Abstrak:
Sebagian besar perusahaan-perusahaan di berbagai industri yang mempunyai Sistem Manajemen Keselamatan dan Kesehatan Kerja (SMK3) mengimplementasikan Job Safety Analysis (JSA). Penelitian-penelitian merekomendasikan agar JSA harus mempunyai kualitas yang tinggi. Penyebab kecelakaan paling tinggi pada operasi pengeboran dan perawatan sumur minyak di PT XYZ berhubungan dengan kualitas JSA. Pada bulan Februari sampai Juli 2019 PT XYZ melakukan pelatihan analisis bahaya. Penelitian ini dilakukan untuk mengetahui pengaruh pelatihan yang diberikan kepada pemimpin kerja operasi perawatan sumur minyak di PT XYZ dalam meningkatkan kualitas JSA. Pelatihan dilakukan dengan jumlah peserta per sesi adalah 10-11 orang, interaktif di dalam kelas, materi merujuk pada prosedur internal PT XYZ tentang analisis bahaya, macro horizon dalam membuat JSA, dan positioning dari risk assessment dalam JSA dengan Risk Register. Peserta dilatih melakukan analisis bahaya sesuai dengan pekerjaan, termasuk mengidentifikasi bahaya, mengidentifikasi proteksi yang diperlukan, mengetahui peran dan tanggung jawab setiap pekerja, diskusi dua arah, praktik membuat JSA, dan mendiskusikannya. Data kualitas JSA diambil dengan observasi secara langsung di tempat pekerjaan dilakukan. Data kemampuan pemimpin kerja diambil dengan meminta mereka membuat JSA dari pekerjaan saat itu dengan diawasi oleh konsultan keselamatan yang ditunjuk oleh peneliti. Untuk menjaga standarisasi penilaian oleh konsultan keselamatan, tahapan yang dilakukan adalah perencanaan, pembekalan, pelaksanaan, dan evaluasi. Konsultan yang sama menilai JSA sebelum dan setelah pelatihan dengan judul sama. Pemimpin kerja yang dinilai sebelum dan setelah pelatihan adalah orang yang sama termasuk tim dan rignya. Hasil perhitungan SPC XL menunjukkan nilai-p (probabilitas kesalahan) dari Uji-t terhadap data sebelum dan setelah pelatihan sebesar 0,000 atau lebih kecil dari dari tingkat signifikansi yang ditetapkan oleh peneliti yaitu α=0,05 yang berarti hipotesis alternatif menyimpulkan terjadi perubahan mean yang signifikan. Dengan keyakinan lebih dari 99% telah terjadi perubahan mean yang signifikan karena (1-p) X 100 = 100 atau hampir 100. Skor pemimpin kerja setelah mendapatkan pelatihan menunjukkan hasil yang lebih tinggi dibandingkan dengan sebelum dilakukan. Skor rata-rata 75 orang pemimpin kerja sebelum pelatihan adalah 44,93 dan skor rata-rata setelah pelatihan mengalami kenaikan menjadi adalah 87,43.

