Abstrak:
Porsi terbesar kecelakaan kerja di pengeboran, workover, dan wellservice berdasarkan bagian tubuh terjadi pada jari dan tangan. Penelitian ini dilakukan untuk mengetahui karakteristik pekerjaan, faktor manusia, pekerjaan, dan organisasi yang berkontribusi pada kejadian cedera tangan. Penelitian ini mempergunakan disain studi kuantitatif dengan pendekatan cross-sectional. Hasil penelitian menunjukkan bahwa karakteristik pekerjaan yang terdapat kasus cedera tangan memberikan porsi yang hampir sama namun dengan konsekuensi cedera yang berbeda. Faktor manusia yang berkontribusi pada kejadian cedera tangan yaitu: mistake/mental slip, prosedur tidak dilakukan, analisa bahaya tidak dipergunakan, dan penggunaan alat yang tidak benar. Faktor pekerjaan yang berkontribusi yaitu: desain tidak memadai dan tidak tersedia peralatan standar. Faktor organisasi yang berkontribusi yaitu: analisa bahaya tidak memadai/bahaya tidak teridentifikasi, prosedur tidak memadai, prosedur tidak ada, tidak ada analisa resiko, tidak dilatih, dan arahan kerja tidak memadai. Sindrom metabolik menurut American Heart Association (AHA) 2021 merupakan kumpulan dari 5 faktor risiko yang dapat memicu terjadinya serangan jantung, diabetes, stroke dan penyakit kardiovaskular lainnya. 5 faktor risiko itu adalah (1) Meningkatnya kadar trigliserida, (2) lingkar perut semakin besar, (3) meningkatnya gula dalam darah (4) menurunnya level high density lipoprotein (HDL), dan (5) naiknya tekanan darah. Berdasarkan hasil Medical Check Up (MCU) 2021 semua pekerja di PT X diketahui prevalensi kejadian sindrom metabolik naik menjadi 22,4% dibandingkan tahun sebelumnya (16,2%). Tujuan dari penelitian ini untuk mengetahui gambaran sindrom metabolik pada pekerja lapangan lepas pantai di PT X tahun 2021 dengan metode cross sectional. Dari hasil penelitian diketahui bahwa variabel riwayat keluarga, work shift, lama bekerja, aktivitas fisik, perilaku sedentari, durasi tidur dan merokok tidak berhubungan dengan kejadian sindrom metabolik (p-value > 0,05). Variabel usia, dan IMT memiliki hubungan dengan kejadian sindrom metabolik (p-value < 0,05). Tidak ada perbedaan antara pola makan responden yang memiliki riwayat sindrom metabolik dengan pola makan responden yang tidak memiliki riwayat sindrom metabolik. Oleh karena itu perlu dilakukan promosi kesehatan pekerja yang baik untuk meningkatkan kesadaran pekerja akan kesehatan.
Metabolic syndrome according to the American Heart Association (AHA) 2021 is a collection of 5 risk factors that can trigger heart attacks, diabetes, stroke and other cardiovascular diseases. The 5 risk factors are (1) increased triglyceride levels, (2) greater abdominal circumference, (3) increased blood sugar (4) decreased high density lipoprotein (HDL) levels, and (5) increased blood pressure. Based on the results of the 2021 Medical Check Up (MCU), all workers at PT X found that the prevalence of metabolic syndrome had increased to 22.4% compared to the previous year (16.2%). The purpose of this study was to describe the metabolic syndrome in offshore field workers at PT X in 2021 using the cross sectional method. From the research results it is known that the variables of family history, work shift, length of work, physical activity, sedentary behavior, sleep duration and smoking are not associated with the incidence of metabolic syndrome (p-value > 0.05). Age and BMI variables were associated with the incidence of metabolic syndrome (p-value <0.05).There was no difference between the eating patterns of respondents who had a history of metabolic syndrome and the eating patterns of respondents who did not have a history of metabolic syndrome. Kecelakaan besar dalam industri minyak dan gas bumi meskipun relatif jarang terjadi namun sering bersifat katastropik, yang menyebabkan kematian pada pekerja dalam jumlah besar, kerusakan aset perusahaan yang bernilai tinggi dan pencemaran lingkungan. Meskipun penyebabkan utama kecelakaan sering disebabkan oleh faktor manusia, namun kegagalan manajemen tanggap darurat dalam menangani kecelakaan, memberikan kontribusi besar yang menyebabkan kecelakaan lebih parah dan kerugian semakin besar. Kesiapan manajemen tanggap darurat pada operasi hulu minyak dan gas mutlak diperlukan dalam upaya mempersiapkan penanganan setiap kecelakaan dan kondisi darurat. Dalam upaya untuk terus menjaga tingkat kesiapan dan efektifitas manajemen tanggap darurat secara regular perlu dilakukan proses evaluasi.
