Abstrak:
Tesis ini membahas mengenai kajian implementasi metoda modifikasi Keselamatan Berbasis Perilaku (KBP) sebagai perangkat utama pada program pencegahan Kecelakaan Serius dan Kematian (KSK) atau Serious Injury and Fatality (SIF). Metoda modifikasi keselamatan berbasis perilaku diterapkan melalui pengamatan perilaku perkerja dalam mentaati aturan keselamatan bekerja pada kegiatan beresiko tinggi. Program observasi perilaku keselamatan dilakukan dengan memodifikasi beberapa bagian dari konsep Keselamatan Berbasis Perilaku atau Behavior-based Safety (BBS) yang telah dikenal saat ini. Modifikasi dilakukan pada bagian pelaku dari pengamatan perilaku, referensi perilaku yang dikehendaki, dan proses perbaikan perilaku yang diperlukan untuk membentuk perilaku selamat dari pekerja. Pengamatan perilaku menggunakan metode Modifikasi Keselamatan Berbasis Perilaku (MKBP) dilakukan di salah satu perusahaan Minyak dan Gas di Indonesia selama periode 3 tahun yang dimulai pada tahun 2017 sampai dengan 2019. Evaluasi dilakukan setiap enam bulan sekali untuk menelaah kemajuan perbaikan perilaku melalui perbaikan Index Perilaku Selamat (IPS) para pekerja dan tindak lanjut perbaikan yang perlu dilakukan setiap tahunnya. Pada akhir tahun ke tiga didapatkan bahwa program MKBP berhasil meningkatkan Index Perilaku Selamat pekerja dan berkontribusi untuk menekan angka kecelakaan serius bahkan dapat menghilangkan kejadian kecelakan yang berakibat kematian.

Modification of Behaviour Based Safety methode was used as a main tool to support in serious injury and fatality (SIF) prevention program. The modified behaviour-based safety is implemented by observing worker’s behaviour on their adherence to high risk safety protocol for working in high-risk activities. The observation program is excecuted by modifying some parts of Behavior-Based Safety (BBS) concept popularly known today. Modifications are made by modifying observer type, the reference of expected workforce’s behavior, and process of behaviour improvement. Behavioral observations by using Modified Behavior-Based Safety (MBBS) method was conducted in one of Oil and Gas companies in Indonesia over a period of 3 years started 2017 up to 2019. Index Perilaku Selamat (IPS) or Safe behaviour index was used as a parameter to monitor the progress of workforce’s improvement behaviour. The progress review was conducted once in every six months to review reliability of the method and to identify improvement opportunity in shaping work foce safe behaviour. At the end of the third year of observation, the program succesfuly improved workforce’s safe behaviour and contributed to reduction of serious injury accident as well as eliminating fatality accidents.

Abstrak: HFACS ( Human Factor Analysis and Classification System ) adalah pendekatan yang telah banyak di gunakan dalam melakukan investigasi kecelakaan untuk mengetahui kelemahan dalam suatu organisasi. HFACS ini terdiri dari empat lapisan yaitu unsafe act, precondition to unsafe act, unsafe supervision dan organizational influence. Kecelakaan dapat di cegah dengan menutup lubang pada lapisan pertahanan. Prinsip HFACS ini implementasikan untuk mengidentifikasi faktor yang menjadi kelemahan pada perilaku mengemudi yang tidak aman tahun 2020 di PT. X, sebuah perusahaan minyak di wilayah Indonesia. HFACS (Human Factor Analysis and Classification System) is an approach that has been widely used in investigating accidents to find out weaknesses in an organization. This HFACS consists of four layers, namely unsafe act, precondition to unsafe act, unsafe supervision and organizational influence. Accidents can be prevented by closing holes at each defense layer. The HFACS principle is  implemented to identify factors that contribute at unsafe driving behavior at PT. X, an oil company in Indonesia. Based on research finding, drivers who commit unsafe behavior are drivers with age 44 - 55 years (77.7%), experience over 16 years (63.1%), carried out on public roads (80.6%), on the afternoon shift (81, 6%), with a position as a team member (64.1%) and in the northern part of the company (56.3%). Weaknesses at the unsafe act level are decision errors (58.3%) ie not consistently applying risk assessment and not implementing safe work procedures. Weakness at level 2 is adverse mental state (52.4%), namely lack of focus in driving. Weakness at level 3 is inadequate supervision (65%) lack of consistent level of supervision to conduct safety supervision. Tesis ini membahas mengenai kesesuaian implementasi sistem penanggulangan tumpahan minyak PT. Z, khususnya elemen prosedur penanggulangan tumpahan minyak (OSCP), terhadap peraturan pemerintah Indonesia yaitu Peraturan Menteri Perhubungan No.58 tahun 2013 tentang penanggulangan pencemaran di perairan dan pelabuhan, SKKMigas PTK-005 tahun 2018 tentang Pengelolaan Kesehatan, Keselamatan Kerja dan Lindungan Lingkungan di Kegiatan Usaha Hulu Minyak dan Gas Bumi dan standard internasional yaitu IPIECA-IOGP tahun 2019 mengenai Oil spill preparedness and response: An introduction, IPIECA-IOGP tahun 2015 mengenai Contingency planning for oil spills on water serta kesesuaian dengan menggunakan assessment tool berdasarkan National Fire Protection Association (NFPA) 1600. Penelitian ini dilakukan dengan metode deskripti kualitatif. Berdasarkan hasil penelitian, diketahui bahwa prosedur penanggulangan tumpahan minyak PT. Z telah memenuhi ketentuan peraturan pemerintah indonesia dan standar internasional yang berlaku dimana sebagai suatu prosedur penanggulangan tumpahan minyak (OSCP). This thesis was looking for compliance for the implementation of oil spill response in PT. Z, especially for the oil spill response procedure (OSCP) element, conformity with standard of the Indonesian government regulation, Ministry of Sea Transportation  No.58, 2013 related oil spill response at waters and port, SKKMigas PTK-005, 2018 related management of health, safety and environmental protection in upstream oil and gas activities and international standard, IPIECA-IOGP, 2019 related oil spill preparedness and response: an introduction, IPIECA-IOGP 2015 related contingency planning for oil spills on water and research used assessment tool from Naational Fire Protection Association ( NFPA) 1600 edition 2013. The research was qualitative descriptive method. Based on reseach, PT. Z oil spill response procude (OSCP) was comply with Indonesian government regulation and international standar. In addition, PT. Z has emergency response team, oil spill response personnel, oil spill response equipment, and oil spill response exercise program.
