Abstrak:
Kadmium adalah unsur toksik yang terdapat di lingkungan dan tidakdibutuhkan oleh tubuh manusia. Tujuan penelitian ini adalah untuk mengetahuihubungan konsumsi air sumur yang terpajan kadmium dengan kandungankadmium di dalam urin. Penelitian ini dilaksanakan menggunakan desain cross sectional yangdilakukan di sekitar TPA Desa Namo Bintang Kecamatan Pancur Batu KabupatenDeli Serdang. Populasi adalah seluruh laki-laki dan perempuan dewasa, dengansampel berjumlah 96 orang yang diambil dengan metode stratified randomsampling. Analisis statistik yang digunakan adalah analisis regresi logistik.Hasil penelitian menunjukkan sebanyak 63 orang (65,6%) terpajan pada airsumur dengan kandungan kadmium yang telah melebihi baku mutu dan seluruhsampel masyarakat (100%) terdapat kadmium di dalam urin yang telah melewatinilai ambang batas. Tidak ada hubungan yang signifikan antara konsumsi airsumur yang terpajan kadmium dengan kadmium di dalam urin. Pada analsiismultivariat menunjukkan bahwa masyarakat yang mengkonsumsi air sumur yangterpajan Cd lebih dari 0,005 mg/l memiliki risiko 2,657 kali terdapat kadmiumdalam urinnya setelah dikontrol dengan variabel umur, jenis kelamin, pekerjaan,kebiasaan merokok, durasi pajanan, dan jarak dari TPA. Oleh karena itu,masyarakat dianjurkan untuk tidak menggunakan air sumur sebagai sumber airminum dan diharapkan dapat dimanfaatkan oleh pemerintah Kabupaten DeliSerdang sebagai dasar untuk membuat perencanaan daerah bidang air minum dansanitasi di wilayah studi.Kata kunci : Kadmium, Kadmium Urin, Tempat Pembuangan AkhirSampah

Abstrak:
Tempat Pembuangan Akhir (TPA) sampah yang menggunakan teknik open dumping dalam pengelolaan sampahnya dinilai sangat berisiko terhadap lingkungan di sekitarnya karena adanya proses perlindian yang dapat mencemari tanah. Masyarakat yang menggunakan air tanah sebagai sumber air minumnya berisiko untuk terkena dampak kesehatan dari kandungan logam berat yang berasal dari cemaran air lindi. Kadmium dan timbal merupakan logam yang dapat bersifat toksik apabila dikonsumsi secara berlebihan. Penelitian ini menggunakan metode analisis risiko kesehatan lingkungan untuk mengestimasi risiko kesehatan akibat pajanan logam kadmium dan timbal yang terdapat pada air minum masyarakat di sekitar TPA sampah Namo Bintang. Terdapat 96 sampel individu dewasa yang diperoleh melalui data sekunder dalam penelitian ini yang berasal dari dusun IV dan V Desa Namo Bintang. Sampel lingkungan adalah sampel air sumur masyarakat yang telah diambil pada penelitian sebelumnya oleh Ashar (2013) untuk kandungan timbal dan Ashar (2013) untuk kandungan kadmium. Dari hasil perhitungan analisis risiko menunjukkan bahwa tingkat risiko pajanan kadmium dan timbal melalui air minum pada masyarakat dengan skenario intake minimal dan rata-rata tidak berisiko atau aman untuk pajanan realtime maupun lifespan (RQ < 1) sedangkan untuk skenario maksimal, pajanan kadmium dan timbal untuk pajanan realtime dan pajanan lifespan dinyatakan berisiko (RQ > 1) sehingga diperlukan manajemen risiko yaitu dengan mengurangi konsentrasi asupan dan laju asupan. Selain itu, masyarakat juga harus menjaga TPA dengan tidak membuang sampah kembali disana dikarenakan TPA tersebut sudah ditutup, dan pemerintah juga dapat membantu dengan menyuplai air, baik itu air minum maupun air bersih dari sumber lain yang aman untuk memenuhi kebutuhan masyarakat. Population that use groundwater as their drinking water source has health risk from heavy metal exposure. Cadmium and lead are toxic if they consumed in exessive ammount. This study use health environment risk analysis to estimate health risk from cadmium and lead exposure through drinking water at population around landfill Namo Bintang. There are 96 samples of secondary data from Dusun IV and V Namo Bintang village. Environment samples is well water from population households from Ashar (2013) for lead and Ashar (2016) for cadmium contains. From the calculation of risk analysis showed that risk level of cadmium and lead exposure through drinking water with minimum and mean scenario are not risky or safe for realtime and lifespan