Abstrak:

Sistem informasi rumah sakit adalah salah satu sarana untuk menjalankan suatu sisem manajemen rumah sakit. Dalam sistem manajemen, sistem informasi mempunyai peran dalam perencanaan dan penetapan sasaran, baik yang bersifat strategis maupun teknis. Salah satu sistem informasi yang dikembangkan dalam penelitian ini adalah sistem informasi rekam medik yang dimanfaatkan untuk pembuatan laporan.Penelitian ini bertujuan untuk memperoleh gambaran tentang bagaimana pengembangan sistem pelaporan rumah sakit, berdasarkan data rekam medik dengan menggunakan kaidah pengembangan sistem informasi yang memenuhi syarat.Metodologi yang digunakan adalah operational research dengan pendekatan kualitatif. Penelitian dilakukan pada saat sistem masih beroperasi, yang akan mengidentifikasi masalah operasional, mengevaluasi beroperasinya sistem lama dan memberikan alternative pemecahan masalah.Dengan menerapkan metode ini dalam penelitian di rumah sakit PMI Bogor, telah didapatkan hasil penelitian bahwa subsistem yang sedang berjalan adalah subsistem manajemen, subsistem personalia, subsistem sarana & prasarana, subsistem pelayanan kesehatan rawat inap/rawat jalan/penunjang, subsistem keuangan,, subsistem rekam medik.Pada subsistem rekam medik telah diidentifikasi tiga komponen yang digunakan untuk penyusunan sistem informasi. Tiga komponen tersebut adalah basis data & informasi, prosedur, dan sarana prasarana.Dari hasil penelitian dapat diidentifikasi bahwa tiga komponen tersebut telah memenuhi syarat untuk pengembangan sistem pelaporan rumah sakit berdasarkan rekam medik. Dengan pertimbangan tiga komponen pokok tersebut maka dibuatlah suatu rancangan sistem dengan memakai pendekatan metode SDLC (system development life cycle). Dengan menerapkan metode ini diperoleh keunggulan dari sistem yang dikembangkan yaitu, mempercepat proses pembuatan laporan eksternal/internal dan ketepatan informasi, serta dapat dipakai untuk mengoreksi silang data masukan (antara laporan dari ruang dan dari berkas rekam medik).Daftar bacaan : 8 (1992-2000)

The Development of Hospital Management Reporting System Based on the Medical Record Data at General Hospital of Indonesia Red-cross, Bogor, 2002Hospital information system is one of .the means to run a hospital management system. In management system, information system has role on planning and target decision, both strategically and technically. One of information system that developed in this study is medical record information system that utilized in making report.The objective of this study is to obtain the description on how the development of hospital reporting system, based on medical record by using rule of information system that meets with the standard.The methodology used operational research; the data is collected by qualitative approach. The study is conducted when the system is still operated, to identify operational problem, to evaluate the old operational system and gives alternative problem solving.By applying this method in this study at General Hospital of Indonesia Red-cross, Bogor, it obtained that the sub-system that currently operated are management sub-system, personnel sub-system, means and infrastructure sub-system, inpatient/outpatient health services sub-system, financial system, and medical record sub-system.On the medical record sub-system, it was found three components that used to arrange the information system. Those are basic data and information, procedure, and means and infrastructure.Based on this study it can be identified that the three components have met with the development of hospital reporting system based on medical record. By considering the three main components, it is made a system design by using System Development Life Cycle (SDLC).By applying this method, it is obtained the superiority of the system that developed. Those are to speed up the process in making external/ internal reports and the accurateness of information, and it can be used to revise cross data entry (between the report from room and from medical report sheet).References: 8 (1992-2000).

