X is one of the concretecompany that has a problem with the high number of accidents that occurred intransportation. Based on data from the accident that occurred in PT X from 2008 untilthe year 2014, it is known that the number of accidents transportation sector is themost common accidents each year than other occupational accidents. Total accidentsthat occurred from 2008 to 2014 amounted to 159 cases, where the number ofaccidents in the transport sector amounted to 131 cases while non-transport accidentsas many as 28 cases.The Accident factors that could cause transport accidents in PT X could be classifybased on of the human factor are less skilled driver, the driver's lack of observation,the driver does not discipline and driver fatigue. The vehicle factors are consists of avehicle brake failure and lack of maintenance. The environmental factors consist ofcollapsed road, street corners, narrow roads, slippery roads, bad roads potholes, blindspot and security.

This study discusses the assessment of occupational safety risks in off-road adventure tourism activities at the Merapi Volcano Tour Tourism Object in 2022. This study aims to determine the process of several off-road adventure tourism activities involving car operators, tourists, and tourist videographers, the potential hazards that exist in each tourism activity, the potential for unwanted events, and the probability and consequences of each event, and the level of risk. This research is in the form of descriptive research using a qualitative approach. Hazard and risk identification are carried out using the Hazard Identification Risk Assessment Determining Control (HIRADC) table which has been modified as needed and risk analysis using qualitative risk analysis based on HB 205-2004 Australian Standards. The results of this study obtained 4 types of work, 9 activities, 33 hazards, 64 unexpected events, and 114 impacts with details of 35 low risk, 41 moderate risks, 37 high risks, and 1 extreme risk. From the results of this study, it is recommended that control measures be taken to reduce the level of risk and conduct socialization or education related to the existing hazards and risks. An OHS management system also needs to be implemented and developed to manage risk sustainably.
X company is one of a company that produce ready use concrete. Most of their work process are involving vehicle (truck, tank, loader, car, etc). The type of workplace accident that usually happened is traffic accident. Controls are being planned and applied, but the accident still happen. To make an appropriate controls, company needs to do the risk analysis about risk, hazard, and unexpected event in the plant. Risk analysis can be done by the use of AS/NZS 4360 : 2004 standard and assess the probability, consequences, and level of risk. Method that used is descriptive-qualitative including observation and deep interview. The result showed that the potential hazard found are 32 and risk found are 46, which is the level of risk is 16 extremely high, 15 high, 5 moderete, and 10 low. Abstrak Penulis : Gde Yulian Yogadhita Judul : Determinan Kejadian Cedera Berat pada Kecelakaan Lalu Lintas yang Ditangani oleh IGD RSUP Fatmawati Jakarta pada Periode Maret – Juli 2016 Cedera kecelakaan lalu lintas merupakan salah satu tipe cedera tak disengaja yang berkontribusi sebagai penyebab kematian terbesar ke-tiga di Indonesia, menurut laporan status keselamatan Jalan dunia yang diterbitkan WHO pada 2013 berdasarkan data Kepolisian Republik Indonesi, diestimasikan sebanyak 37 000 – 47 000 korban meninggal dunia setiap tahunnya dan 46 000 mengalami cedera berat. Pilot project surveilans cedera kementerian Kesehatan Republik Indonesia di IGD RSUP Fatmawati mencoba menggambarkan besaran magnitude cedera beserta komponen dan factor risikonya. Penelitian ini bertujuan untuk mengetahui faktor risiko terjadinya cedera berat pada kejadian kecelakaan lalu lintas yang terdokumentasikan oleh form surveilans cedera di IGD RSUP Fatmawati pada periode Maret – Juli 2016. Desain penelitian yang digunakan adalah potong lintang dengan jumlah sampel sebesar 600 cedera yang diakibatkan oleh kecelakaan lalu lintas. Hasil analisis karakteristik dan faktor risiko sebagai prediktor yang mempengaruhi terjadinya cedera berat akibat kecelakaan lalu lintas adalah jenis kelamin laki-laki dengan OR 2.03 (95% CI 1.37-3.02); usia di atas 30 tahun dengan OR 1.57 (95% CI 1.11 – 2.22); tingkat pendidikan rendah (tidak bersekolah hingga lulusan SMA) dengan OR 1.59 (95% CI 1.12-2.25); hari kejadian akhir pekan dengan OR 1.53 (95% CI 1.08 – 2.17) dan tipe pengguna jalan pesepeda dengan OR 4.84 (95% CI 0.87-29.0). Berdasar pada penelitian ini, Kementerian Kesehatan RI untuk terus melakukan advokasi penggunaan form surveilans cedera di instalasi gawat darurat di rumah sakit agar dapat memberikan gambaran yang lebih lengkap lagi mengenai karakteristik dan faktor risiko cedera dan memberi edukasi dan melakukan komunikasi risiko terkait cedera kecelakaan lalu lintas kepada masyarakat. Abstract Penulis : Gde Yulian Yogadhita Judul : Determinan Kejadian Cedera Berat pada Kecelakaan Lalu Lintas yang Ditangani oleh IGD RSUP Fatmawati Jakarta pada Periode Maret – Juli 2016 A traffic accident injury is one type of unintentional injury that contributes to the third leading cause of death in Indonesia, according to the WHO 2013 Global Road Safety Report based on Indonesian Police, estimated at 37 000 - 47 000 fatalities annually and 46 000 experience severe injuries. Injury surveillance pilot project by the Ministry of Health Republic of Indonesia in Fatmawati Hospital Emergency Departement is trying to describe magnitude of injury along with its components and risk factors. This study aims to determine the risk factors of severe road traffic injury documented by injury surveillance form in Fatmawati Hospital Emergency Departement in March to July 2016. The research design used was cross sectional with the number of samples of 600 road traffic injury cases. The results of characteristic analysis and risk factors that contributed as predictor for severe road traffic injury are male gender with OR 2.03 (95% CI 1.37-3.02); age group above 30 years with OR 1.57 (95% CI 1.11 - 2.22); low education group (not attending school until high school graduate) with OR 1.59 (95% CI 1.12-2.25); working days with OR 1.53 (95% CI 1.08 – 2.17) and cyclists with OR 4.84 (95% CI 0.87-29.0). Based on this research, the Ministry of Health of the Republic of Indonesia need to continue advocating the use of injury surveillance forms at hospital emergency department to provide complete picture of injury characteristics and risk factors, and to educate and develop road traffic injury prevention and risk communication for community.