exposure (RQ < 1). For maximum intake scenario, cadmium and lead exposure are risky (RQ > 1) and need risk management with reduce consuming rate and agent concentration. Since the landfill was closed, the population also have to keep the landfill clean and do not throwing the waste into the landfill. The governance also can helping the population to fulfill their need of water with supplying water from safe and clean source. Latar Belakang: Penurunan bone mass density merupakan salah satu dampak negatif timbal. Timbal dapat ditemukan pada lindi hasil penguraian dari timbunan sampah secara open dumping. TPA Namo Bintang sudah tutup tahun 2013 tapi diperkirakan proses penguraian masih berlangsung sampai saat ini sehingga timbal dalam lindi menjadi faktor risiko untuk masyarakat yang tinggal disekitar tempat pembuangan akhir sampah tersebut. Tujuan: Untuk menganalis pengaruh timbal terhadap densitas tulang. Metode: Penelitian ini menggunakan desain cross sectional. Data yang dikumpulkan di Desa Namo Bintang Kecamatan Pancur Batu Kabupaten Deli Serdang Provinsi Sumatera Utara Indonesia berjumlah 96 orang responden yang umurnya >18. Pengumpulan data dilakukan dengan wawancara dan pengukuran. Umur, jenis kelamin, indeks massa tubuh, olahraga, kebiasaan merokok, konsumsi alkohol, frekuensi konsumsi makanan pencegah osteoporosis, konsumsi kopi dikumpulkan dengan interview sedangkan BMD diukur dengan Densitometer QUS (Quantitative Ultra Sound) dan kadar timbal diukur dengan Atomic Absorption Spectroscopy. Hasil: Sejumlah 24% responden memiliki kadar timbal tinggi, dan sebanyak 49% mengalami osteoporosis. Osteoporosis dipengaruhi oleh umur (p=0,008) dan konsumsi susu (p=0,002). Faktor yang mempengaruhi bone mass density adalah umur dan konsumsi susu, sedangkan timbal tidak berhubungan. Saran: Osteoporosis dapat cegah dengan melakukan pola hidup sehat seperti olah raga dan konsumsi makanan yang mengandung kalsium. Kata Kunci: Densitas Tulang, Timbal, Tempat Pembuangan Akhir
Background: The reduction of bone mass density is one of the negative impacts of lead. Lead can be found in decomposition of garbage piles at open dumping. TPA Namo Bintang had closed in 2013 but estimated that the decomposition process is still ongoing so that lead in leachate is a risk factor for people who lived around the landfill. Objective: To analyze the influence of lead against bone density. Method: This study used cross sectional design. Data collected in Desa Namo Bintang Kecamatan Pancur Batu Kabupaten Deli Serdang Provinsi Sumatera Utara Indonesia amounted to 96 respondents who age> 18. Data collection was done by interviewed and measurement. Age, sex, body mass index, exercise, smoking habit, alcohol consumption, frequency of osteoporotic prevention food consumption, and coffee consumption was collected by interviewed while BMD was measured by QUS Densitometer (Quantitative Ultra Sound) and Lead Blood Level measured by Atomic Absorption Spectroscopy. Results: 24% of respondents had high lead levels, and 49% had osteoporosis. Osteoporosis was affected by age (p = 0.008) and milk consumption as osteoporotic prevention food (p = 0.002). Factors affecting bone mass density were age and milk consumption, while lead was unrelated. ABSTRAK Nama : Febri Hardiyanti Program Studi : Ilmu Kesehatan Masyarakat Judul : Asosiasi Antara Kadar Merkuri dalam Rambut dan Gejala Gangguan Fungsi Ginjal Pada Masyarakat yang Tinggal di Sekitar TPA Cipayung Depok Jawa Barat 2019 Pembimbing : Dr. Ema Hemawati, S.Si., MKM Merkuri adalah zat kontaminan beracun, namun keberadaannya sampai saat ini masih terus dibuang ke perairan, tanah, dan atmosfer. Salah satu organ tubuh yang sensitif dengan jenis logam berat adalah ginjal. Salah satu tanda gejala gangguan fungsi ginjal adalah adanya proteinuria. Penelitian ini bertujuan untuk mengetahu asosiasi antara kadar merkuri dalam rambut dan kejadian gejala gangguan fungsi ginjal pada masyarakat di sekitar TPA Cipayung. Penelitian ini menggunakan desain cross sectional. Sampel penelitian adalah 98 masyarakat yang tinggal di sekitar TPA Cipayung. Data pengkajian dan