Abstrak:
Permasalahan produktivitas tenaga kesehatan akibat manajemen institusi yang belum optimal masih sering terjadi, termasuk di Rumah Sakit Jiwa dr. H. Marzoeki Mahdi (RSJMM). Perawat sebagai tenaga kesehatan terbanyak di rumah sakit dengan tingkat intervensi pasien tinggi, bila terdapat perlakuan yang tidak tepat berisiko pada menurunnya produktivitas kerja. Tujuan penelitian ini untuk menganalisis pengaruh kompensasi finansial, kompensasi non finansial, dan disiplin kerja terhadap produktivitas perawat di RSJMM. Penelitian menggunakan pendekatan kuantitatif dengan menyebar kuesioner pada 205 perawat. Hasil penelitian menunjukkan secara parsial (1) kompensasi finanisal tidak berpengaruh signifikan terhadap produktivitas perawat (nilai sig. 0,953 > 0,05, β=0,002); (2) kompensasi non finansial berpengaruh positif dan signifikan terhadap produktivitas perawat (nilai sig. 0,016 < 0,05, β=0,073); (3) disiplin kerja berpengaruh positif dan signifikan terhadap produktivitas perawat (nilai sig. 0,000 < 0,05, β=0,293). Variabel disiplin kerja (0,293) paling dominan mempengaruhi produktivitas perawat. Secara simultan, ketiga variabel berpengaruh moderat terhadap produktivitas perawat sebesar 0,576 atau 57,6%, sedangkan 42,4% sisanya dipengaruhi oleh variabel lain. Hasil ini memiliki keterbatasan yaitu didasarkan dari persepsi responden. Disarankan RS perlu mengoptimalkan produktivitas perawat dengan meningkatkan fasilitas RS, pelatihan, kesempatan jenjang pendidikan, transparansi dan keadilan sistem kompensasi, pengakuan dan umpan balik bagi kinerja pegawai, serta kepatuhan disiplin kerja.

Problems in the productivity of health workers due to suboptimal institutional management still often occur, including at the dr. H. Marzoeki Mahdi Mental Hospital (RSJMM). Nurses as the most health workers in hospitals with a high level of patient intervention, if there is improper treatment risk decreasing work productivity. The purpose of this study was to analyze the effect of financial compensation, non-financial compensation, and work discipline on nurse productivity at RSJMM. The study used a quantitative approach by distributing questionnaires to 205 nurses. The results showed partially (1) financial compensation did not have a significant effect on nurse productivity (Sig. value 0.953 > 0.05, β=0.002); (2) Non-financial compensation has a positive and significant effect on nurse productivity (Sig. value 0.016 < 0.05, β=0.073); (3) Work discipline has a positive and significant effect on nurse productivity (Sig. value 0.000 < 0.05, β=0.293). The variable of work discipline (0.293) most dominantly affects nurse productivity. Simultaneously, the three variables had a moderate effect on nurse productivity by 0.576 or 57.6%, while the remaining 42.4% was influenced by other variables. This result has limitations that are based on respondent’s perceptions. It is suggested that hospitals need to optimize nurse productivity by improving hospital facilities, training, educational opportunities, transparency and fairness of compensation systems, recognition and feedback for employee performance, and compliance with work discipline.