The highest cause of incidents in drilling and workover operations of PT XYZ was related to JSA quality. From February to July 2019, PT XYZ conducted hazard analysis trainings. This study was conducted to determine the effect of training provided for workover operations leaders at PT XYZ in improving the quality of JSA. Training was conducted with 10-11 participants, interactive in classroom, referred to PT XYZ's procedures on hazard analysis, macro horizon in making JSA, and positioning of risk assessment in JSA with the Risk Register. Participants were trained to do hazard analysis in accordance with work, including identifying hazards and safeguards, knowing roles and responsibilities of each worker, two-way discussion, practice to develop JSA, and discussion. JSA quality data was taken by direct observation at work area. Ability of work leaders data was taken by asking them to make JSA from current work, supervised by a safety consultant appointed by the researcher. To maintain the standardization of assessments by safety consultants, the steps taken were planning, briefing, implementation, and evaluation. The same consultant assessed JSA before and after training with the same title. Work leaders who were assessed before and after training are the same people including team and rig. The SPC XL calculation results show the p-value (probability of error) of the t-Test on data before and after training is 0,000 or smaller than the significance level set by the researcher of α = 0.05 which means that the alternative hypothesis concludes that there is a change in the mean significantly. With a confidence level more than 99%, there is a significant change in the mean because (1-p) X 100 = 100 or almost 100. The work leader's score after training shows higher than before. Kegiatan maintenance dan operational produksi crude oil pasca pemboran merupakan salah satu aktivitas yang berpotensi terjadinya kecelakaan kerja di industri minyak dan gas bumi lepas pantai. kecelakaan yang terjadi bisa menyebabkan cidera ringan, serius dan bahkan sampai menyebabkan kematian, kerusakan peralatan dan lingkungan. Tujuan penelitian ini adalah untuk mengetahui kualitas pembuatan analisa keselamtan kerja pada pekerja di departemen Produksi dan Perawatan di anjungan lepas pantai PT X. Desain penelitian Deskriptif dengan pendekatan kualitatif dilakukan dengan cara wawancara dan observasi. Hasil penelitian menunjukkan hubungan antara pelaksanaan analisa keselamatan kerja dilapangan dengan kecelakaan kerja yang terjadi dilapangan selama periode 2013-2014. Dari hasil penelitian ini disarankan bagi perusahaan untuk melakukan pelatihan dan pelatihan penyegaran yang berhubungan dengan pelaksanaan analisa keselamatan kerja, melakukan evaluasi secara berkala oleh para pimpinan dilapangan, meningkatkan kesadaran pekerja dalam melakukan analisa keselamatan kerja dengan baik dan konsisten. Kecelakaan kerja masih menjadi isu penting di tempat kerja, termasuk di PT X, sebuah perusahaan yang bergerak di bidang telekomunikasi. Salah satu upaya preventif yang dilakukan adalah pelatihan keselamatan dan kesehatan kerja (safety training) bagi pekerja lapangan. Penelitian ini bertujuan menganalisis efektivitas safety training dalam meningkatkan pengetahuan K3 pada pekerja lapangan PT X tahun 2025. Penelitian menggunakan mix method dengan desain pre-eksperimental (One Group Pre-Test Post-Test) pada 55 peserta pelatihan. Data kuantitatif diperoleh melalui pre-test, post-test, dan kuesioner kepuasan, sedangkan data kualitatif dikumpulkan melalui observasi dan telaah dokumen. Analisis statistik dilakukan dengan paired sample t-test, sementara data kualitatif dianalisis secara deskriptif berdasarkan model evaluasi CIRO (Context, Input, Reaction, Outcome). Hasil: Hasil penelitian menunjukkan adanya peningkatan nilai rata-rata pengetahuan K3 dari 87,64 pada pre-test menjadi 92,73 pada post-test atau naik 5,81% dengan perbedaan yang signifikan (p < 0,05). Seluruh peserta (100%) mencapai nilai di atas Kriteria Ketuntasan Minimal (KKM) sebesar 70. Rata-rata kepuasan peserta terhadap pelatihan mencapai 4,14 (kategori tinggi), dengan penilaian tertinggi pada kompetensi instruktur. Evaluasi aspek konteks dan input menunjukkan bahwa pelatihan sesuai kebutuhan, difasilitasi dengan baik, dan disampaikan oleh instruktur kompeten. Secara keseluruhan, safety training dinyatakan efektif dalam meningkatkan pengetahuan K3.