merkuri dalam rambut diperoleh dari BBTKLPP Jakarta. Kadar merkuri dalam rambut diukur dengan metode EPA 2007 Methode 7473 (SW-846). Kadar proteinuria diukur dengan uji diagnostik sederhana rapid test dipstick urine. Hasil penelitian ini ditemukan asosiasi lama tinggal dengan kadar merkuri dalam rambut (OR=5,5; CI 95%: 1,1-27,4). Terdapat asosiasi antara kadar merkuri dengan gejala gangguan fungsi ginjal (OR=10,7; CI 95%: 2,1-23,8). Terdapat asosiasi antara tekanan darah dengan gejala gangguan fungsi ginjal (OR=2,9; CI 95%: 1,2-7,0). Analisis multivariat, ditemukan asosiasi antara kadar merkuri dalam rambut dengan gejala fungsi ginjal setelah dikontrol variabel tekanan darah (OR=9,403; CI 95%: 1,8- 48,9). Terkait dengan adanya pencemaran merkuri di area TPA, diharapkan pemerintah dapat membangun barrier antara area TPA dan permukiman masyarakat, serta dapat memberikan edukasi tentang pola hidup sehat. One of the organs sensitive to the type of heavy metals is kidneys. One sign of symptoms of impaired renal function is the presence of proteinuria. This research aims to do the association between mercury levels in hair and the incidence of impaired renal function symptoms in communities around Cipayung landfill. This research uses cross sectional design. The research samples were 98 people living around Cipayung landfill. Data assessment and mercury in hair is obtained from BBTKLPP Jakarta. Mercury levels in the hair were measured by method EPA 2007 Methode 7473 (SW-846). Proteinuria levels were measured with a simple diagnostic test of rapid test dipstick urine. The results of this study were found long-lived associations with mercury levels in the hair (OR = 5,5; CI 95%: 1,1-27,4). There are associations between mercury levels and symptoms of impaired renal function (OR = 10,7; CI 95%: 2,1-23,8). There is an association between blood pressure and symptoms of impaired renal function (OR = 2,9; CI 95%: 1,2-7,0). Multivariate analysis, found an association between mercury levels in the hair with symptoms of impaired renal function after a variable-controlled blood pressure (OR = 9,403; CI 95%: 1,8-48,9). Due to mercury pollution in the landfill area, the government is expected to build a barrier between landfill area and community settlements, and can provide education on healthy lifestyles. Abstrak:
Air merupakan kebutuhan esensial yang harus dipenuhi untuk kelangsungan hidupmanusia. Namun, pelayanan air bersih di wilayah Bekasi khususnya KelurahanSumur Batu, Kecamatan Bantargebang sebagian besar menggunakan sumursebagai sumber air minum dan kebutuhan sehari-hari lainnya. Mangan dan besimerupakan zat gizi esensial yang dibutuhkan oleh tubuh tetapi apabila dikonsumsidengan jumlah berlebih dapat bersifat toksik. Penelitian ini menggunakan metodeanalisis risiko kesehatan lingkungan untuk mengestimasi pajanan mangan danbesi dan tingkat risiko kesehatan yang diterima dari air minum pada pendudukdewasa di pemukiman sekitar tempat pembuangan akhir Bantargebang, KotaBekasi. Selain itu, penelitian ini juga menguji korelasi antara mangan, besi, TDS,DHL, dan pH sehingga ditemukan persamaan linear untuk memperkirakankeberadaan mangan atau besi. Populasi penelitian adalah seluruh penduduk usiadewasa yang tinggal di sekitar TPA Bantargebang. Penentuan sampel denganmenentukan kriteria inklusi yaitu penduduk usia lebih dari 18 tahun danmengonsumsi air sumur minimal 2 bulan. Pengambilan sampel dengan cara nonrandom sampling dan quota sampel. Sampel lingkungan adalah air sumur yangberada di rumah tangga terpilih sebagai responden. Selain itu, dilakukanpengambilan data antropometri berupa laju asupan, durasi pajanan, dan beratbadan. Jumlah asupan mangan dan besi pada populasi penelitian belummemenuhi angka kecukupan gizi masing-masing yaitu 0,26 dan 0,02 mg/l. Darihasil perhitungan estimasi risiko didapatkan hasil bahwa tingkat risiko mangandan besi di sekitar TPA Bantargebang tidak berisiko atau aman. Selain itu,diperoleh dari uji korelasi hubungan yang signifikan antara mangan dengan pH,DHL, dan TDS (p<0,05) sehingga ditemukan persamaan linear sederhana untukmasing-masing variabel.

