ABSTRAK Nama : Egidius Ulukyanan Program Studi : Ilmu Kesehatan Masyarakat Judul : Manajemen Pengelolaan Obat Dalam Keadaan Bencana di Rumah Sakit Darurat RSUD dr.H.Ishak Umarella Provinsi Maluku Tahun 2019 Pembimbing : Dr. Dra.Dumilah Ayunigtyas, MARS Pelayanan Kefarmasian merupakan kegiatan yang bertujuan untuk mengidentifikasi, mencegah, dan menyelesaikan masalah terkait obat. Pada dasarnya pengelolaan obat di instalasi farmasi rumah sakit meliputi perencanaan, pengadaan, penerimaan, penyimpanan, pendistribusian, penghapusan dan pengendalian dimana keempat tahap ini saling terkait dan saling mempengaruhi sehingga harus terkoordinasi dengan optimal. Penelitian ini bertujuan untuk mengetahui manajemen pengelolaan obat yang dilakukan rumah sakit umum daerah dr.H.Ishk Umarella Maluku dalam keadaan bencana alam terjadi. Dengan desain penelitian kualitatif yang dilakukan dengan cara wawancara mendalam terhadap informan yang bertanggungjawab terhadap farmasi RSUD dr.H.Ishak Umarella Maluku. Hasil penelitian ini yaitu perencanaan yang dilakukan rumah sakit dalam penyediaan obat yaitu dengan menyusun sesuai formula nasional dan formula rumah sakit dengan metode konsumsi yang dilakukan oleh tim perencana yang ditetapkan oleh rumah sakit, proses pengadaan dilakukan menggunakan e-catalog, proses penerimaan obat dilakukan oleh tim penerima obat yang berkompeten dibidang farmasi, seluruh obat disimpan dalam ruangan/tenda yang menyesuaikan dengan kondisi untuk menjaga mutu dan kualitas obat dengan sistem penyusunan sesuai dengan jenis sediaan obat. Proses pendistribusian obat dilakukan dengan resep perorangan, baik pasien rawat jalan maupun pasien rawat inap. Obat yang telah melewati batas kadaluarsa akan dilakukan pemusnahan dan obat yang disimpan dikendalikan dengan stok opname untuk menghindari kekosongan obat. Kendala yang dihadapi pada saat bencana yaitu ketersediaan sarana untuk penyimpanan obat. Disarankan kepada RSUD dr.H.Ishak Umarella agar dapat melakukan manajemen pengelolaan obat sesuai dengan standar yang telah ditetapkan nasional meskipun tidak maksimal dikarenakan dalam keadaan bencana. This study aims to determine the management of drug management conducted by the regional general hospital Dr.H.Ishak Umarella Maluku in the event of a natural disaster. With a qualitative research design carried out by means of in-depth interviews with respondents who are responsible for the pharmacy Dr.H.Ishak Umarella Maluku General Hospital. The results of this study are the planning carried out by the hospital in the supply of drugs, namely by compiling in accordance with national formula and hospital formula with the consumption method carried out by the planning team prepared by the hospital, the procurement process is carried out using e-catalogs, the process of receiving drugs is carried out by the receiving team competent medicine in the pharmaceutical field, all drugs are stored in a room / tent that adjusts to the conditions to maintain the quality and quality of the drug with a drafting system according to the type of drug. The process of drug distribution is done by individual prescriptions, both outpatients and inpatients. Drugs that have passed the expiration date will be destroyed and the stored drug is controlled with the stock ofname of the drug to avoid the vacancy of the drug. The obstacle faced at the time of the disaster is the availability of facilities for drug storage. It is recommended to the RSUD Dr.H.Ishak Umarella to be able to manage the management of drugs in accordance with national standards, although not optimal because in a state of disaster. Background: Covid-19 is a new pneumonia outbreak that has caused a public health emergency, including in Indonesia. Various efforts have been made by the Indonesian government and in collaboration with local governments for the prevention and control of Covid-19. One of the efforts made by the regional government together with Forkopimda is to form a Covid-19 disaster-resilient village at the village level. Therefore, this study aims to analyze the implementation of Kampung Tangguh in efforts to prevent and control Covid-19 at Payakumbuh city. Method: This study uses a qualitative approach with a case study method. Data collection techniques are conducted through in-depth interviews, observations and document studies.And data validity efforts are carried out through triangulation of methods and sources. Result: The implementation of Kampung Tangguh Payobasung and Kubang Gajah in an effort to prevent and control Covid-19 in Payakumbuh City has been successful until October, as evidenced by the absence of positive cases in both Kampung Tangguh. However, since the implementation of Kampung Tangguh has been relaxed, especially picket activities at the post, in November until Now, an increase in positive cases has been found in both Kampung Tangguh. Besides that, in 2021, in 2021, there are several transfer of cadre roles adapted to the latest policies, especially in the success of vaccination.Cadre activities in socializing and facilitating citizens for vaccines have managed to achieve vaccine coverage above 30% in both Kampung Tangguh. Conclusion: The loose implementation of Kampung Tangguh Kubang Gajah is caused by underfunding and decreased awareness of citizens since the enactment of New Habit Adaptation. It is expected that all parties involved can collaborate to increase their respective roles in optimizing the implementation of resilient villages as an effort to prevent and control Covid-19. Furthermore, for other villages can apply / adopt the cohesiveness of cadres in cooperation, IT benefits by Kampung Tangguh Kubang Gajah and Hatinya PKK program as Food Security of Kampung Tangguh Payobasung for the prevention and control of Covid-19 in their respective villages
