Abstrak:
Terapi antiretroviral mampu menekan replikasi HIV, mencegah morbilitas dan mortalitas. Kepatuhan pengobatan dibutuhkan untuk mencapai kesuksesan terapi, mencegah resistensi obat antiretroviral dan risiko penularan HIV ditengah masyarakat. Penelitian ini bertujuan untuk mengetahui factor-faktor yang mempengaruhi kepatuhan pengobatan obat antiretroviral pasien HIV/AIDS di empat rumah sakit di DKI Jakarta tahun 20182019. Penelitian ini merupakan penelitian observasional dengan rancangan cross sectional dari data baseline penelitian INA-PROACTIVE (data sekunder). dimana sebanyak 666 ODHA dipilih sebagai sampel. Kepatuhan pengobatan diukur berdasarkan self report. Data dianalisa dengan menggunakan cox proportional hazard regression dengan perangkat lunak STATA12. Hasil penelitian menunjukkan proporsi kepatuhan <95% sebesar 17,9%. Analisis faktor determinan kepatuhan berobat pada penelitian ini menggunakan analisis multivariat cox regresi dan besar pengaruh dinyatakan dalam prevalensi rasio (PR) dengan confident interval (CI) 95%. Penelitian ini menunjukkan faktor sosio-demografi yang berhubungan dengan ketidakpatuhan pengobatan pada ODHA yang mendapat terapi ARV adalah variabel jenis kelamin, usia, status pernikahan dan rute transmisi HIV. Faktor klinis yang mempunyai hubungan dengan kepatuhan minum obat ARV adalah variabel adanya riwayat infeksi oportunistik sifilis dan nilai CD4. Faktor pengobatan yang mempunyai hubungan dengan kepatuhan minum obat ARV adalah variabel jenis paduan ARV dan lama pengobatan ARV. Semua variabel tersebut tidak berpengaruh signifkan secara statistik dengan nilai p value > 0,05

HIV/AIDS masih menjadi masalah kesehatan global. Cakupan pengobatan ARV di Indonesia baru mencapai 70%, di bawah target UNAIDS 95%, sehingga berpotensi meningkatkan loss to follow up (LTFU). Papua Tengah memiliki angka LTFU tertinggi (67%) dan Kepulauan Bangka Belitung terendah (20%). Penelitian ini bertujuan untuk menganalisis determinan loss to follow up pengobatan ARV pada orang ODHIV di Provinsi Papua Tengah dan Kepulauan Bangka Belitung tahun 2025. Penelitian menggunakan desain cross-sectional. Analisis data dilakukan menggunakan regresi logistik multivariat dengan pelaporan adjusted odds ratio (AOR) dan interval kepercayaan 95%. Di Papua Tengah, LTFU berhubungan dengan usia > 30 tahun (AOR = 0,648; 95% CI: 0,523-0,803), tempat tinggal luar wilayah Papua Tengah (AOR = 0,357; 95% CI: 0,216-0,589), kelompok populasi kunci (AOR = 0,421; 95% CI: 0,261-0,679) dan inisiasi pengobatan ARV > 7 hari (AOR = 0,694; 95% CI: 0,564-0,854). Di Kepulauan Bangka Belitung, LTFU berhubungan dengan asal rujukan pada ODHIV datang sendiri (AOR = 0,687; 95% CI; 0,490-0,964), kelompok populasi kunci (AOR = 0,588; CI: 0,406-0,852) dan inisiasi pengobatann ARV > 7 hari (AOR = 0,687; CI: 0,488-0,969) dan terdapat interaksi antara jenis kelamin pada kelompok populasi (AOR = 3,621; 95% CI: 1,670-7,852; p = 0,001). Penelitian ini menunjukkan bahwa determinan LTFU pengobatan ARV bersifat kontekstual. Pada LTFU tinggi dipengaruhi oleh tempat tinggal di luar wilayah Papua Tengah, sedangkan LTFU rendah dipengaruhi oleh perempuan pada populasi umum.