Abstrak:
Latar belakang. Sampah yang menumpuk di tempat pembuangan akhir dapat mengakibatkan tercemarnya lingkungan dan berisiko terhadap kesehatan penduduk setempat. Salah satu penyebab tercemarnya adalah air lindi. Tempat Pengolahan Sampah Terpadu (TPST) Bantar Gebang terletak di Kecamatan Bantar Gebang, Bekasi, Jawa Barat. TPST Bantar Gebang mengolah air lindi di Instalasi Pengolahan Air Sampah (IPAS). Metode. Penelitian ini dilakukan secara deskriptif untuk mengetahui efisiensi pengolahan air lindi serta mengetahui kadar kadmium dan beberapa parameter lainnya. Penelitian dilakukan dengan pengambilan sampel pada inlet, outlet dan air permukaan pada hari berbeda. Penelitian ini juga ditambah data pengukuran yang dilakukan TPST Bantar Gebang. Hasil yang diperoleh dianalisis dengan baku mutu PerMenLH nomor 5 tahun 2014 dan PerMenLHK nomor 59 tahun 2016 untuk air lindi sedangkan air permukaan dengan baku mutu PP nomor 82 tahun 2001. Baku mutu air bersih mengacu pada PerMenKes nomor 416 tahun 1990. Hasil. Pada penelitian menunjukkan kadar kadmium, suhu, pH, TSS, TDS, BOD dan COD pada outlet tidak melebihi baku mutu. Namun pada air permukaan kadar BOD dan COD tinggi, hal tersebut dapat disebabkan adanya limbah industri dan rumah tangga. Tingkat efektivitas pada hari senin, TDS 87,76%; TSS 82,58%; BOD 98,28%; COD 98,24%. Pada hari rabu, TDS 77,84%; TSS 78,02%; BOD 95,61%; COD 95,92%. Pada hari jumat, TDS 85,47%; TSS 78,7%; BOD 97,43%; COD 97,58%. Hasil pengukuran oleh TPST Bantar Gebang pada September 2017 pada IPAS 1, IPAS 2 dan IPAS 3, menunjukkan ada beberapa parameter yang kadar outletnya lebih tinggi dibandingkan inlet. Hasil pengukuran oleh TPST Bantar Gebang pada Oktober 2017 pada hulu, tengah dan hilir sungai Asem dan sungai Ciketing, menunjukkan pada hulu dan hilir, parameter yang diukur kadarnya tinggi. Hasil pengukuran Bantar Gebang pada September 2017 pada air sumur masyarakat, pada beberapa titik sampel, kadar TDS dan coliform tinggi. Kesimpulan. Pengolahan air lindi dilakukan menggunakan bak ekualisasi, bak fakultatif, bak aerasi, polishing pond, bak pengendap, bak pengolahan kimia dan biologi, kolam lumpur dan sand filter. Waste which accumulates in landfills can lead to contamination of the environment and risk to the health of the local population. One cause of contamination is leachate. Integrated Waste Treatment Plant (TPST) Bantar Gebang is located in Bantar Gebang District, Bekasi, West Java. Bantargebang TPST treated leachate water in Waste Water Management Site (IPAS). Method. This research was conducted to know leachate water treatment efficiency and to know cadmium content and some key parameters. This study was conducted by sampling on inlet, outlet and surface water on different days. This study also added measurement data conducted by TPST Bantar Gebang. The results were analyzed and compared to the regulatory standards of the Minister of Environment No. 5 of 2014 and No. 59 of 2016 for leachate water, while the surface water with the quality standard of PP number 82 of 2001. The standard of clean water quality refers to the regulatory of the Minister of Health No. 416 of 1990. Result. The results showed cadmium, temperature, pH, TSS, TDS, BOD and COD at outlets do not exceed the quality standard. However, in surface water whose high of BOD and COD, it can be caused by industrial and household waste. The effectiveness level on Monday, TDS 87.76%; TSS 82.58%; BOD 98.28%; COD 98.24%. Effectiveness level on Wednesday, TDS 77.84%; TSS 78.02%; BOD 95.61%; COD 95.92%. Effectiveness level on Friday, TDS 85.47%; TSS 78.7%; BOD 97.43%; COD 97.58%. The results of Bantar Gebang measurement in September 2017 on IPAS 1, IPAS 2 and IPAS 3, indicate that there are some parameters whose outlet content is higher than inlet. The results of Bantar Gebang measurement in October 2017 on upstream, middle and downstream of Asem and Ciketing rivers show upstream and downstream, measured parameters are high. The results of Bantar Gebang measurement in September 2017 on community clean water, at some sample points, TDS and coliform levels are high. Conclusion. Leachate treatment is using equalization basin, facultative basin, aeration basin, polishing pond, sedimentation basin, chemical and biological treatment basin, mud pool and sand filter. Abstrak:
Infeksi Saluran Pernapasan Akut (ISPA) merupakan penyebab utama morbiditas dan mortalitas pada bayi dan anak-anak. ISPA bisa terjadi karena pencemaran kualitas udara di luar maupun di dalam ruangan. Salah satunya gas sulfur dioksida (SO2) yang ada di tempat pembuangan sampah dapat mengganggu sistem pernapasan pada balita. Balita lebih berisiko tertular ISPA karena kekebalan tubuh yang dialami balita belum terbentuk sempurna. Penelitian ini bertujuan untuk mengetahui kondisi udara dalam rumah dan karakteristik balita terhadap kejadian ISPA pada balita di sekitar Tempat Pembuangan Akhir Sampah Sukawinatan Kelurahan Sukajaya Palembang. Jenis penelitian analitik, desain penelitian cross sectional. Variabel terukur adalah kondisi udara dalam rumah, karakteristik balita, dan kejadian ISPA pada balita. Populasi penelitian adalah anak balita berumur 12-59 bulan yang bertempat tinggal di Kelurahan Sukajaya dan sampel berjumlah 94 orang. Data dianalisis dengan uji chi-square, t-test independent, dan regresi logistik. Period Prevalence kejadian ISPA pada balita sebesar 59,6%. Variabel penggunaan obat anti nyamuk, perokok dalam rumah, ventilasi, status gizi dan status imunisasi secara statistik menunjukkan hubungan yang bermakna terhadap kejadian ISPA pada balita, sedangkan variabel kadar SO2 dalam rumah dan umur balita secara statistik tidak menunjukkan hubungan yang bermakna terhadap kejadian ISPA pada balita. Hasil analisis multivariat diperoleh bahwa variabel ventilasi rumah merupakan variabel yang paling dominan berhubungan dengan kejadian ISPA pada balita. Ventilasi dapat menjadi faktor risiko terhadap terjadinya ISPA, karena ventilasi mempunyai fungsi sebagai sarana sirkulasi udara sehingga dapat mengurangi pencemaran udara dalam rumah. Kata Kunci : ISPA, Sulfur dioksida, Ventilasi Acute Respiratory Infections (ARI) is a major cause of morbidity and mortality in young children. ARI can occurs because indoor and outdoor air pollution. One of them is gas sulfur dioxide (SO2) in landfills that it can be irritate the respiratory tract in young children. Young children have higher risk of contracting ARI because the immune of young children not yet fully formed. This research aims to find out Indoor air Pollution and Characteristics of acute respiratory infection in under-fives in Sukawinatan Landfills. Type of research was analitic, cross-sectional study design. Measurement of indoor air pollution, characteristics of young children, and prevalence of acute respiratory infection. The population of this research was young children aged 12-59 months who lived in Kelurahan Sukajaya and 94 samples. Data were analyzed by chi-square, t-test independent, and logistic regression. Period Prevalence of acute respiratory infection in young children about 59,6%. Using mosquito repellent, smokers in the house, ventilation, nutrition and immunization status were significant correlation to acute respiratory infection in young children. While SO2 levels in the home and age of young children were insignificant correlation to acute respiratory infection in young children. Multivariate analysis showed that the variables of ventilation with SO2 levels were the most dominant variable related to acute respiratory infection in young children. One of risk factor of acute respiratory infection is ventilation, because its function as air circulation to reduce indoor air pollution.