Abstrak: Tenaga perawat di ICU RSU Tabanan yang memiliki dasar pelatihan perawat ICUsebagai suatu keharusan, kurang dari 50%. Profesionalisme perawat merupakanbagian intergral dari pelayanan asuhan kesehatan didasarkan pada ilmu danketerampilan keperawatan menuju pelayanan kesehatan yang bermutu.Tesis ini tentang hubungan antara sistem rekrutmen, sistem penempatan danorientasi serta pengembangan SDM terhadap kompetensi perawat melaluipendekatan kuantitatif dan kualitatif. Populasi penelitian kuantitatif adalahperawat ICU sedangkan informan untuk kualitatif adalah manajemen SDM rumahsakit umum Tabanan.Hasil penelitian menunjukkan kompetensi perawat di ICU RSU Tabanan kurangdari nilai yang diharapkan, namun ada hubungan yang bermakna antara sistemrekrutmen, sistem penempatan dan orientasi serta pengembangan SDM dengankompetensi perawat. Belum adanya standar sistem penempatan tenaga perawatsesuai kompetensinya serta sistem rekrutmen, orientasi perawat danpengembangan SDM belum berjalan sesuai standar yang telah ditetapkan.Oleh karena itu perlunya disusun standar yang baku untuk meningkatkankompetensi perawat sehingga akan menghasilkan tenaga yang berkompeten danberkinerja tinggi.
Intensive Care Nurse as health care provider in Tabanan Hospital that have basicskill as intensive care is a necessary quality, less than 50%. Lackof standardization of nurse placement system suitable for their competencies, aswell asrecruitment system,nurse orientation and human resource development notdeveloping according to a predetermined standard.Because of this reasons, we need to establish a standardized approach to improvenurse competency to produce competent human resource and better inperformance

Abstrak:
Dari berbagai pendapat dan analisis para pengelola pendidikan maupun pemakai tenaga kesehatan di pelayanan, ada simpulan dan benang merah yang sama yaitu "Mutu lulusan yang berkualitas masih menjadi harapan yang belum jadi kenyataan". Banyak faktor yang mempengaruhi mutu lulusan dan belum pernah diteliti. Dengan demikian penelitian ini bertujuan untuk mengetahui mutu lulusan Akper di Propinsi kalimantan Barat dan faktor-faktor yang berhubungan dengan mutu lulusan Akper di Propinsi Kalimantan Barat tahun 2000. Disain penelitian ini menggunakan pendekatan dua studi yaitu kuantitatif dan kualitatif. Pada studi kuantitatif menggunakan. rancangan penelitian potong lintang dengan jumlah sampel 83 orang, sementara studi kualitatif dengan menggunakan metode wawancara mendalam dengan 8 informan. Pengolahan data dengan menggunakan analisis univariat, bivariat dan multivariat, sedang untuk kualitatif dengan menggunakan analisis isi. Hasil analisis univariat menunjukkan bahwa nilai rerata mutu lulusan Akper adalah 2,57 dengan range 1,01. Sementara hasil bivariat menunjukkan adanya hubungan bermakna antara kemampuan kualitatif (r= 0,404 ; p<0,05) dan kemampuan kuantitatif (r= 0,321; p<0, 05) dengan mutu lulusan Akper. Hasil analisis mutivariat terlihat bahwa kemampuan kualitatif dan kemampuan kuantitatif merupakan faktor dominan yang berhubungan dengan mutu lulusan Akper. Saran untuk meningkatkan mutu lulusan Akper di Propinsi Kalimantan Barat agar Sipensimaru dilaksanakan sesuai ketentuan yang berlaku, serta informasi tentang penerimaan Sipensimaru Diknakes, khususnya D-III Keperawatan lebih optimal lagi untuk menjaring calon pendaftar sebanyak-banyak.