Dalam penelitian ini penulis melakukan penelitian dengan melakukan evaluasi sistem sistem manajemen tanggap darurat di perusahaan hulu minyak dan gas yang beroperasi di laut dalam, dengan ketentuan pada National Fire Protection Association (NFPA) 1600 edisi 2013.

Cedera jari dan tangan sangatlah umum terjadi pada Divisi Maintenance. Hal inidikarenakan tipe pekerjaan pada Divisi Maintenance adalah pemasangan danpelepasan komponen yang bervariasi tahapannya, pekerjaan dilakukan saatberjalannya proses, menggunakan mesin yang rumit dan bekerja dengan tekananwaktu. Tujuan dari penelitian ini adalah untuk menjelaskan gambaran kecelakaankerja dan analisis penyebab kecelakaan kerja pada insiden cedera jari dan tangandi Divisi Maintenance PT X. Penelitian ini menggunakan desain penelitiancampuran (mixed methods research) yang diawali dengan pengumpulan 102 datakecelakaan kemudian wawancara mendalam pada 6 orang yang mengalamicedera jari dan tangan, pengawas dan manajemen. Penyebab langsung palingbanyak adalah tidak mengetahui adanya potensi bahaya, mengambil keputusanyang tidak tepat dan penggunaan perkakas secara tidak tepat. Penyebab dasarpaling banyak adalah pengembangan PSP yang tidak memadai, perilaku amanyang penting tidak cukup diidentifikasi. Pekerja masih kurang memiliki kesadaranterkait kesehatan dan keselamatan kerja. Peran supervisor dalam mencegahkecelakaan adalah dengan mengingatkan pekerja terkait aspek keselamatan.Manajemen berperan dalam membuat dan mengembangkan prosedur kerja dalammencegah kecelakaan.

Analisis Faktor Manusia pada Kasus Cedera Tangan dan Jari dengan Menggunakan Kerangka HFACS di PT. X

Abstrak:
Pendahuluan: Di dalam industri hulu migas, cedera tangan dan jari merupakan tantangan besar. Setidaknya 50% dari kasus cedera di dalam industri migas merupakan cedera tangan dan jari. Di beberapa perusahaan, proporsi tersebut dapat menjadi lebih besar. Dalam analisis yang pernah dilakukan terhadap kecelakaan di perusahaan anggota IOGP, lebih dari delapan puluh persen diakibatkan karena factor manusia, baik yang disebabkan oleh faktor pribadi ataupun faktor organisasi. Studi ini dilakukan untuk menganalisis factor manusia di dalam kasus cedera tangan dan jari yang teradi di PT. X sepanjang tahun 2014 hingga 2020 dengan menggunakan kerangka HFACS. Metode: Penelitian dengan metode deskriptif kualitatif dengan menggunakan data sekunder berupa data kasus cedera tangan dan jari di PT. X dari tahun 2014 hingga 2020. Hasil: Cedera tangan dan jari paling banyak disebabkan oleh skill-based errors dan routine violations. Kasus cedera yang diakibatkan skilled-based errors juga diperparah oleh pelanggaran aturan yang dilakukan bersama secara terus menerus (routine violations). Kondisi yang menjadi prekondisi dari tindakan tidak aman yang berkontribusi dalam cedera tangan dan jari paling banyak terkait crew/ resource management. berbagai faktor yang termasuk ke dalam kategori ini di antara lain komunikasi, koordinasi, perencanaan dan kerja tim yang mempengaruhi kinerja. Pengawasan yang tidak aman yang paling banyak terjadi adalah failed to correct known problems dan inadequate supervision. Inadequate supervision terkait dengan pengelolaan personil dan sumber daya termasuk pelatihan, panduan professional dan kepemimpinan operasional. Sedangkan failed to correct known problems terkait dengan kekurangan pada individu, peralatan, pelatihan atau area keselamatan lain “diketahui” oleh supervisor, namun dibiarkan tidak dikoreksi. Di level 4, pengaruh organisasi yang terbesar adalah Organisational Process. Organisational process adalah proses formal di mana visi sebuah organisasi dijalankan termasuk operasi, prosedur, dan kesalahan di antaranya. Kesimpulan: Gambaran HFACS pada kasus cedera tangan dan jari di PT. X sejalan dengan gambaran umum HFACS yang ada dalam industri hulu migas, kecuali di level 2 yakni preconditions for unsafe acts. Sistem pembelajaran kejadian di PT. X masih dipengaruhi oleh teori domino dan belum mengintegrasikan konsep faktor manusia secara menyeluruh. Kerangka HFACS dapat membantu PT. X dalam menelaah lebih dalam defisiensi di dalam faktor manusia untuk dapat menetapkan tindakan perbaikan yang lebih tepat.
