Abstrak:
Tesis ini mengkaji adanya risiko pajanan Sulfur Dioksida (SO2) terhadap kesehatan para pekerja terkait dengan gejala gangguan pernapasan di Sentra Industri Keramik Plered, Kabupaten Purwakarta. Industri keramik menghasilkan SO2 yang berasal dari penggunaan bahan bakar kayu dalam proses pembakarannya. Penelitian ini menggunakan desain Public Health Assessment (PHA), dimana metode pararosanilin digunakan dalam pengambilan sampel udara pada 6 titik lokasi. Hasil pengukuran SO2 menunjukkan bahwa konsentrasi masih dibawah baku mutu yang ditetapkan, yakni rata-rata 0,042 ppm. Besar sampel dalam penelitian adalah 97 pekerja, dalam mengukur keluhan kesehatan pekerja terkait dengan gangguan pernapasan digunakan metode wawancara, dimana sebanyak 69 pekerja (71,1%) mengalami gejala gangguan pernapasan diantaranya batuk, dahak, sesak napas, mengi, nyeri dada, dan napas berat. Intake pajanan SO2 dihitung berdasarkan pada pola aktivitas dan karakteristik antropometri pekerja hingga didapatkan nilai rata-ratanya sebesar 0,0109 mg/kg/hari. Sedangkan estimasi besar risiko menyatakan bahwa sebanyak 3 pekerja (3,1%) berada pada kelompok berisiko atau tidak aman. Adapun tidak adanya perbedaan atau hubungan antara gejala gangguan pernapasan antara intake ≤ 0,0109 mg/kg/hari dengan intake > 0,0109 mg/kg/hari, meskipun pekerja dengan intake > 0,0109 mg/kg/hari berpeluang 2,2 kali lebih besar untuk mengalami gejala gangguan pernapasan dibandingkan pekerja dengan intake ≤ 0,0109 mg/kg/hari (OR=2,206; CI 95%: 0,891-5,465). Pentingnya upaya penyuluhan dalam rangka meningkatkan kesadaran para pekerja terkait penggunaan APD yang dapat didukung oleh seluruh pihak yang terkait demi menjaga dan meningkatkan produktivitas kerja

Abstrak:
Sentra industri keramik Plered berbentuk home industry yang mana proses dan teknologi yang digunakan masih sederhana sehingga emisi yang dihasilkan pun belum terlalu menjadi perhatian. Dengan demikian, jika tidak dikelola dengan baik dapat menyebabkan pencemaran udara yang dapat berisiko pada kesehatan manusia. Penelitian ini untuk menganalisis hubungan risiko kesehatan dan gangguan fungsi paru pada pekerja akibat pajanan PM10 di udara pada lingkungan sentra industri keramik Plered. Penelitian ini merupakan penelitian kuantitatif dengan desain penelitian cross sectional serta dengan pendekatan metode campuran (mix method) Analisis Risiko Kesehatan Lingkungan (ARKL) dan Epidemiologi Kesehatan Lingkungan (EKL). Pengukuran mencakup pengukuran konsentrasi PM10 menggunakan High Volume Air Sampling (HVAS) dengan metode gravimetri sesuai pedoman SNI 7119.15:2016, pengukuran berat badan dengan timbangan, pengukuran tinggi badan dengan mikrotoa, wawancara dengan kuesioner dan tes spirometri/pengukuran fungsi paru dengan spirometer. Jumlah sampel pekerja sebanyak 107 orang dan pada 30 orang sampel pekerja dilakukan tes spirometri dengan kriteria masa kerja yang terlama. Konsentrasi PM10 sebesar 0,2 mg/m 3 telah melebihi NAB sebesar 0,1 mg/m 3 sehingga terdapat risiko yang perlu dikendalikan. Namun tingkat risiko kesehatan pekerja (risk quotient/RQ) (0,008) masih rendah (RQ<1) yang mana dipengaruhi oleh rata-rata asupan pada pekerja/intake (I) yang juga masih rendah (0,02 mg/kg/hari) masih jauh dari nilai default RfC (2,42 mg/kg/hari). Hal tersebut dipengaruhi rata-rata berat badan/IMT pekerja dalam kategori normal dan walaupun konsentrasi PM10 sudah di atas NAB ternyata baru pada konsentrasi PM10 sebesar 23 mg/m 3 menghasilkan rata-rata asupan pada pekerja/intake (I) sebesar 2,46 mg/kg/hari yang mana melebihi nilai RfC sebesar 2,42 mg/kg/hari sehingga menghasilkan tingkat risiko kesehatan pekerja (risk quotient/RQ) sebesar 1,01 (RQ>1). Sementara kejadian gangguan fungsi paru (fungsi paru tidak normal) pada pekerja cukup tinggi dimana sebanyak 27 orang dari 30 orang sampel pekerja (90 %) menderita gangguan fungsi paru (fungsi paru tidak normal). Ditemukan tidak ada hubungan antara tingkat risiko kesehatan pekerja (risk quotient/RQ) dengan gangguan fungsi paru (fungsi paru tidak normal) pada pekerja (p=1,000). Kemungkinan karena RQ masih rendah, sumber pajanan lain dan faktor di luar tempat kerja. Terdapat perilaku pekerja yang sebagian besar (88 orang (82.24%) pekerja) belum menggunakan masker saat bekerja yang mana secara statistik berhubungan dengan tingkat risiko kesehatan pekerja (risk quotient/RQ) (p=0,028). Nilai RQ yang masih rendah, adanya sumber pajanan lain dan faktor di luar tempat kerja serta perilaku pekerja yang belum menggunakan masker selama bekerja mendorong perlu adanya pemantauan kesehatan lingkungan dan kesehatan kerja secara rutin serta penyuluhan dalam menumbuhkan kesadaran pribadi pekerja untuk menggunakan masker selama bekerja