Analysis of the Quality of Nurse Academy Graduates and Factors that Correlate with the Quality of the Graduates in West Kalimantan Province in Year 2000Referring to comments and analyses obtained from the management of various nurse academies and from users of health workers in health services, it may be concluded that "Excellent graduates of nurse academies are still far from reality". There may he various factors that influence the quality of the graduates. However, these factors have not been revealed much in existing studies. Therefore, this study is aimed at investigating the quality of nurse academy graduates in West Kalimantan Province as well as investigating factors that correlate with quality of the nurse academy graduates in West Kalimantan Province in year 2000. This study employed two research approaches, namely quantitative and qualitative. In term of quantitative approach, the design of study was cross sectional with 83 people as study sample while the quantitative one employed an in-depth interview method with 8 informants. Quantitative data gathered were analyzed using univariate, bivariate and multivariate techniques while qualitative data obtained from the informants were analyzed by using content analysis technique. Results from univariate analysis show that the mean of the quality score of nurse academy graduates is 2, 57 with 1, 02 range while results from bivariate analysis show that there is a significant correlation between qualitative competence (r= 0,404; p<0, 05) and quantitative competence (r= 0,321; p<0, 05) with the quality score. The multivariate analysis revealed that both qualitative and quantitative competences are dominant factors that correlate with the quality of nurse academy graduates. It is recommended that to increase the quality of nurse academy graduates in West Kalimantan Province, the Sipensimaru (freshmen recruitment system) should be executed in compliance with the prevailing standards furthermore information regarding the opening of freshmen recruitment for health workers education (Diknakes), particularly for D-11I Nursing Program, should be disseminated widely to recruit as many enrollers as possible.

Abstrak:
Angka kematian bayi dan angka kematian ibu di Kalimantan Barat masih tinggi, karena cakupan pertolongan persalinan oleh tenaga kesehatan masih rendah. Cakupan pertolongan persalinan di Kabupaten Sanggau paling rendah dari Kabupaten dan Kota lainnya di Kalimantan Barat, kemungkinan karena kinerja bidan PTT di desa masih rendah. Tujuan penelitian ini untuk mengelahui gambaran kinerja bidan PTT di desa dan faktor-faktor yang berhubungan dengan kinerja tersebut meliputi: status perkawinan, pelatihan, lama kerja, tempat tinggal, supervisi, jumlah dukun bayi dan dukungan pemerintah desa di Kabupaten Sanggau tahun 1999. Metode penelitian dengan rancangan potong lintang dan sample seluruh populasi sebanyak 137 responden. Pengumpulan data dilakukan wawancara langsung. Analisis data dengan univariat, bivariat menggunakan chi Kuadrat dan multivariat menggunakan Regresi Logistik. Hasil penelitian kinerja bidan PTT di desa relatif rendah, dimana cakupan pertolongan persalinan S 27% dari target sebesar 50,1%. Variabel yang terbukti bermakna adalah pelatihan, lama kerja dan supervisi. Pelatihan memberikan kontribusi relatif dominan terhadap peningkatan kerja bidan PTT di desa. Disarankan adanya kebijakan pelatihan, meneruskan perpanjangan kontrak kerja dan kewenangan pengangkatan bidan PTT atau PNS oleh pemerintah daerah. Adanya program perencanaan prioritas pelatihan, penempatan dan kemandirian bidan PTT serta supervisi. Selanjutnya agar diadakan penelitian jenis latihan dan supervisi yang efektif dan efisien serta lama kerja yang dapat meningkatkan kinerja.

Factors that Correlate with Delivery Performance of On-Contract-Basis Midwives in Village in Sanggau District West Kalimantan Year 1999 Infant Mortality and Maternal Mortality rates in West Kalimantan remain high as consequence of coverage of delivery service by health workers is still small. The coverage of delivery service in Sanggau District is the lowest one compare to other districts or cities in West Kalimantan. This may be related to low delivery performance of on-contract-basis midwives of villages in the district. The purpose of this study was to investigate the performance of on-contractbasis midwives in villages and factors that correlate with the performance. The factors are : marital status, training, duration of work in village, residence, supervision, number of traditional birth attendant and support from the village office in Sanggau District in 1999. The study employed a cross sectional design and the number of selected sample from the population was 137 respondents. Data were gathered by direct interviews. Data analysis was carried out by means of univariat, bivariat tests using Chi-Square and multivariat test using Logistic Regression. The study result reveals that the delivery performance of on-contract-basis midwives in villages in the district is relatively low in which the coverage of the delivery service of S 27% of the target reached from 50.1%. It is also revealed that training, duration of work and supervision correlate significantly with the performance. Training contributes relatively dominantly to the improvement of the delivery performance of on-contract-basis midwives in the villages. This study recommends that training policies be established, work contract be extended and recruitment of on-contract-basis midwives and state-employed midwives be authorized- to the local government. This study also recommends that there should be programs that cover training priority planning, distribution and independence of on-contract-basis midwives as well as supervision. There should also be studies focusing on effective and efficient training and supervision programs as well as on duration of work thay may lead to improvement of delivery performance.

