A reporting information system design and evaluation of a computerizedmonitoring created to replace the old system is still manual. The methodologyuses the stages in the method of the System Development Life Cycle (SDLC)starting from the stage of analysis, database design, interface design toimplementation. This study is limited only to the stage of system design. Theresults of the monitoring and evaluation system development implementationand realization of the health budget in Bengkulu Provincial Health Officeenables users to get the data allocation, realization, performance activities,and the rest of the budget. Ketidakmerataan pembangunan kesehatan tennasuk didalamnya pemerataan tenaga kesehatan dan sarana pelayanan kesehatan sudah menjadi masalah yang menahun di Kabupaten Tasikmalaya. Pengangkatan tenaga dan pembangunan sarana sejatinya harus melalui perencanaan yang matang dengan memperbatikan faktor kependudukan. wilayah dan tenaga serta fasilitas kesehatan milik pemerintah maupun swasta. Keadaan ini menjadikan masyarakat kurang akses terhadap tenaga dan fasilitas kesehatan. Untuk mendapatkan pelayanan kesehatan tidak sedikir masyarakat menempuhjarak yangjauh dan mengeluarkan uang yang banyak karena diwilayahnya sangat minim akan tenaga dan fasilitas kesehatan. Kebijakan Dinas Kesehatan terhadap pemberian izin sarana. pelayanan kesehatan swasta seperti halnya masalah diatas, tidak dilakukan atas petirnbangan Sistem infonnasi yang ada saat ini belum mampu menyediakan informasi yang akurat, efektif dan efisien dalam mcmberikan data dan infurmasl mengenai perizinan kepada pengambU kebijakan. Untuk itu diper1ukatl pengembangan sistem infonnasi yang disesuaikan dengan kebutuhan informasi perizinan sarana pelayanan kesehatan, dises-uaikan dengan ketersediaan sumber daya manusia dan peralatan pendukung yang dimiliki. Pengembangan sistem informasi dilakukan dengan mengkaji sistem yang ada saat ini kernudian mengidentifikasi permasalahan sistem informasi serta mengk:aji kebutuhan infonnasi dari para pengguna informasi dalam rangka manajemen pemecahan masaiah. Hasil kajian ini menjadi dasar dalam mendisain sistem yang baru. Hasil pengembangan sistem informasi yaitu terbangunnya prototype yang diharapkan menjadi solusi masalahan sistem informasi perizinan sehingga infonnasi yang dihasilkan dapat menjadi dasar pengambilan keputusan untuk mernecahkan masalah kesehatan. Beberapa keunggutan dari prototype yang dihasilkan antara lain adalah kemampuan prototype penghasiikan infonnasi yang dibutuhkan, kemudahan dan kecepatan dalam pengeloiaan dan penyajian data, penyajian infonnasi dalam bentuk tabei dan peta serta penggunaan basis data sehingga menghasilkan analisis yang sangat bennanfaat bagi pengambil kebijakan terutama da!am pengangkatan.
Uneven distribution of health development including health professionals and health care facilities distribution is one of health issues in Tasikmalaya District. Recruitment of health professional and development of health care facilities ideally have to be done in a good planning by taking demography, geography and professional and the existing government and private health care facilities factors Into account. The consequences is that the public have test access to health professional and facilities. order to obtain health care, they have to take a long distant and spend much money. Tasikmalaya district health office policy on licensing for private health care Current existing information system is unable to provide accurate. effective and efficient information in providing data and information considering licensing to regional autorities and head of health office, Therefor. a development of infonnation system from old system is needed that fitted health care facilities licensing information requirement, and fits the availability of existing human resource and support equipment. Development of information system was conducted by assessing the existing system and then identify lssues in information system and assess information need from information user in management of problem solving. The output is used as a basis in new system design. The purpose of this information system development was to produce a prototype that is hoped to be a solution in licensing infonnation system issue, thus produced information can be used as a basis for decision makers to solve health issues specially in distribution of professional and health care facilities. Advantages from this prototype are its ability in producing required infom1ation, simplicity, and its speed in data processing and presentation, percentation of information in tabel fonn and communicative map and data base utilization so it can be a very useful analysis for policy maker mainly in recruitment and distribution of health professional and as a consideration in giving license to private health facilities founding applicant The author hopes that the utilization of the purposed prototype is followed.
Tujuan : Tujuan dari penelitian ini adalah mengembangkan basis data di puskesmas dan Dinas Kesehatan yang bermanfaat sebagai alat bantu dalam menghasilkan informasi yang dapat dijadikan acuan dalam menentukan kebijakan, perencanaan, monitoring dan evaluasi program dalam rangka menurunkan prevalensi karies dan meningkatkan cakupan UKGS.
Metoda : Metoda yang digunakan adalah metoda pendekatan System Development Life Cicle (SDLC) dengan cara observasi dan wawancara mendalam dengan informan.
Hasil : Pengembangan sistem dimulai dari penetapan kebutuhan sistem, pemodelan sistem serta penetapan perangkat lunak dan keras yang digunakan dengan harapan dapat menghasilkan informasi dalam menyusun perencanaan, monitoring dan evaluasi program UKGS.Sistem informasi UKGS dikembangkan dengan rancangan formulir input dan tampilan output yang berisi cakupan UKGS, Indikator, grafik dan Peta sebaran tenaga Kesehatan gigi di Kabupaten Sumedang.
Kesimpulan : Pelaksanaan sistem informasi ini agar berjalan dengan baik dan berkelanjutan membutuhkan komitmen dan kebijakan yang kuat dari penentu kebijakan, aturan yang jelas tentang organisasi pelaksana, motivasi yang kuat dari pelaksana dan dukungan dana yang berkesinambungan.

Background: The result from a quick survey on dental health at Sumedang district in 2004 shows that 64 % prevalency caries affected children from the 6th grade students, DMF-T average 1.78 and PTI value at 5,2 %. However, In 2010 the Departement of Health targetting prevalency caries < 50 %. Indeks DMF-T average < 1.00 and PTI > 50%. On the evaluation in report of UKGS activities in 2003, it is said that 50% of Puskesmas did not report their UKGS activities. The reason for this was because the expected UKGS activities was not running well.
Objective : The purpose of this research is to develop information data base in Puskesmas and Dinas Kesehatan, as a result will be useful as a guide line in making policy, planning, monitoring and evaluating program in effort to reduce the presentage of prevalency of caries. In addition increase UKGS objectivities.
Methodes: The method used in this research is called development life cycle system (SDLC) by observing and extensivelly interviewing the informant.
Results: In developing a system it should begin with an implication of required model and hardware and software. This system will be used to obtain information of arranging a plan, monitoring and evaluation of UKGS program. UKGS information system is developed with the input design form and output sheet that contains UKGS covarage, indicator, graphic and distribution map of dental health personnel in Sumedang District.
Conclution: Good and continuity of system implementation need the policy and commitment of decision maker, clear regulation of system organization, good motivation of the system operator and continuity of financial support.
