Citicoline is the most widely used neuroprotective to repair neurological deficit inischemic stroke patients, however the effectiveness of citicoline is stillcontroversial and raise arguments against scientific research because it providedheterogeneous results.The objectives of the study are to identify citicoline effecton neurological function improvement using The National Institute of HealthStroke Scale (NIHSS) and functional ability improvement using Barthel Index(BI) in the treatment of ischemic stroke patients at 18 hospitals involved inIndonesia stroke registry. The design of this study is retrospective cohort studyusing stroke registry data. Improvement of neurological function assessed bychanges of NIHSS score >2 and improvement of functional ability assesed bychanges of Barthel Index score > 20 as measured at the time of admission anddischarge of the hospital.The result shows that the probability of functionalneurological improvement on citicoline treatment group is higher than nociticoline treatment group with adjusted RR by neurorestoration is 1,34 (95% CI1.058 to 1.658, p=0,0014). There is no difference of functional abilityimprovement between citicoline and no citicoline treatment group, with adjustedRR by neurorestoration is 1.07 (CI95% 0.879 to 1.293; p=0,53).Keywords : Barthel Index; Citicoline; Ischemic stroke; NIHSS.
Stroke is the third disease which often cause death in Indonesia. Prevalence of stroke in Indonesia from Riskesdas 2007 is 8,3 per 1000 person. Prevalence of stroke in Jakarta is still higher then national prevalence that is 12,5 per 1000 person and cause a lot of problems both in terms of social and economic. This research aims to identify different risk factors of ischemic and hemorragic stroke in inpatient of stroke at National Cardiovascular Center Harapan Kita 2012. The study was conducted by analyzing secondary data from patient medical record by using cross sectional study. Results showed that there were patients with ischemic stroke (10,4%) and hemorrhagic (89,6) with a characteristic age ≥62 years (51,5%), male (62%), low education (17,8%), smoking behaviour (37,4%), hypertension (81,6%) and diabetes (50,3%). Based on bivariate analysis, result shows that there is no statistical difference between risk factors and incidence of stroke, but the difference proportion of risk factors in ischemic stroke always greater than hemorrhagic stroke.
Stroke merupakan penyakit penyebab kematian tertinggi kedua dan penyumbang kecatatan ketiga terbanyak di dunia. Riset Kesehatan Dasar menyebutkan bahwa prevalensi stroke di Indonesia pada tahun 2018 sangat tinggi yaitu 10,8‰ penduduk. Tujuan penelitian ini adalah untuk mengetahui pengaruh resiliensi keluarga terhadap kualitas hidup penderita stroke iskemik selama tiga bulan di Rumah Sakit Otak Dr. Drs. M. Hatta Bukittinggi. Jenis penelitian ini merupakan penelitian gabungan kuantitatif dan kualitatif dengan strategi eksplanatoris sekuensial. Jenis studi kuantitatif yang digunakan adalah studi longitudinal sedangkan penelitian kualitatif menggunakan rancangan Rapid Assessment Procedure (RAP). Sebanyak 206 sampel dianalisis secara kuantitatif. Informan yang dilibatkan dalam studi kualitatif melalui wawancara mendalam adalah keluarga dari penderita stroke iskemik sebanyak tujuh orang, tokoh adat Minangkabau sebanyak empat orang. Informan dari tenaga medis yang diikutkan dalam penelitian ini adalah kelompok terapis dan perawat masing-masing enam orang melalui metode Diskusi Kelompok Terarah. Uji yang digunakan pada analisis multivariat adalah Generalized Estimating Equations (GEE), dan analisis data kualitatif menggunakan model analisis editing. Hasil penelitian menemukan bahwa pengaruh resiliensi keluarga terhadap kualitas hidup penderita stroke iskemik paling signifikan terjadi pada bulan pertama pasca stroke (OR=3,5; 95%CI:1,7-7,2) setelah dikontrol faktor usia, suku, dan ketekunan latihan. Untuk meningkatkan kualitas hidup penderita stroke dapat dilakukan dengan meningkatkan resiliensi keluarga melalui komunikasi terapeutik saat discharge planning di rumah sakit. Selain itu, rekomendasi pengajuan kebutuhan tenaga Pekerja Sosial Medis di rumah sakit juga dinilai penting dalam rangka menangani masalah sosial pasien dan keluarga di komunitas. Diseminasi informasi juga disampaikan kepada Dinas Kesehatan Kota Bukittinggi untuk dapat menjadi input edukasi kepada keluarga penderita stroke, populasi berisiko stroke, dan masyarakat umum.
Stroke is the second highest cause of death and the third largest contributor to disability in the world. Basic Health Research states that the prevalence of stroke in Indonesia in 2018 was very high, namely 10.8‰ population. The purpose of this study was to determine the effect of family resilience on the quality of life of ischemic stroke sufferers for three months at dr. Drs. M. Hatta Bukittinggi Brain Hospital. This type of research is a combined quantitative and qualitative research with sequential explanatory strategy. The type of quantitative study used is a longitudinal study while qualitative research uses a Rapid Assessment Procedure (RAP) design. A total of 206 samples were quantitatively analyzed. The informants involved in the qualitative study through in-depth interviews were the families of seven ischemic stroke sufferers, four Minangkabau traditional figures. The informants from the medical personnel included in this study were a group of therapists and nurses of six people each through the Focus Group Discussion method. The tests used in multivariate analysis are General Estimation of Equation (GEE), and qualitative data analysis using editing analysis models. The results of the study found that there is an influence of family resilience on the quality of life of ischemic stroke sufferers (OR=3,5; 95%CI:1,7-7,2) after controlling the age, ethnicity, and perseverance of exercise factors. The effect of family resilience on the quality of life of ischemic stroke sufferers is greatest in the first month post – stroke. To improve the quality of life of stroke patients, it can be done by increasing family resilience through therapeutic communication during discharge planning in the hospital. In addition, recommendations for filing the need for Medical Social Workers in hospitals are also considered important in order to deal with social problems of patients and families in the community. Information dissemination was also conveyed to the Bukittinggi City Health Office to be an input for education to families of stroke sufferers, populations at risk of stroke, and the general public.