HIV/AIDS remains a global health problem, including in Indonesia. ARV treatment is the only primary therapy to maintain the health of people living with HIV. However, ARV treatment coverage in Indonesia is still relatively low at 70%, lower than the UNAIDS target of 95%, potentially increasing the incidence of loss to follow-up (LTFU). Central Papua has the highest LTFU rate (67%), while the Bangka Belitung Islands have the lowest rate (20%) in Indonesia. This study aims to analyze the determinants of loss to follow-up on ARV treatment in people living with HIV in Central Papua Province and the Bangka Belitung Islands in 2025. The study used a cross-sectional design. Data analysis was performed using multivariate logistic regression with reporting adjusted odds ratios (AOR) and 95% confidence intervals. In Central Papua, high LTFU was significantly associated with age > 30 years (AOR = 0.648; 95% CI: 0.523-0.803), residence outside Central Papua (AOR = 0.357; 95% CI: 0.216-0.589), key population groups (AOR = 0.421; 95% CI: 0.261-0.679) and initiation of ARV treatment > 7 days (AOR = 0.694; 95% CI: 0.564-0.854). In the Bangka Belitung Islands, LTFU was associated with the origin of referral in PLHIV coming alone (AOR = 0.687; 95% CI; 0.490-0.964), key population groups (AOR = 0.588; CI: 0.406-0.852) and ARV treatment initiation > 7 days (AOR = 0.687; CI: 0.488-0.969) and there was an interaction between gender in population groups (AOR = 3.621; 95% CI: 1.670-7.852; p = 0.001). This study demonstrated that the determinants of long-term survival (LTFU) on ARV treatment are contextual. High LTFU was influenced by residence outside of Central Papua, while low LTFU was influenced by female status in the general population.

ABSTRAK Nama : Ade Lies Oktorita Program Studi : Ilmu Kesehatan Masyarakat Judul : Pengaruh Efek Samping Obat Anti Tuberkulosis Terhadap Hasil Akhir Pengobatan Pasien Tuberkuosis di Jakarta Timur Tahun 2017 Pembimbing : Dr. Artha Prabawa, SKM, M.Si Tuberkulosis paru merupakan salah satu penyakit non-communicable disease yang paling banyak menyebabkan kematian dan kecatatan di negara berkembang. Indikator yang digunakan untuk menilai tingkat keberhasilan program pengendalian TB salah satunya adalah keberhasilan pengobatan. Pasien tidak patuh bahkan hingga mengalami default dalam menjalani pengobatan TB dikarenakan timbulnya efek samping OAT. Penelitian ini bertujuan untuk mengetahui pegaruh efek samping OAT terhadap hasil akhir pengobatan pasien TB di Jakarta Timur. Penelitian menggunakan rancangan crosssectional yang dilakukan di 10 puskesmas kecamatan wilayah kerja Suku Dinas Kesehatan Jakarta Timur pada bulan Juli-Oktober 2018. Sampel ditentukan menggunakan purposive sampling dan diperoleh sampel sejumlah 273 orang. Hasil penelitian menunjukkan bahwa pasien yang mengalami kejadian efek samping OAT mempunyai peluang untuk tidak berhasil pada pengobatan TB sebesar 1,9 kali (95% CI: 0,9-4,1) dibandingkan pasien yang tidak mengalami efek samping OAT setelah dikontrol oleh variabel keberadaan PMO. The indicator used to assess the success rate of the TB program is the success of treatment. Patient who is not compliant and even experiences default in undergoing TB treatment due to the occurrence of side effects of anti tuberculosis. This study aims to determine the relation of side effect of anti tuberculosis on the treatment result of TB patients in East Jakarta. The study used a crosssectional design carried out in 10 sub-district health centers in the East Jakarta in July- October 2018. Samples were determined using purposive sampling and obtained a sample of 273 people. The results showed that patients who experienced side effect of anti tuberculosis had a chance of not succeeding in TB treatment by 1.9 times (95% CI: 0.9- viii 4.1) compared to patients who did not experience side effect of anti tuberculosis after being controlled by presence of PMO.