Introduction: Hand and finger injuries have always been major challenges in upstream oil and gas industry. At least 50% of injuries in upstream oil and gas impacting hand and fingers. In some companies, the proportion could be larger. More than 80% of incidents in IOGP members were caused by human factors, both personal and organizational factors. This study aims to analyze human factors in hand and finger injuries at PT. X by using HFACS framework. Methodology: The study was conducted by applying qualitative descriptive analysis by using secondary data, investigation report of hand and finger injures from 2014 to 2020. Results: Skill-based errors and routine violations contributed in most of hand and finger injuries in PT.X. Routine violations were found as aggravating factors in skill-based errors injuries/ crew resource management were dominating level 2, preconditions for unsafe acts, it consists of coordination, communication, planning and team work that impacting performance. Unsafe supervision that occurred the most are inadequate supervision and failed to correct known problems. Inadequate supervision related to personnel and resources management including trainings, professional guidance and operational leadership. Failed to correct known problems related to deficiencies in individual, equipment, training or the safety area “known” to supervisor but left uncorrected. In level 4, Organizational process was the weak chain of organizational influences. Organizational process is a formal process where organization’s vision is implemented on Site, including operations, procedures. Conclusion: HFACS of hand and finger injuries in PT. X is in line with general HFACS description in upstream oil and gas industry, except for level 2, pre-conditions for unsafe acts. Learning from incident system in PT. X was still highly influenced by domino theory and has not yet integrated human factors. HFACS framework can help PT. X to dig deeper in human factors deficiencies in organization so PT. X can define more effective mitigation & preventive measures.

Sektor konstruksi yang memiliki proses dan tahapan kerja yang kompleks membuat tempat kerjanya berisiko tinggi mengalami berbagai jenis kecelakaan kerja, tidak terkecuali cedera tangan dan jari. Cedera tangan dan jari, mulai dari luka sayat hingga cacat permanen pada tangan dan jari dapat mempengaruhi produktivitas bahkan dapat mengakhiri karir seseorang. PT X merupakan perusahaan sektor konstruksi dengan persentase cedera tangan dan jari yang mendominasi selama beberapa tahun terakhir. Penelitian ini bertujuan untuk memberikan gambaran risiko terkait cedera tangan pada sektor konstruksi, lebih tepatnya pada proses pemasangan dan pembongkaran aluminium formwork di Jobsite Y PT X. Penelitian menggunakan desain studi deskriptif dengan metode campuran, yaitu metode kualitatif dengan observasi lapangan dan konsultasi, serta metode semi kuantitatif untuk analisis risiko. Analisis risiko dilakukan secara mendalam mulai dari mengidentifikasi bahaya mekanik dan kejadian tak terduga, menganalisis tingkat kemungkinan, pajanan, dan konsekuensi, serta menghitung penurunan risiko dari upaya pengendalian yang telah dilakukan oleh PT X. Hasil penelitian menunjukkan bahwa tingkat risiko tertinggi terkait cedera tangan dan jari memiliki dampak tangan atau jari terputus, serta terendah dengan dampak cedera ringan akibat terkilir. Terdapat rata-rata penurunan risiko sebesar 37,53% dari upaya pengendalian yang telah dilakukan Jobsite Y PT X terkait cedera tangan dan jari.