The dynamics of COVID-19 cases until 2022 are still ongoing worldwide and resulting high fluctuations in morbidity and mortality rates. Various explosions of cases due to Variant of Concern (VOC) have a direct impact on puskesmas as primary care. To strengthen puskesmas, a policy strategy for controlling COVID-19 is needed in the form of prevention, detect, and response, followed by optimizing the implementation of policies carried out in the field. This study aims to describe the implementation of policies at the puskesmas level in the era of the COVID-19 pandemic based on a literature review. The analysis used in this study is a literature review with the PRISMA approach. The need for literature searches using online database in the form of Google Scholar and GARUDA so that 14 included literatures are obtained. In addition, various literatures obtained in the research are in the form of journal articles, reports from health institutions, text books, and websites in English and Indonesian. The result showed that the implementation of policies at the puskesmas still found various problems and obstacles, including the making of SOPs that were not comprehensive followed by a lack of socialization related to SOPs to puskesmas officers, the limited number of surveillance officers at the puskesmas, training and socialization for tracing and surveillance were still inadequate, and limited health logistics in the form of COVID-19 therapeutic drugs such as oseltamivir, azithromycin, and favipiravir. In conclusion, the implementation of policies at puskesmas has not gone well due to the lack of optimal implementation of preventive, detect, and response policies in controlling COVID-19. Therefore, suggestions that can be given are making a comprehensive SOP for handling COVID-19, strengthening communication strategies and community engagement, planning for surveillance officer estimates, and facilitating health workers to receive training programs.
Community involvement is one of the important keys to handling public health problems, especially during a pandemic. The magnitude of the efforts made by the government needs to be balanced with the active participation of the community in the crisis response to the handling of the COVID-19 pandemic so that it can be handled quickly. Good practice in several countries with high public awareness to be actively involved in supporting the handling of COVID-19, the success rate in overcoming the impact of COVID-19 is quite high when compared to countries with citizens who are quite passive or even do not want to be involved at all (apathetic). The government has limitations, namely not being able to reach all components of people's lives. Thus, community involvement and participation will facilitate the government's task in carrying out the policies that have been made. The design of this study was quantitative with a cross sectional method to assess the correlation between the performance of Community-Based Surveillance (SBM) cadres and independent variables conducted at 11 Puskesmas in Depok City. In order to enrich the results of the discussion and implementation of the role of relevant agencies, the researcher added information sourced from in-depth interviews with key informants. The results showed that variables related to the early detection and reporting of COVID 19 by cadres, including marital status (p value = 0.0001), length of time being a cadre (p value = 0.038), training status (p value = 0.002), and professional behavior p value = 0.033). In addition, the most dominant factors influencing the performance of cadres in the early detection and reporting of COVID-19 is marital status after being controlled by other variables. The confounding variable in this study is the length of time being a cadre. Marital status had an OR of 15.34, meaning that marital status increased 15 times the performance of cadres in early detection and reporting of COVID-19 (95%CI=1.9-118.8) after being controlled by other variables with a p-value of 0.009. Furthermore, community-based surveillance training increased cadres' performance 3 times in early detection and reporting of COVID-19 (95%CI=1.3-5.05). However, when viewed from the p value, the training status gets the smallest number, namely 0.006, so it can be said that training is the most influential variable on the performance of SBM cadres in finding and reporting COVID-19 cases in Depok City.
