Abstrak: Penelitian ini menggunakan disain case control untuk melihat besar risiko anak yang tidak diimunisasi BCG untuk terkena TB Paru. Kasus dan kontrol adalah anak kota Sukabumi usia 0-5 tahun yang diperoleh dari laporan rutin program TB, imunisasi, KIA dan Gizi di dinas kesehatan kota Sukabumi. Hasil analisis bivariat menunjukkan bahwa anak yang tidak diimunisasi BCG memiliki risiko 3,270 kali lebih besar untuk terkena TB paru dibandingkan anak yang diimunisasi BCG. Sedangkan hasil analisis multivariat yang memasukkan variabel kunjungan neonatal sebagai variabel interaksi dan variabel berat badan lahir sebagai confounding menunjukkan bahwa anak yang tidak diimunisasi BCG dan kunjungan neonatal < 3 kali memiliki tingkat Risiko sebesar 5,63 kali lebih tinggi untuk terkena TB paru dibandingkan anak yang diimunisasi BCG dan kunjungan neonatal =3 kali (kelompok referens). Adapun tingkat risiko anak yang tidak diimunisasi BCG dan kunjungan neonatal = 3 kali adalah sebesar 1,13 kali lebih besar untuk terkena TB paru dibandingkan kelompok referens dan anak yang diimunisasi BCG namun kunjungan neonatal<3 memiliki tingkat risiko 4,99 kali lebih besar dari kelompok referens. Dari hasil tersebut diketahui bahwa efikasi vaksin BCG tanpa interaksi adalah dengan 67% sedangkan efikasi vaksin BCG dengan interaksi adalah 82%. ini berarti, efikasi vaksin di Kota Sukabumi mengalami peningkatan dengan adanya interaksi variabel kunjungan neonatal dengan imunisasi BCG. Kata kunci: Imunisasi BCG, TB Paru anak, Tuberculosis This study used case control design to look at the risk of children not immunized with BCG for pulmonary tuberculosis. Case and control are children under 5 years old in Sukabumi City obtained from regular reports of TB program, immunization, KIA and Nutrition at health office in Sukabumi city. The result of bivariate analysis showed that children who were not immunized with BCG had a risk at 3.270 times higher to have pulmonary TB than children were immunized with BCG. While the results of multivariate analysis that included neonatal visit variables as interaction variables and born weight variables as confounding showed that children were not immunized with BCG and neonatal visits < 3 times had a Risk level at 5.63 times higher for pulmonary tuberculosis than immunized children BCG and neonatal visits = 3 times (reference group). The risk of children not immunized by BCG and neonatal visits = 3 times was 1.13 times higher for pulmonary tuberculosis than the referent group and children immunized with BCG but neonatal visits <3 had a risk level 4.99 times greater than the group Referens. From these results it is known that the efficacy of BCG vaccine without interaction is 67 % while the efficacy of BCG vaccine with interaction is 82 %. Abstrak:

Difteri masih menjadi masalah kesehatan masyarakat di Kabupaten Garut terdapat 100 kasus difteri dengan CFR sebesar 17,2%. Penelitian ini bertujuan mengetahui hubungan status imunisasi dan riwayat kontak dengan kejadian difteri di Kabupaten Garut tahun 2023–2024. Penelitian ini menggunakan pendekatan mix-method. Penelitian kuantitatif menggunakan desain case control (1:2) dengan matching individu berdasarkan usia dan tempat tinggal. Jumlah responden sebanyak 141 anak (47 kasus dan 94 kontrol) yang dipilih menggunakan teknik total sampling dari data KLB. Analisis kondisional regresi logistik dilakukan untuk menghitung odds ratio (OR) dan 95% Confidence Interval (CI). Pendekatan kualitatif dilakukan melalui wawancara mendalam dengan informan terkait menggunakan pendekatan fenomenologi. Sebagian besar anak pada kelompok kasus tidak diimunisasi (74,47%) dan memiliki riwayat kontak dengan penderita difteri (76,6%). anak yang tidak diimunisasi dan memiliki riwayat kontak berisiko 9 kali untuk terkena difteri dibandingkan dengan yang imunisasi lengkap (95%CI: 2,67-31,79). Hasil analisis menunjukkan terdapat hubungan signifikan antara status imunisasi dan kejadian difteri (OR=5,82; 95% CI: 2,17–15,61; p=0,0001) sebelum mengontrol kovariat. Setelah mengontrol kovariat, terdapat hubungan antara status imunisasi dan kejadian difteri (OR=3,08; 95% CI: 1,40–23,79; p=0,008). Hasil kualitatif menunjukkan bahwa faktor sosial-budaya, agama, penolakan imunisasi, serta sistem surveilans yang belum optimal sebelum KLB turut memengaruhi kejadian. Diperlukan peningkatan cakupan imunisasi, edukasi masyarakat, serta penguatan sistem surveilans untuk pencegahan difteri di wilayah berisiko tinggi.

Background: Diphtheria remains a public health problem in Garut District, with 100 reported cases and a case fatality rate (CFR) of 17.2%. This study aimed to determine the relationship between immunization status with the incidence of diphtheria in Garut District in 2023–2024. Methods: A mixed-method approach was used. The quantitative component employed a case-control design (1:2) with individual matching based on age and residence. A total of 141 children (47 cases and 94 controls) were selected using total sampling from outbreak data. Conditional logistic regression analysis was conducted to calculate odds ratios (OR) and 95% confidence intervals (CI). The qualitative approach was carried out through in-depth interviews with key informants using a phenomenological method. Result: Most children in the case group were not immunized (74.47%) and had a history of contact with diphtheria patients (76.6%). Children who were not immunized and had contact history were 9 times more likely to contract diphtheria compared to those who were fully immunized (95% CI: 2.67–31.79). A significant association was found between immunization status and diphtheria incidence (OR=5.82; 95% CI: 2.17–15.61; p=0.0001), and remained significant after controlling for covariates (OR=3.08; 95% CI: 1.40–23.79; p=0.008). The qualitative findings revealed that socio-cultural and religious factors, vaccine refusal, and a suboptimal surveillance system prior to the outbreak contributed to the cases. Conclusion: Strengthening immunization coverage, community education, and surveillance systems is needed to prevent diphtheria in high-risk areas.