The construction sector, which has complex work processes and stages, puts workplaces at high risk of experiencing various types of work accidents, including hand and finger injuries. Hand and finger injuries, ranging from cuts to permanent defects in the hands and fingers, can affect productivity and can even end a person's career. PT X is a construction sector company with a dominant percentage of hand and finger injuries over the last few years. This research aims to provide an overview of the risks related to hand injuries in the construction sector, more precisely in the process of installing and dismantling aluminum formwork at Jobsite Y PT X. The research uses a descriptive study design with mixed methods, namely qualitative methods with field observations and consultations, as well as semi-quantitative methods for risk analysis. Risk analysis is carried out in depth starting from identifying mechanical hazards and unexpected events, analyzing the level of probability, exposure and consequences, as well as calculating risk reduction from control efforts that have been carried out by PT X. The results of the study showed that the highest level of risk related to hand and finger injuries was due to a severed hand or finger, and the lowest was due to minor injuries due to sprains. There is an average risk reduction of 37.53% from the control efforts that have been carried out by Jobsite Y PT X regarding hand and finger injuries.

Abstrak:
Pendahuluan: Hidrokarbon merupakan bahan yang mudah terbakar yang dapat menyebabkan kecelakaan besar kecelakaan dan ledakan di instalasi anjungan lepas pantai pengolahan hidrokarbon. Kebakaran dan ledakan di anjungan lepas pantai adalah kecelakaan yang relatif jarang terjadi tetapi dapat memiliki konsekuensi tak terduga yang berdampak signifikan terhadap kematian dan kehilangan aset. Metode: Metode deskriptif dengan desain kuantitatif dari data sekunder tahun 2020 (cross sectional) dan studi literatur tanpa melakukan intervensi pada objek penelitian (non-experimental) dianalisis menggunakan perangkat lunak (PHAST) untuk mengevaluasi model konsekuensi kebakaran dan ledakan. Analisis frekuensi dengan metode fault tree dan event tree analysis, untuk menganalisa kemungkinan terjadinya overpressure dan terjadi nya kecelakaan besar pada fasilitas pengolahan hidrokarbon gas dan minyak di anjungan lepas pantai yang merupakan Major Hazard Plant. Hasil: Tingkat risiko tertinggi untuk kematian personel yang bekerja di anjungan lepas pantai tersebut berada pada tingkat ALARP Region dari kontributor skenario terbesar flash fire dengan jumlah fatality sebanyak 10 orang dan nilai frekuensi 3,26E-08/year artinya 1 dari 30.674.847 peluang skenario flash fire dalam 1 tahun dapat terjadi hingga menyebabkan kematian 10 orang, sedangkan risiko terhadap aset berada pada tingkat risiko yang dapat diterima/Acceptable dari kontributor skenario terbesar jet fire dengan nilai kehilangan aset sebesar 40.590.800,00 dan nilai frekuensi tertinggi 6,31E-08/year artinya 1 dari 15.847.861 peluang skenario jet fire dalam 1 tahun dapat terjadi hingga menyebabkan kehilangan aset sebesar $ 40.590.800 dari kebakaran dan ledakan skenario overpressure yang berpotensi terjadi di anjungan baru lepas pantai dengan mempertimbangkan beberapa sistem pengaman yang telah ditentukan dalam desain. Kesimpulan: Tidak diperlukan adanya tambahan mitigasi dikarenakan sistem pengaman yang telah ditentukan dalam desain cukup untuk mencegah kecelakaan besar yang dapat terjadi sehingga anjungan baru lepas pantai dinyatakan aman untuk dioperasikan

