Goals Hospitals in providing public health services should be able to maintain quality of services. One of the government's efforts to improve the quality of home services through accreditation, which is compulsory and required by government regulation. This study focus on two goals of Prima Medika Hospital in preparation of KARS 2012 accreditation: International Patient Safety Goals and Millennium Development Goals. The purpose of this study is to determine the stages, problems and monitoring in the preparation process of accreditation on the patient's safety goals and the Millennium Development Goals. This research uses qualitative method by involving 7 participants working group of accreditation. Data collected by in-depth interview technique and document tracing, then analyzed using content analysis. The results of the interviews show that the preparation phase begins with the commitment of all hospitals, the accreditation working group established by the director, the compilation of documents of the assessment elements according to the accreditation standards, program dissemination to medical and non-medical staff, then the implementation. The next stage are monitoring and evaluation, through monev report, quality indicator and field inspection. In conclusions, the very important stages in preparation process are: commitment from all level of hospital, approaches to the specialist doctors formally or informally, more internal training. Penelitian ini bertujuan untuk menggali faktor-faktor yang mendukung dan menghambat pencapaian SKP di RS Pertamina Balikpapan, berdasarkan hasil Akreditasi Rumah Sakit tahun 2016. Desain penelitian adalah kualitatif studi kasus, dengan informan jararan direksi dan pejabat struktural dan fungsional terkait. Hasil penelitian menunjukkan faktor pendukung meliputi dukungan manajemen berupa dokumen kebijakan, panduan, pedoman. Faktor penghambat meliputi peran KKPRS (Komite Keselamatan PAsien Rumah Sakit) belum maksimal, belum ada anggaran khusus untuk program keselamatan pasien, belum ada pelatihan yang cukup untuk petugas, belum ada sistem champion dan IT (informasi Teknologi) yang memadai untuk pelaksanaan program. Rekomendasi adalah mengaktifkan peran KKPRS (Komite Keselamatan PAsien Rumah Sakit) dan penganggaran yang memadai untuk program keselamatan pasien. This study aims to explore the factors that support and inhibit the achievement of PSG (Patient Safety Goal) at Pertamina Hospital Balikpapan, based on the results of Hospital Accreditation in 2016. The study design is a qualitative case study, with informants are the directors and the managers of structural and functional related officials. The results showed supporting factors include management support in the form of policy documents, guides, guidelines. Inhibiting factors include the role of KKPRS (Hospital Patient Safety Committee) has not been maximal, there is no special budget for the patient safety program, there is not enough training for officers, there is no champion and IT (Information Technology) system adequate for program implementation. Recommendations are to enable the role of KKPRS (Hospital Patient Safety Committee) and adequate budgeting for the patient safety program. Keyword : Patient Safety, Evaluation, Qualitative
it is necessary to measure patient safety culture especially among nurses.This study aims to describe the nurses perception about patient safety culture andhow the implementation of patient safety goals at Setia Mitra Hospital. Thisdescriptive research conducted with quantitative and qualitative approaches.Population in this study is all nurses in Setia Mitra hospital that consist of 77 nurseswith sample 68 nurses.The result of study shows the nurse's perception about patient safety in SetiaMitra hospitals is 72.1% good. Dimension with the most good nurse perception ishospital handoffs and transitions dimension by 75% and at least perceived good ismanagement support dimension of 1.5%. Statistically, age (p value = 0.048), workperiod (p value = 0.016) and the level of nurses (p value = 0.049) have arelationship with the nurses perception on patient safety culture (p <0.05). In theimplementation of patient safety goals in Setia Mitra Hospital, namely 55.15% with agood understanding. It is recommended that patient safety team in the Setia MitraHospital further optimized its duties and functions so that the patient safety culture inhospitals can be improved.Keywords : Patient Safety Culture, Nurse Perception, Patient Safety Goal.