Abstrak:
Infeksi Saluran Pernapasan Akut (ISPA) masih menjadi salah satu penyebab utama morbiditas pada anak balita, terutama di negara berkembang. Status imunisasi dasar dan riwayat berat lahir rendah (BBLR) merupakan faktor yang secara teoritis berhubungan dengan kejadian ISPA pada anak, namun belum adanya penelitian yang secara khusus mengevaluasi efek gabungan antara status imunisasi dasar dan riwayat berat lahir terhadap kejadian ISPA pada balita. Sehingga penelitian ini bertujuan untuk menganalisis hubungan antara kedua faktor tersebut dengan kejadian ISPA pada anak usia 12–59 bulan di DKI Jakarta baik secara terpisah (independent), maupun kombinasi (gabungan) berdasarkan data Survei Kesehatan Indonesia tahun 2023 dengan menggunakan desain cross-sectional. Sampel penelitian berjumlah 711 anak usia 12–59 bulan di DKI Jakarta yang memenuhi kriteria inklusi, yaitu memiliki data lengkap pada setiap variabel yang diteliti. Variabel dependen adalah kejadian ISPA, yaitu anak yang tercatat dalam 1 bulan terakhir didiagnosis ISPA oleh tenaga kesehatan dan atau memiliki gejala demam disertai batuk/pilek/nyeri tenggorok, sedangkan variabel independen utama meliputi status imunisasi dasar, riwayat berat lahir, dan variabel gabungan status imunisasi dasar dan riwayat berat lahir. Variabel kovariat terdiri atas usia anak, status gizi, ASI eksklusif, paparan asap rokok dalam rumah, usia ibu, pendidikan ibu, status pekerjaan ibu, dan status ekonomi. Analisis dilakukan secara univariat, bivariat dan multivariat menggunakan regresi generalized linear model dengan pendekatan backward elimination. Hasil penelitian menunjukkan prevalensi ISPA pada anak usia 12–59 bulan sebesar 37,6%. Setelah mengotrol faktor perancu, anak dengan imunisasi dasar tidak lengkap memiliki prevalensi ISPA 13% lebih tinggi dibandingkan anak dengan imunisasi lengkap (aPR=1,13; 95%CI: 0,93–1,39), namun tidak bermakna secara statistik. Anak dengan riwayat BBLR juga memiliki prevalensi ISPA 33% lebih tinggi dibandingkan dengan anak yang tidak BBLR (aPR=1,33; 95%CI: 0,95–1,88). Pada analisis variabel gabungan, kelompok anak dengan imunisasi lengkap dan riwayat BBLR memiliki prevalensi ISPA tertinggi (aPR=1,62; 95%CI: 0,93–2,83) dibandingkan degan kelompok imunisasi lengkap dan tidak BBLR. Penelitian ini menyimpulkan bahwa status imunisasi dasar dan riwayat BBLR menunjukkan kecenderungan peningkatan risiko ISPA meskipun tidak bermakna secara statistik. Upaya pencegahan ISPA tetap perlu difokuskan pada peningkatan cakupan imunisasi dasar dan pemantauan kelompok anak dengan riwayat BBLR.

Acute Respiratory Infection (ARI) remains one of the leading causes of morbidity among children under five, particularly in developing countries. Basic immunization status and a history of low birth weight (LBW) are theoretically associated with the incidence of ARI in children; however, limited research has specifically evaluated the combined effect of these two factors on ARI among under-five children. Therefore, this study aimed to analyze the association between basic immunization status and birth weight history with the incidence of ARI among children aged 12–59 months in DKI Jakarta using data from the 2023 Indonesian Health Survey through a cross-sectional design. The study sample consisted of 711 children aged 12–59 months in DKI Jakarta who met the inclusion criteria and had complete data for all study variables. The dependent variable was the incidence of ARI, defined as a child who, within the previous month, had been diagnosed with ARI by a healthcare professional and/or experienced symptoms of fever accompanied by cough, runny nose, or sore throat. The main independent variables included basic immunization status, birth weight history, and a combined variable of basic immunization status and birth weight history. Covariates consisted of child age, nutritional status, exclusive breastfeeding, exposure to cigarette smoke inside the house, maternal age, maternal education, maternal employment status, and socioeconomic status. Data were analyzed using univariate, bivariate, stratified, and multivariate analyses with generalized linear model regression and a backward elimination approach. The results showed that the prevalence of ARI among children aged 12–59 months was 37.6%. After adjusting for potential confounders, children with incomplete basic immunization had a 13% higher prevalence of ARI compared with fully immunized children (aPR = 1.13; 95% CI: 0.93–1.39), although the association was not statistically significant. Children with a history of LBW had a 33% higher prevalence of ARI compared with those without LBW (aPR = 1.33; 95% CI: 0.95–1.88). In the combined-variable analysis, children with complete immunization and a history of LBW had the highest prevalence of ARI (aPR = 1.62; 95% CI: 0.93–2.83) compared with children who had complete immunization and no history of LBW. This study concludes that incomplete basic immunization status and a history of LBW tend to increase the risk of ARI, although the associations were not statistically significant. Efforts to prevent ARI should continue to focus on improving basic immunization coverage and monitoring children with a history of LBW.