Fires and explosions on offshore platforms are relatively rare accidents but can have unforeseen consequences that can have a significant impact on fatality and loss of assets. Methods: Descriptive method with quantitative design from secondary data in 2020 (cross sectional) and literature study without intervention on the research object (non-experimental) using software (PHAST) to evaluate the consequences of fire and explosion models. Frequency analysis with fault tree and event tree analysis methods, to analyse the possibility of overpressure and major accidents events on offshore platforms hydrocarbon processing facilities which are Major Hazard Plants. Abstrak:
Pekerja di lapangan migas, khususnya di lepas pantai memiliki risiko yangtinggi terhadap pajanan BTX di area kerja. Pajanan bersumber dari aktifitas yanglangsung bersentuhan dengan uap dan gas hidrokarbon yang sifatnya mudahmenguap pada suhu kamar (Volatile organic compounds - VOC) sehinggamemungkinkan terhisap oleh para pekerja dan menimbulkan efek kesehatan.Penelitian ini bertujuan untuk memperkirakan tingkat risiko nonkarsinogenik dankarsinogenik dari Pajanan BTX terhadap pekerja lepas pantai beserta manajemenrisiko yang harus dilakukan. Penelitian ini merupakan studi potong lintangmenggunakan pendekatan analisis risiko kesehatan lingkungan (ARKL) yangmeliputi 4 langkah penting: identifikasi bahaya, analisis dosis-respon, analisispajanan dan karakterisasi risiko. Jumlah sampel berupa 95 orang pekerja tetap diperusahaan hulu migas X. Data penelitian diperoleh melalui wawancara danpengukuran langsung, tingkat risiko dihitung dengan cara membagi asupandengan dosis referensi BTX. Sebagai pembanding (control) dilakukan jugaperhitungan terhadap 7 orang pekerja lepas pantai yang bekerja hanya di kantor(office). Hasil penelitian menunjukkan risiko pajanan benzene non karsinogenikharus diwaspadai bagi pekerja lepas pantai dimana dari perhitungan diketahuinilai RQ (Risk Quotient) yang lebih dari satu baik untuk pajanan realtime (ada21,05% pekerja) maupun pajanan lifetime (61,05% pekerja). Sementara untukrisiko pajanan non karsinogenik dari toluene dan xylene termasuk rendah. Iniditunjukkan dari hasil perhitungan RQ untuk realtime maupun lifetime yangsemuanya (100%) bernilai kurang dari satu (RQ <1). Untuk risiko kesehatanpajanan karsinogenik benzene, diperoleh bahwa 20% pekerja lepas pantaimemiliki efek karsinogenik pada pajanan realtime dan 60% pekerja pada pajananlifetime. Disimpulkan bahwa perlu dilakukan manajemen risiko terhadap pajanansenyawa benzene di lingkungan kerja lepas pantai, agar pekerja terhindar daririsiko kesehatan baik risiko nonkarsinogenik dan risiko karsinogenik jangkapanjang.