Workplace accidents remain a major concern, including at PT X, a company engaged in the telecommunications sector. One preventive effort implemented is occupational health and safety (OHS) training for field workers. This study aims to analyze the effectiveness of safety training in improving OHS knowledge among field workers at PT X in 2025. A mixed-method approach was used with a pre-experimental design (One Group Pre-Test Post-Test) involving 55 training participants. Quantitative data were collected through pre-tests, post-tests, and satisfaction questionnaires, while qualitative data were gathered through observation and document review. Statistical analysis was conducted using the paired sample t-test, and qualitative data were analyzed descriptively based on the CIRO evaluation model (Context, Input, Reaction, Outcome). Results showed an increase in the average OHS knowledge score from 87.64 (pre-test) to 92.73 (post-test), representing a 5.81% improvement, with a statistically significant difference (p < 0.05). All participants (100%) achieved scores above the minimum passing criteria (≥ 70). The average satisfaction score was 4.14 (high category), with the highest rating in instructor competence. Evaluation of the context and input aspects indicated that the training was relevant, well-facilitated, and delivered by competent instructors. Overall, the safety training was effective in enhancing workers’ OHS knowledge.

Abstrak:
Proses pengemasan merupakan kegiatan utama dalam industri manufaktur yang melibatkan banyak pekerja. Interaksi antara pekerja dan mesin pengemas memiliki bahaya yaitu anggota badan terjebak antara 2 benda (terjepit) yang porsinya mencapai 50% dari jenis kecelakaan kerja yang terjadi di area pengemasan, dengan penyebab langsung adalah perilaku tidak aman. Pemantauan terhadap penerapan pengendalian risiko pada proses pengemasan merupakan faktor yang sangat berpengaruh terhadap upaya menekan potensi kecelakaan kerja. Dari hasil kajian pada proses pengemasan di PT. XYZ diperoleh hasil bahwa pengendalian dilakukan pada sisi mesin dengan penggunaan safety cover, penyediaan tombol emergency stop dan penggunaan simbol peringatan bahaya pada mesin. Pengendalian pada sisi mesin memerlukan perhatian karena dalam observasi ditemukan beberapa mesin tidak memiliki safety cover dan simbol peringatan bahaya. Pengendalian pada sisi manusia (pekerja) berupa praktik perilaku aman berupa mematikan mesin jika terjadi permasalahan pada mesin dan anjuran menerapkan prosedur pengoperasian mesin. Informasi mengenai proses pelatihan formal dalam pengoperasian mesin sangat minim diperoleh dari penelitian ini. Faktor-faktor penyebab terjadinya kecelakaan jari terjepit adalah terjadinya penumpukan kerak pada sisi horizontal sealer, dan target produksi. Pengaturan suhu sealer perlu dipertimbangkan untuk menghindari terjadinya penumpukan kerak yang berpotensi menyebabkan sachet gagal turun dan target produksi sebaiknya diimbangi dengan kinerja mesin pengemas.

Sistem Manajemen Keselamatan dan Kesehatan Kerja (SMK3) sangat penting dalam industri pengolahan susu karena dapat membantu dalam meminimalkan risiko kecelakaan kerja dan penyakit akibat kerja, sehingga dapat memastikan kesehatan dan keselamatan para pekerja. PT. ABC adalah perusahaan yang bergerak di bidang pengolahan susu dan produk turunannya. Perusahaan ini berlokasi di Bogor. PT. ABC dalam kegiatan produksinya melibatkan banyak mesin dan peralatan yang memerlukan perawatan dan pengoperasian yang baik agar tidak menimbulkan risiko kecelakaan kerja. Oleh karena itu, perusahaan ini memutuskan untuk mengembangkan aplikasi Era-K3 yang merupakan aplikasi untuk meningkatkan efektivitas keselamatan kerja di tempat kerja. Oleh sebab itu perlu diadakan penelitian mengenai efektivitas analisis implementasi aplikasi Era-K3 (keselamatan dan kesehatan kerja) pada PT. ABC untuk mengumpulkan data dan informasi yang diperlukan guna mengevaluasi apakah aplikasi secara efektif menyelesaikan berbagai masalah. Berdasarkan hasil dari analisis kinerja SMK3 sebelum implementasi aplikasi Era-K3 yang dilakukan dapat dilihat pemenuhan elemen dari analisis efektivitas sebelum implementasi PT. ABC berhasil memenuhi persyaratan sebanyak 142 elemen, dimana persentase pemenuhan secara keseluruhan sebesar 79.3 %. Setelah implementasi aplikasi Era-K3 PT. ABC berhasil memenuhi persyaratan sebanyak 174 elemen, dimana persentase pemenuhan secara keseluruhan sebesar 96.9 %. Dengan pemenuhan 96.9 % artinya PT. ABC telah memiliki peningkatan kinerja sistem manajemen K3 dari sebelumnya 142 kriteria menjadi 174 dengan presentasi kenaikan sebesar 17.9%.. Angka tersebut dapat menggambarkan bahwa terjadi peningkatan signifikan pada element Perencanaan, Review, Pengembangan Kebijakan dan Prosedur dan Pelatihan.