Abstrak: Latar Belakang Rujukan merupakan suatu sistem penyelenggaraan pelayanan kesehatan yang melahirkan pelimpahan tanggung jawab timbal balik terhadap satu kasus penyakit atau masalah kesehatan secara vertikal atau secara horizontal, dengan tujuan untuk meningkatkan derajat kesehatan melalui pelayanan kesehatan paripurna berdasarkan pendekatan peningkatan, pencegahan , penyembuhan, dan pemulihan Rumah Sakit Umum Pusat Fatmawati sejak tahun 2000 memiliki status sebagai rumah sakit perusahaan jawatan, tetapi untuk kedepan status rumah sakit akan berubah lagi menjadi Badan Layanan Umum (BLU) . Perubahan status rumah sakit ini mengarah pada kegiatan organisasi sebagai badan usaha, sehingga rumah sakit harus mulai berubah dalam melakukan manajemennya, lebih berorientasi ke manajemen bisnis. Tujuan Tujuan penelitian ini untuk mengetahui mekanisme prosedur rujukan yang diterapkan oleh RSUP Fatmawati Jakarta mengingat telah ditetapkan sebagai pusat rujukan pelayanan Bedah Orthopaedi. Metode Penelitian ini merupakan penelitian observasional dengan menggunakan metode kualitatif dan kuantitatif, lokasi penelitian di RSUP Fatmawati Jakarta. informan adalah rujukan pasien rawat jalan, rawat inap, yang dilaksanakan pada bulan Juli 2005. Alat ukur penelitian dengan kuesioner, panduan wawancara, check list. Variabel penelitian meliputi input, proses, output. Hasil penelitian Hasil penelitian yang telah dilakukan menunjukkan bahwa kebijakan kurang dipahami, SOP kurang sosialisasi oleh RSUP Fatmawati Jakarta sebagai pemberi pelayanan, SDM dari segi kualitas & kuantitas dokter spesialis sesuai standar, perawat mahir masih kurang, sarana dan fasilitas lokasi ruang ter pencar-pencar, alat kesehatan kurang tersedia lengkap, ruang rawat inap kelas 1 tidak ada. Dalam observasi dan cek dokumen diketemukan bahwa rujukan balik ke RS pengirim belum pernah diberikan. Prosedur rujukan pasien di RSUP Fatmawati Jakarta tidak sesuai dengan pedoman yang ada. Kesimpulan dan Saran Kesimpulan, RSUP Fatmawati sebagai RS unggulan nasional sekaligus sebagai RS Pusat Rujukan pelayanan Bedah Orthopaedi, pelaksanaannya masih belum optimal, SDM Medis cukup tapi perawat masih kurang, sarana dan fasilitas belum memadai, sosialisasi kepada masyarakat kurang. Saran, RSUP Fatmawati dapat lebih meningkatkan komitmen untuk melaksanakan sistem rujukan, mengupayakan kelancaran komunikasi dengan semua jenjang rujukan. Kata kunci : Prosedur Rujukan, Rumah Sakit

Back Ground Reference is a system of health service implementation, which results a delegation of responsibility on both sides toward a disease case or health problem vertically or horizontally, with an aim to increase any health level through a complete heath service based on the approach of improvement, prevention, curing and recovery. Rumah Sakit Umum Fatmawati since year 2000 has had a status as a service company hospital, however for the next future the status will be changed into Public Service Division (BLU). The status change of this hospital refers to an organization program as a business so this hospital has to start to change its management and be more oriented toward business management. Objective The aim of this research is to find out a mechanism Reference procedure which has been applied by RSUP Fatmawati Jakarta considering it has been pointed as a Reference Center of Orthopedics Surgery Service Methods The informants are referrals of on going patients and on stay patients that were conducted in July 2005. The measuring tools of the results were questionnaire, interview guidelines and check list. The variable of the research includes input, process and output. Results The result of the finished research shows that the policy is not really understandable, the Standard Operational Procedure (SOP) is not socialized well by RSUP Fatmawati Jakarta as the giver of the service, Human Resource in qualitatively and quantitatively of medical specialists is on the proper standard, skilled nurses are lacking, media and facilitation of room location are dispersed, health equipments are still uncompleted, and there is no first class room for a stay service. In some observation and document check, there is found something that the return reference to the hospital sender has not been given. The procedure of the patient reference is not suitable with its available guidelines. Conclusions Conclusion, the service of RSUP as a National Superior Hospital and Reference Center for Orthopedics Surgery Service, is not optimum yet in applying its system. There are enough medical Human Resources but lacking of skilled nurses, media and medical facilitation and socialization to the public. Suggestion, RSUP Fatmawati could improve its commitment more to carry out the reference system, and make a serious effort for the smoothness of communication with the whole reference levels. Abstrak: A hospital is defined as a place where all people with health disorders are accommodate and being an inpatient to be care far. A hospital is also a place where every type of health and medicine expertise and experience are c ncentrated, as well as every medical instruments and appliances 0f modem health.Until now, Cimacan Hospital (CH) is still on aking a great effort to process the official approval from MOH of Inadonesia. as the-Letter of AnnoW1cement of Bupati Cianjur Number 42 of Year 2001 t at the claSsification of CH is not decided yet.The study is also produce a prototype on the Information System for Registration Installation with Medical Record-based that need a continuity on strong commitment from the director of the hospital and the entire medical record staffs, to be implemented and a persistence input for improvements.