Tesis ini membahas tentang potensi dan risiko kebakaran pada pekerja yang bekerja di anjungan minyak lepas pantai atau disebut dengan platform yang disebabkan oleh bahaya hidrokarbon dengan cara mengidentifikasi bahaya tersebut. Melakukan kalkulasi bahaya untuk mengetahui lokasi spesifik area yang mempunyai risiko bahaya, kemudian melakukukan penghintungan individual risk per annum (IRPA) dilanjutkan dengan hasil final dari potential loss of live (PLL). Hasil tersebut dibandingkan dengan kriteria atau standar dan menentukan posisi pekerja dalam kondisi ALARP atau tidak. Penelitian ini adalah penelitian kuantitatif dengan desain deskriptif. Hasil penelitian menyarankan perusahaan mempertahankan program dan kegiatan yang selalu mencegah terjadinya kebakaran, walaupun anjungan CB masuk kedalam acceptable region. Psychological safety yang rendah telah berkontribusi terhadap kecelakaan – kecelakaan besar yang terjadi di industri minyak dan gas bumi. PT. X yang merupakan salah satu industri minyak dan gas bumi di Indonesia merujuk pada hasil Health & Safety Engagement Survey dan analisis cidera akibat kerja menunjukkan bahwa profil psychological safety pada pekerja fasilitas produksi minyak dan gas bumi lepas pantai di PT. X perlu ditingkatkan. Oleh karena itu, peneliitian ini bertujuan untuk menganalisis profil psychological safety dan faktor – faktor yang berhubungan dengan profil psychological safety guna dapat memberikan rekomendasi yang tepat dalam rangka peningkatan profil psychological safety pada pekerja fasilitas produksi minyak dan gas bumi lepas pantai di PT. X. Penelitian ini dilakukan pada periode Maret – Juni 2024 di 4 (empat) lokasi menggunakan metode penelitian campuran, yaitu metode kuantitatif dengan desain cross-sectional dan metode kualitatif dengan desain studi kasus. Sampel untuk metode kuantitatif berjumlah 255 responden dan sampel untuk metode kualitatif berjumlah 8 (delapan) informan. Hasil penelitian menunjukkan bahwa sebagian besar responden (65,5%) memiliki profil psychological safety rendah yang berarti sebagian besar responden merasa takut untuk melakukan hal – hal yang berkaitan dengan HSE. Kemudian, dengan menggunakan uji chi square didapatkan bahwa faktor – faktor yang berhubungan dengan profil psychological safety pada pekerja fasilitas produksi minyak dan gas bumi lepas pantai di PT. X, yaitu kompetensi keselamatan (OR 3,37, 95% CI 1,96 – 5,78), kepercayaan (OR 5,35, 95% CI 3,06 – 9,36), tekanan rekan kerja (OR 5,27, 95% CI 3,00 – 9,26), hubungan interpersonal (OR 6,05, 95% CI 3,40 – 10,75), tekanan pekerjaan (OR 4,88, 95% CI 2,80 – 8,50), kecerdasan emosional manajemen lapangan (OR 7,43, 95% CI 3,97 – 13,89), kebijakan, poses dan prosedur (OR 6,94, 95% CI 3,91 – 12,33), penekanan hirarki atasan bawahan (OR 1,88, 95% CI 1,07 – 3,29). Selain itu, faktor lain yang berhubungan dengan profil psychological safety yang terungkap berdasarkan metode kualitatif melalui wawancara semi terstruktur yaitu proses investigasi.
Low psychological safety has contributed to major accidents occurred in oil and gas industry. PT. X is an oil and gas industry in Indonesia which referred to the result of Health & Safety Engagement Survey and analysis of occupational injuries showed that psychological safety profile of offshore oil and gas production facility’s workers in PT. X need to be improved. Therefore, this research aimed to analyze the psychological safety profile and associated factors of the psychological safety profile in order to provide effetive recommendations to improve psychological safety profile of offshore oil and gas production facility’s workers in PT. X. This research was conducted during March – June 2024 at 4 (four) locations using mixed methods, which were quantitative method with cross-sectional design and qualitative method with case study design. Samples for the quantitative research were 255 respondent and samples for the qualitative method were 8 (eight) informants. The result showed that most of the respondents (65,5%) have low psychological safety profile which means that most of respondents are afraid to do HSE related matters. Then, used chi square test found that associated factors of the psychological safety profile of offshore oil and gas production facility’s workers in PT. X are safety competence (OR 3,37, 95% CI 1,96 – 5,78), trust (OR 5,35, 95% CI 3,06 – 9,36), co-worker’s pressure (OR 5,27, 95% CI 3,00 – 9,26), interpersonal relationship (OR 6,05, 95% CI 3,40 – 10,75), work pressure (OR 4,88, 95% CI 2,80 – 8,50), emotional intelligence of field management (OR 7,43, 95% CI 3,97 – 13,89), policy, process and procedure (OR 6,94, 95% CI 3,91 – 12,33), superior-subordinate hierarchy emphasizing (OR 1,88, 95% CI 1,07 – 3,29). In addition, another associated factor of the psychological safety profile revealed based on the qualitative method by semi-structured interview is investigation process.