The Occupational Safety and Health Management System (SMK3) is very important in the dairy processing industry because it helps minimize the risk of work accidents and occupational diseases, thus ensuring the health and safety of workers. PT. ABC is a company engaged in the processing of milk and its derivative products. This company is located in Bogor. PT. ABC production activities involve many machines and equipment that require proper maintenance and operation to prevent work accidents. Therefore, the company decided to develop the Era-K3 application, which is an application to improve work safety effectiveness in the workplace. Therefore, research is needed on the effectiveness of the analysis of the implementation of the Era-K3 application (occupational safety and health) at PT. ABC to collect the necessary data and information to evaluate whether the application effectively addresses various problems. Based on the results of the performance analysis of SMK3 before the implementation of the Era-K3 application, it can be seen that the fulfillment of elements from the effectiveness analysis before the implementation, PT. ABC managed to meet the requirements of 142 elements, with an overall fulfillment percentage of 79.3%. After the implementation of the Era-K3 application, PT. ABC managed to meet the requirements of 174 elements, with an overall fulfillment percentage of 96.9%. With a fulfillment of 96.9%, it means PT. ABC has had an increase in K3 management system performance from the previous 142 criteria to 174, with an increase of 17.9%. These figures can illustrate that there has been a significant increase in elements such as Planning, Review, Policy and Procedure Development, and Training

Secara global, industri pertambangan telah lama dianggap sebagai salah satu industri paling berbahaya dengan risiko kesehatan dan keselamatan yang besar bagi para pekerjanya. Frekuensi kejadian kecelakaan kerja dapat di cegah dengan perencanaan, pelaksanaan dan pengawasan Keselamatan dan Kesehatan Kerja (K3) di tempat kerja sekaligus perilaku perilaku tenaga kerja pada khususnya dan masyarakat pada umumnya, yang mulai menyadari pentingnya penerapan norma K3 di tempat kerja. Kematangan tingkat budaya keselamatan (safety maturity level) sangat penting untuk pencegahan perilaku tidak aman, terutama berbagai sector industry dengan tingkat cedera dan kematian yang tinggi. Konsep ini bertujuan untuk membantu organisasi dalam menetapkan tingkat kematangan budaya keselamatan mereka saat ini dan mengidentifikasi tindakan yang diperlukan untuk meningkatkan budaya keselamatan mereka, sehingga diketahui dimensi dan aspek budaya keselamatan yang perlu diperbaiki . Tujuan dari penelitian ini mendesain instrumen pengukuran budaya yang sesuai dengan kondisi di lingkungan PT. XYZ dengan 10 variabel penelitian commitment & support, Leadership, policy & strategy, engagement & involvement, value, safety cost, competency & training, information & communication dan safety performance, melakukan pengukuran budaya keselamatan pada PT. XYZ berdasarkan model tingkatan kematangan budaya keselamatan, dan melihat hubungan antar dimensi budaya keselamatan . Metode yang digunakan pada penelitian ini adalah mix methode, kuantitatif dan studi literature untuk memilih dimensi budaya keselamatan yang dijadikan variable penelitian dengan jumlah sampel penelitian 123 orang. Hasil penelitian dianalisis menggunakan Structural Equation Modelling (SEM) . Hasil penelitian menunjukan penilaian terhadap safety maturity level dengan 10 (sepuluh) dimensi budaya keselamatan di PT.XYZ berada pada poin 3.56 (Compliant to Proactive), terdapat 9 (sembilan) hipotesis yang dapat diterima, terdapat hubungan yang bermakna antara Leadership (L) dengan Policy & Strategy (PS), dimensi Policy & Strategy (PS) dengan Safety Cost (SC), dimensi Commitment & Support (CS) dengan Process (P), dimensi Leadership (L) dengan Engagement & Involvement (EI), dimensi Safety Cost (SC) dengan Competency & Training (CT), dimensi Process (P) dengan Information & Communication (IC), dimensi Engagement & Involvement (EI) dengan Value (V), dimensi Competency & Training (CT) dengan Safety Performance (SP), Value (V) dengan Safety Performance (SP) dan 1 hipotesis yang tidak diterima, tidak terdapat hubungan yang bermakna antara dimensi Information & Communication (IC) dengan Safety Performance (SP).