This is a quantitative study with a cross sectional research design and a mixed method approach to Environmental Health Risk Analysis (ARKL) and Environmental Health Epidemiology (EKL). Measurements include measurements of PM10 concentrations using High Volume Air Sampling (HVAS) using gravimetric method according to the guidelines of SNI 7119.15:2016, measurement of body weight with scales, height measurement with microtoa, interviews with questionnaires and spirometry tests with spirometer. The number of sample workers was 107 people and the sample of 30 workers was subjected to a spirometric test with the longest working period criteria. PM10 concentration of 0.2 mg/m 3 has exceeded the NAV of 0.1 mg/m 3 so there are risks that need to be controlled. But the level of health risks of workers (risk quotient/RQ) (0.008) is still low (RQ <1) which is influenced by the average intake of workers/intake (I) which is also still low (0.02 mg/kg/day) is far from the default value of RfC (2.42 mg/kg/day). This is influenced by the average body weight/BMI of workers in the normal category and even though the PM10 concentration was above the NAV, it was found that the PM10 concentration was 23 mg/m 3 resulting in an intake (I) of 2.46 mg/kg /day which exceeds the value an RfC of 2.42 mg/kg/day, resulting in a worker's health risk level (risk quotient / RQ) of 1.01 (RQ> 1). While the incidence of lung function disorder in workers is quite high where as many as 27 people from 30 workers sample (90%) suffer from lung function disorder. There was no relationship between the level of health risks of workers (risk quotient/ RQ) with lung function disorder in workers (p = 1.000). This may be due to the low RQ, other sources of exposure and factors outside the workplace. There is the behavior of workers who most (88 people (82.24%) workers) have not used a mask when working which is statistically related to the level of health risks of workers (risk quotient/RQ) (p = 0.028). Abstrak:
Pembakaran batu bara di pembangkit listrik menghasilkan polutan yang salah satunya sulfur dioksida. Sulfur dioksida dapat menyebabkan batuk, sakit tenggorokan, mengi, sesak napas, sesak dada hingga menyebabkan edema, bronkopasme, dan pneumonitis. Penelitian ini bertujuan untuk mengestimasi besaran risiko kesehatan pada pekerja akibat pajanan konsentrasi SO2 pada pekerja di PLTU Suralaya. Penelitian ini menggunakan metode Analisis Risiko Kesehatan Lingkungan (ARKL). Sampel dalam penelitian ini sebanyak 75 pekerja yang diambil secara purposive sampling. Hasil pengukuran sulfur dioksida rata-rata di empat titik sebesar 0.0335 mg/m 3 dan masih dibawah baku mutu. Hal penelitian ini menunjukkan nilai intake dari pajanan SO2 pada pekerja didapatkan nilai rata-rata sebesar 0.00047 mg/kg/hari sedangkan nilai besaran risiko yang didapatkan sebesar 0.0187 yang artinya tingkat risiko pada pekerja masuk ke dalam kelompok aman. Hasil proyeksi terhadap tingkat risiko pada tahun ke 5 hingga tahun ke 30 mengalami peningkatan. Pekerja juga mengalami gejala gangguan pernapasan diantaranya batuk, dahak, sesak napas, mengi, nyeri dada, dan napas berat. Pentingnya upaya preventif pada pekerja di PLTU agar dapat meminimalisir pajanan SO2 dengan menggunakan APD serta pihak PLTU dapat mengembangkan teknologi modern agar meminimalisir polutan akibat pembaran batu bara

This thesis discusses the risk of SO2 exposure to the respiratory tract. SO2 exposurethrough inhalation can irritate the respiratory tract which triggers respiratory symptoms. SO2 is one of the gases emitted by the steel industry. People who live in industrial areas can be exposed to SO2 more often and experience health impacts.