Abstrak:

Di Indonesia kebanyakan pasien kanker serviks datang pada stadium lanjut (62%) yang merupakan 66% dari penyebab kematian ginekologik. Pemeriksaan Pap Smear merupakan salah satu cara untuk mendeteksi secara dini kanker serviks sehingga penanganan kanker serviks dapat dilakukan sebelum menyebar ke luar rahim.Tujuan penelitian ini adalah untuk mengetahui peranan faktor-faktor yang menyebabkan mereka terlambat memeriksakan diri di RSUPN Dr. Cipto Mangunkusumo Jakarta. Penelitian ini memadukan metoda kuantitatif dan kualitatif. Data primer diambil dengan menggunakan kuesioner, wawancara mendalam dan membaca catatan dokumen.Hasil penelitian menunjukkan bahwa variabel-variabel yang berhubungan seeara statistik dengan keterlambatan pasien kanker serviks memeriksakan diri adalah pengetahuan, sikap, ketersediaan pelayanan Pap Smear dan dorongan suami. Biaya dan dorongan petugas kesehatan tidak berhubungan secara statistik tetapi penting khususnya penghasilan untuk membayar biaya pemeriksaan. Ketersediaan pelayanan Pap smear merupakan variabel yang dominan mempengaruhi variabel dependen.Berdasarkan wawancara mendalam dapat disimpulkan bahwa memang tidak tersedianya pelayanan Pap Smear disamping kurangnya pengetahuan yang menjadi penyebab informan terlambat memeriksakan diri dengan alasan tidak ada satupun petugas kesehatan atau orang lain yang menyampaikan informasi mengenai Pap Smear dan kanker serviks.Mempertimbangkan hasil penelitian maka disarankan kepada semua pihak yang terkait untuk meningkatkan upaya penanganan kanker serviks melalui KIE secara terkoordinir lintas sektoral kepada masyarakat umumnya, terutama kepada wanita masa reproduksi dan lansia agar memperhatikan pelayanan deteksi dini (Pap Smear).


 

An Analysis on the Delay of Cervix Cancer Patient in Examining Their selves in The National Hospital of Dr. Cipto Mangunkusumo, JakartaIn Indonesia most of the cervix cancer patients come to see doctors after advanced stadium (62 %) which 66 % ended with gynecological death. The smear test is a method to detect the cervix cancer earlier before spreading outside the uterus.The purpose of this research is to identify factors related to the delay of the health examination in The National Hospital of Dr. Cipto Mangunkusumo, Jakarta. This research combines quantitative and qualitative methods by using questionnaire, in-depth interview and examine medical records to supplement the primary data.The research result indicates variables related statistically with the cancer patient delay are; knowledge, attitudes, the availability of the Pap smears service, and the husband's support. Financial problem and encouragement from the health providers are not related statistically but it is important especially the income. The availability of Pap smear service plays as a dominant variable in affecting the dependent variable.Based on depth interview it is concluded that the unavailability of Pap smear services despite the lack of knowledge has caused the informant did not use the early detection service (Pap smear) with reason there was no health provider or other people gave information about the Pap smear and cervix cancer.Considering the research result, it is suggested that all related parties improve the handling of the cervix cancer through Communication, Information and Education (KIE), which is coordinated through cross sector way to the public, especially KIE should be focused on women during their reproductive term and the elder women so that they will pay more attention to early detection service (Pap smear).