Globally, the mining industry has long been considered one of the most dangerous industries with great health and safety risks to its workers. The frequency of work accidents can be prevented by planning, implementing and monitoring Occupational Safety and Health (K3) in the workplace as well as the behavior of workers in particular and the community in general, who are starting to realize the importance of implementing OSH norms in the workplace. Maturity level of safety culture (safety maturity level) is very important for the prevention of unsafe behavior, especially various industrial sectors with high rates of injury and death. This concept aims to assist organizations in determining the maturity level of their current safety culture and identify the actions needed to improve their safety culture, so that the dimensions and aspects of safety culture need to be improved . The purpose of this study is to design a cultural measurement instrument that is appropriate to the conditions in the PT. XYZ with 10 research variables commitment & support, Leadership, policy & strategy, engagement & involvement, value, safety cost, competency & training, information & communication and safety performance, measured safety culture at PT. XYZ is based on a safety culture maturity level model, and looks at the relationship between safety culture dimensions. The method used in this study is a mix method, quantitative and literature study to select the dimensions of safety culture which are used as research variables with research sample of 123 people. The results were analyzed using Structural Equation Modeling (SEM). The results of the study show that the assessment of the safety maturity level with 10 (ten) dimensions of safety culture at PT. Globally, the mining industry has long been considered one of the most dangerous industries with great health and safety risks to its workers. The frequency of work accidents can be prevented by planning, implementing and monitoring Occupational Safety and Health (K3) in the workplace as well as the behavior of workers in particular and the community in general, who are starting to realize the importance of implementing OSH norms in the workplace. Maturity level of safety culture (safety maturity level) is very important for the prevention of unsafe behavior, especially various industrial sectors with high rates of injury and death. This concept aims to assist organizations in determining the maturity level of their current safety culture and identify the actions needed to improve their safety culture, so that the dimensions and aspects of safety culture need to be improved . The purpose of this study is to design a cultural measurement instrument that is appropriate to the conditions in the PT. XYZ with 10 research variables commitment & support, Leadership, policy & strategy, engagement & involvement, value, safety cost, competency & training, information & communication and safety performance, measured safety culture at PT. XYZ is based on a safety culture maturity level model, and looks at the relationship between safety culture dimensions. The method used in this study is a mix method, quantitative and literature study to select the dimensions of safety culture which are used as research variables with research sample of 123 people. The results were analyzed using Structural Equation Modeling (SEM). The results of the study show that the assessment of the safety maturity level with 10 (ten) dimensions of safety culture at PT.
