IntroductionDramatically, Anti-Retroviral drug Therapy (ART) has reduced morbidity andmortality of People Living with HIV/AIDS (PLWHA). However, adherence toantiretroviral therapy has become a challenge because this therapy must beendured for a lifetime. Adherence to antiretroviral therapy is one of the factorsthat determine the success of treatment. Poor adherence to ARV therapy inIndonesia is arround 23-55%. The objective of this study was to determine theinfluence of medication non-adherence to the 3-years survival of patients withHIV/AIDS.MethodsThis study used a retrospective cohort design at RSPI Prof. Dr. Sulianti Saroso in2010-2012.ResultsThe cumulative survival probability of patients with HIV/AIDS at RSPI Prof. dr.Sulianti Saroso in the second year (24th month) was 95.6% and the third year (inthe 36th) was 91%. Multivariate analysis with Cox regression showed the factorsthat affected the 3-years survival of patients with HIV/AIDS are non-adherence toART, after controlled by initial CD4 count (aHR = 7.608; 95% CI: 1.664 to34.790), and non-compliance appointments, after controlled by opportunisticinfection, age and initial CD4 count (aHR = 2.456; 95% CI: 0.802 to 7.518).Among patient non-adherence to ART, non-compliance appointments affected the3-years survival of patients with HIV/AIDS, after controlled by initial CD4 count,sex, CPT, modes of HIV transmission, WHO clinical stage, opportunisticinfection, and age (aHR = 4.517 ; 95%CI : 0.729-27.987).DiscussionsNonadherence to ART may caused a failure of the suppression on HIV viral, thusincrease the possibility of HIV virus mutations that can lead to drug-resistant andultimately increase the risk of death. Poor adherence to appointments of takingdrugs in the first year also assumed the poor adherence of the next assignment totake drugs in the further, and show disobedience to ART, so it will increase therisk of death.RecomendationMonitoring coverage of medication adherence of patients with HIV/AIDS in aregular basis as the early warning on the risk of death among patients withHIV/AIDS.Keyword : non-adherence to ART, appointment keeping, retrospective cohort
The World Health Organization (WHO) predicts an increase in the number of people with diabetes mellitus (DM) is one of the global health threats. The predicted increase in the number of people with diabetes in Indonesia from 8.4 million in 2000 to around 21.3 million in 2030.Diabetics and its complications bring huge economic losses to diabetics and their families, the national health system and economy through direct medical costs, lost work and income. Low adherence to medication can result in an increased risk of treatment costs, an increased risk of complications and the risk of hospitalization. Identification of the factors associated with low patient compliance with DM treatment is the aim of this study so that this study is expected to provide solutions and strategies to improve treatment adherence. This study was an observational analytic study with a cross sectional design. The data used in this study are secondary data from Riskesdas 2018 throughout Bali Province with logistic regression analysis and the size of the relationship is expressed in the prevalence odds ratio (POR) with α = 0.05. In the multivariate analysis, the final model that relates to adherence to DM treatment in Bali in 2018 is statistically namely work (POR 1.164 95% CI 1.019-1.329 p-value 0.002), residence (POR 0.864 95% CI 0.764-0.978 p-value 0.021 ), gender (POR 0.816 95% CI 0.717-0.929 p-value 0.002), and age (POR 0.779 95% CI 0.665-0.912 p-value 0.002) while the level of education was not statistically significant
Gagal jantung merupakan salah satu jenis penyakit jantung dengan insiden, prevalen serta mortalitas yang terus meningkat. Penelitian ini bertujuan untuk mengetahui pengaruh keteraturan berobat terhadap kesintasan lima tahun penderita gagal jantung kongestif (GJK). Desain penelitian adalah kohort retrospektif. Sampel sebanyak 402 orang penderita baru GJK yang didiagnosis antara tahun 2001 s.d. 2002 dan dirawat di Rumah Sakit Pusat Jantung dan Pembuluh Darah Harapan Kita. Ditemukan penderita GJK yang meninggal selama lima tahun follow up adalah 78 orang (19,4%). Probabilitas kesintasan penderita GJK adalah sebesar 88,65% (tahun pertama), 80,11%(tahun ke dua). 72.22% (tahun ke tiga), 63,75% (tahun ke empat) dan 54,41% (tahun ke lima). Penderita GJK yang tidak teratur berobat mempunyai risiko kematian lebih tinggi dari pada yang berobat teratur. Pada analisis Cox regression keteraturan berobat merupakan yariabel independen pada kesintasan penderita GJK (HR:1,95; 95% Cl: 1.23-3.11). Faktor-faktor Iain yang juga bermakna terhadap kesintasan penderita GJK adalah Ejection Fraction (HR:1,91; 95% Cl:1,18-3,08), Diabetes Melitus (HR:1,85; 95% Cl:1,08-3,18). Beberapa variabel pada penelitian ini hubungannya tidak bermakna terhadap kesintasan penderita GJK yaitu: umur, rokok,functional, riwayat PJK , hipertensi , kreatinin dan tindakan pengobatan. Keteraturan berobat terbukti mempengaruhi probabilitas kesintasan penderita GJK. Penderita GJK disarankan untuk senantiasa melakukan pemeriksaan dan pengobatan secara teratur.
Heart failure is one of cardiovascular disease which incidence, prevalence and mortality remain height and increased. The purpose of this study was to evaluate the effect of routine medical evaluation (compliance) on five year survival rate of patients hospitalized due to congestive heart failure. The Study design used in this study is retrospective cohort with 402 patients of newly diagnosis congestive heart failure (CHF) admitted in year 2000 to 2001 at National Cardiovascular Center - Harapan Kita, Jakarta. During 5 year follow-up, 78 patients died. Survival at 1 to 5 years was in order of 88,65%, 80,11%, 72,22%, 63,75%, and 54,41%, respectively. CHF patients who did not underwent routine medical evaluation had higher prognostic of death than CHF patients who had medical evaluation routinely. By Cox regression analyses, the independent predictors of mortality were routine evaluation (HR:1,95; 95% CI: 1.23-3.11). low ejection fraction (HR:1,91; 95% CI:1,18-3,08), and diabetes mellitus (HR:1,85; 95%CI:1,08-3,18). Other predictors were not statistically significant, i.e: age, gender, smoking, functional class, coronary heart disease, creatinine, and the medication. The status of compliance is an independent predictor of survival for patients with CHF, besides low ejection tiaction and diabetes mellitus. These evaluation, like the other research, suggested the importance of compliance in the treatment of CHF.