This study aims to determine the level of risk and influence of SO2 exposure to respiratory tract symptomsin housewives living around the steel industry. This study uses the Environmental Health Risk Assessment (EHRA) approach to estimate the health risk in the area around the steel industry and uses the design of a cross-sectional study to illustrate the effect of intake SO2 on respiratory symptoms carried out in November - December 2019.
Samples in this research numbered 109 housewives obtained by proportional stratified random sampling. SO2 concentrations were measured at 9 points in 3 zones around the steel industry by the pararosanilin method and analyzed by spectrophotometer.
The results showed that Sukadanau Village has an RQ (Risk Quotient) ≤ 1. As many as 39,4% of housewives experience symptoms of respiratory tract disorders and obtained no influence of intake SO2 on respiratory symptoms. Recommendations that can be developed to carry out risk management in areas around the steel industry are monitoring SO2 concentrations on a regular basis so that SO2 remains at safe concentrations <0,201 mg/m3

Gangguan fungsi paru dipengaruhi oleh akumulasi pajanan polusi udara ke tubuh manusia. Stasiun Pengisian Bahan Bakar (SPBU) sebagai tempat yang padat kendaraan bermotor dengan bahan bakar yang berisiko tinggi untuk menyebabkan pajanan sulfur dioksida (SO2) terhadap petugas. Tujuan dari penelitian ini adalah mengetahui hubungan antara konsentrasi pajanan SO2 udara ambien dengan kejadian gangguan fungsi paru pada petugas SPBU di Kota Bandar Lampung Provinsi Lampung. Penelitian ini menggunakan desain cross sectional. Instrumen pada penelitian ini menggunakan kuesioner untuk wawancara, Spektrofotometer Uv-vis untuk mengukur sulfur dioksida dan spirometri untuk mengukur fungsi paru. Populasi penelitian ini adalah seluruh petugas yang bekerja di 37 SPBU. Jumlah sampel penelitian ini sebanyak 125 responden dan jumlah pengukuran SO2 adalah 30 sampel. Hasil penelitian berdasarkan uji Regresi Logistik Ganda dengan data kontinyu, diketahui bahwa ada hubungan yang signifikan antara konsentrasi SO2 udara ambien dengan gangguan fungsi paru pada petugas SPBU di Kota Bandar Lampung, dengan nilai p=0.058. Sedangkan hasil dengan data kategorik, diketahui bahwa tidak ada hubungan yang signifikan antara konsentrasi SO2 udara ambien dengan gangguan fungsi paru pada petugas SPBU di Kota Bandar Lampung, dengan nilai p=0.136 dengan dikontrol oleh lama kerja, status gizi dan masa kerja. Perlu adanya penelitian lanjutan mengenai gangguan fungsi paru dengan zat pencemar lainnya.
Impaired lung function is affected by the accumulation of air pollution exposure to the human body. Fuel Filling Stations (SPBU) as places which are densely with motorized vehicles with fuel have high risk caused by sulfur dioxide (SO2) exposure to attendants. The aim of this study is that to determine the relationship between ambient air SO2 exposure concentrations and the incidence of impaired lung function in Fuel Filling Stations (SPBU) attendants at Bandar Lampung City, Lampung Province. This study used a cross sectional design. Moreover, the instruments in this study used a questionnaire for interviews, a UV-Vis spectrophotometer to measure sulfur dioxide and spirometry to measure lung function. The populations of this study were all attendants working at 37 gas stations. In addition, the numbers of samples in this study were 125 respondents and the number of SO2 measurements was 30 samples. The result of the study based on the Multiple Logistic Regression test with continuous data shows a significant relationship between ambient air SO2 concentration and impaired lung function in Fuel Filling Stations (SPBU) in Bandar Lampung City, with a value of p = 0.058. Meanwhile, the results with categorical data shows that there is no significant relationship between ambient air SO2 concentration and impaired lung function in Fuel Filling Stations (SPBU) attendants in Bandar Lampung City with a value of p = 0.136 which is controlled by length of service, nutritional status and years of service. In addition, further research is needed regards to impaired lung function with other pollutant substances.
