Sistem pembayaran rumah sakit secara prospektif merupakan salah satu cara untuk mengendalikan mutu dan biaya pelayanan kesehatan. Sistem INACBG's yang akan diterapkan secara luas di Indonesia pada era Jaminan Kesehatan Nasional merupakan salah satu bentuk sistem pembayaran tersebut. Penelitian ini bertujuan untuk mengetahui faktor-faktor yang berhubungan dengan rerata biaya klaim dan mengetahui estimasi biaya rawat inap pasien Askes di RSUD Kota Bekasi tahun 2012 apabila menggunakan tarif INA-CBG's. Sepuluh diagnosis dengan jumlah biaya klaim rawat inap tertinggi dijadikan kerangka sampel dan ditetapkan 356 kasus sebagai total sampel penelitian. Hasil penelitian : Beberapa faktor yang secara signifikan berpengaruh pada rerata biaya klaim pola Askes adalah faktor spesialisasi DPJP, jenis kelamin DPJP, umur pasien, jenis kelamin pasien dan diagnosis penyakit. Faktor yang secara signifikan berpengaruh terhadap rerata biaya klaim rawat inap pola INACBG's adalah faktor spesialisasi DPJP, jenis kelamin DPJP, umur pasien, jenis kelamin pasien, diagnosis penyakit dan kelas perawatan. Beban biaya klaim rawat inap yang menjadi kewajiban PT. Askes (Persero) berdasarkan INA-CBG's adalah 93 % lebih tinggi dibanding dengan pola tarif saat ini. Tingkat kenaikan bervariasi di antara sepuluh diagnosis yang diteliti. Saran diberikan kepada semua pihak untuk mempersiapkan penerapan sistem INA-CBG’s di Indonesia. Merupakan hal yang penting untuk menjamin kecukupan dana dan kesinambungan pelayanan kesehatan di era Jaminan Kesehatan Nasional.
The Prospective Payment System for hospitals is a system to control the quality and costs of health care. INA-CBG’s that will be widely applied in Indonesia at National Health Insurance era is one form of this payment system.
This study aims to determine the factors associated with the average cost of claims and inpatient cost estimation of PT. Askes at RSUD Kota Bekasi in 2012 under INA CBG’s scheme. The top ten diagnoses with the highest total cost of inpatient claims used as the sample frame and obtained 356 cases as the total sample of study.
RESULTS : Some factors that significantly influence to the average cost of claims under Askes scheme are the specialization of doctor in charge, sex of doctor in charge, age of the patient, sex of the patient and diagnosis of disease. The factors that significantly influence to the average cost of claims under INACBG's scheme are the specialization and sex of doctor in charge, age and sex of the patient, diagnosis of disease and classof room care. The cost of inpatient claims under INA-CBG’s scheme that become responsibilities of PT. Askes (Persero) is 93% higher than the current applied rates. The rate of increment was varied among the top ten diagnoses studied.
Suggestions are given to all parties to prepare for the implementation of INA-CBG's in Indonesia. It is essential to ensure sufficiency of fund and sustainability of care at National Health Insurance era.
Tesis ini membahas hubungan karakteristik perusahaan peserta PT. Asuransi Jiwa InHealth Indonesia dengan pemanfaatan benefit persalinan sectio caesaria produk managed care dengan menggunakan data sekunder yaitu perusahaan yang mengambil benefit persalinan sebanyak 1.441 perusahaan. Disain penelitian ini adalah cross sectional. Hasil penelitian menunjukan adanya hubungan antara karakteristik group yaitu proporsi wanita usia subur, proporsi perempuan menikah, rerata jumlah anak, jenis badan usaha dan besar kelompok dengan pemanfaatan benefit persalinan sectio caesaria, ditemukan variabel dominan yang berhubungan dengan pemanfaatan benefit persalinan sectio caesaria diantaranya kelompok besar, kelompok sedang dan proporsi wanita usia subur. This thesis discusses the relationship characteristics of participating companies PT. Asuransi Jiwa InHealth Indonesia benefit to the utilization of sectio caesaria maternity, managed care product. Using secondary data, the companies that take the benefit of maternity by the number of 1441 companies. This study design was cross sectional. The result showed relationship between the characteristics of the group: the proportion of women of childbearing age, the proportion of married women, the average number of children, types of bussiness and type of groups to the utilization of benefits sectio caesaria and found that the dominant variabel are a large group, average group, and the proportion of women are childbearing age.
